October 15 - November 15 The annual LCPS Open Enrollment will be held October 15 - November 15, 2016 with benefit changes effective January 1, During this time, retirees currently participating in the Cigna Open Access Plus (OAP) plan or Point of Service (POS) plan can change their health insurance plan. No action is required or necessary of retirees participating in the Cigna Medicare Surround plan. If you would like to continue to cover the same dependents under the same plan, you do not have to do anything. Your current election will automatically continue and you and all of your covered dependents will keep the same coverage. If you do want to make a change to the plan that you and/or your dependents are enrolled in, you will need to complete a Retiree Health Insurance Enrollment/Change form for This form is available on the Employee Health, Wellness and Benefits website under the Retiree Open Enrollment page, by calling Employee Health, Wellness and Benefits and at Open Enrollment events. Call to reserve your time today! The Employee Health, Wellness and Benefits Office is open to accept paperwork during regular business hours throughout Open Enrollment. The final day of Open Enrollment, and the deadline for all submissions, is midnight, Tuesday, November 15th (for all faxes and s). For your convenience, we are extending our office hours on Tuesday, November 15, from 8:00 am - 6:00 pm. You are responsible for the timely delivery of your paperwork to Employee Health, Wellness and Benefits. Items lost or delayed in the mail will not be accepted. Keep copies of anything you submit!

2 P A G E 2 Open Enrollment - In General Making Changes We understand that most retirees will want to maintain the same health insurance coverage. If you would like to continue coverage through the same plans and cover the same dependents, you do not have to do anything; your current elections will automatically continue and you and all of your covered dependents will keep the same coverage. If you are in the OAP or POS plan and want to make a change to the plan that you are enrolled in you will need to complete a Retiree Health Insurance Enrollment/Change form. This form will be available on the Retiree Open Enrollment page of the Employee Health, Wellness and Benefits website, in the Employee Health, Wellness and Benefits office (Room 319 of the Administration Building) and at Open Enrollment events. Health Insurance Rates We have developed health insurance rates for and have presented them, on pages 6 and 7. There are two rate schedules so please be sure you are looking at the correct schedule, which is based on your retirement date. Any changes to Medicare Part B premiums payable to Medicare (for Medicare-eligible participants) will be determined by Medicare and communicated to you by Medicare. Cigna Medicare Surround Participants (Medicare-Eligible Retirees) There are no changes to the plan design of the LCPS Cigna Medicare Surround or corresponding LCPS plans for Medicare-eligible retirees. Any changes to your primary Medicare Part A or Part B cost or coverage will be communicated to you by Medicare. The 2017 List of Covered Drugs for Cigna-HealthSpring RX (formulary) will be mailed to all Medicare-eligible participants by Cigna, along with other important information about your plan. Cigna OAP and POS Participants (not Medicare-Eligible) There have been changes approved for the Cigna OAP and POS plans. Please see page 2 and page 4 for plan design changes and more information. The Express Scripts copays will remain the same. It is always possible that a medication could change formulary tiers so visit or call Express Scripts with any questions about your specific medications. On the Davis Vision plan (for POS and OAP participants) the eligibility for benefits will be based on a calendar year (rather than a rolling 12-month period). This means that each January 1, your eligibility for benefits resets. The frequency will remain the same with an eye exam and lenses once every calendar year and frames once every other calendar year. 4 P A G E 4 Open Enrollment Prescription Drug Formulary - OAP and POS participants Each year, and sometimes more frequently, the prescription drug formulary is reviewed by Express Scripts and changes may be made. This could affect the cost of or coverage for a medication in the new year. Express Scripts will send personal notifications to any participants directly affected by these changes before January 1, There ways you can verify that your medications are covered: Call Express Scripts - or use their on-line search tools by logging in to your account at If you do not have an account, choose Register Now. 5 Open Enrollment OAP and POS Plans P A G E 5 Cigna Medical Cost Estimator Did you know that where you receive a medical service or procedure can affect your out-of-pocket cost by sometimes hundreds of dollars? With the Cigna Medical Cost Estimator you can estimate your average medical costs for a specific procedure, then choose your doctor and place for care based on cost, plan and location. The Medical Cost Estimator, found on allows you to enter your procedure and then provides you with estimated out of pocket cost for providers near you. To access the Medical Cost Estimator, go to and log in to your account. If you have not established an account on MyCigna, choose Register and follow the prompts to set up a user name and password. Once on the website, choose Estimate Health Care Costs and then Health Care Professional Directory. Choose the Procedure tab and either enter the name of your procedure or choose it from Popular Medical Searches on the left side. Choose the participant who will be receiving the services and search. Once you execute the search, you will be provided with a list of physicians or facilities and the cost for each based on data that Cigna has on file. Out-of-Network Benefits Out-of-Network WILL cost you more! Through the LCPS health insurance program you have access to providers who do not partner with Cigna through your out-of-network benefit. In all cases, you will pay more to do so. There have been changes to deductibles and out-of-pocket maximums for With out-of-network providers the allowable amount is based on a fixed fee schedule similar to how Medicare reimburses. Cigna will calculate their reimbursement on this allowable amount. This sometimes results in out-of-network providers passing along additional charges to you through balance billing (a provider s way of collecting amounts not reimbursed by Cigna). Cigna s network provides a full complement of providers for all levels of care. If you are currently using out-of-network providers, we strongly encourage you to find in-network alternatives. 9 P A G E 9 Free Flu Shots Avoid the Flu - Get your Free Flu Shots LCPS employees and retirees are eligible for a free flu shot at an LCPS flu shot clinic. Flu shots will be provided by Inova HealthSource: The flu shot is inactive All shots are administered by Inova-employed Nurses Inova will accept all credit cards, cash or check as payment for services rendered to family members Family members will be charged $30.00 per shot Flu shots are available to anyone over the age of 4, including pregnant women (a doctor s note is not required) The influenza vaccines administered at the LCPS flu shot clinics this year are made to protect against the following four viruses: an A/California/7/2009 (H1N1)pdm09-like virus; an A/Hong Kong/4801/2014 (H3N2)-like virus; a B/Brisbane/60/2008-like virus. a B/Phuket/3073/2013-like virus. *Please refer to the Centers For Disease Control and Prevention website for more information: flu/keyfacts.htm. Passport must be completed by December 31, 2016 and returned to Employee Health, Wellness and Benefits no later than January 31, First, you need a Retiree Wellness Works! Passport. If you don t have one, print one from our website, call Employee Health, Wellness and Benefits or wellness@lcps.org to request one. Please specify that you need a Retiree Wellness Works! Passport. Next, complete the requirements listed on Pages 2-3 of the Wellness Handbook by December 31, 2016 (turn your passport in early so you don t forget during the holiday season!). For your convenience, you have until January 31, 2017 to turn in your completed passport, however, all requirements must be completed between the dates of January 1, December 31, Biometric Information An important reminder about your health assessment. Biometrics must be included to receive your reward! While you can receive free biometric screenings monthly through our partnership with INOVA, one of the best ways to get this information - and more - is to visit your doctor for your FREE ANNUAL PHYSICAL! Medicare-eligible participants should check the Medicare preventive visit schedule to determine the preventive care services for which they are eligible. Reporting data is critical to ensuring that we are aware of and meeting the needs of our retirees relating to wellness initiatives and disease management programs, and it is mandatory to receive your reward. When do I get my $120 reward? If you complete all required steps before December 31, 2016, and your submission is received by Employee Health, Wellness and Benefits by January 31, 2017, you will receive your reward before March 30, 2017, but no earlier than February 1, There s Still Time! The Summary of Benefits and Coverage (SBC) summarizes your coverage and provides practical cost examples. The Summary Plan document (SPD) is the official coverage document that details what is covered, how it is covered, exclusions and limitations. This document takes precedence over any summary information published by LCPS or the insurance providers. Creditable Coverage Notice Are you - or a dependent on your insurance - eligible for Medicare? If so, there are a couple things you need to know about your prescription coverage. Medicare prescription drug coverage is available to everyone with Medicare and it provides a standard level of coverage set by Medicare. The LCPS prescription drug plan, through Express Scripts, provides coverage that exceeds the standard set by Medicare. This is called Creditable Coverage and means you will not pay a penalty if you elect a Medicare drug plan later during a special enrollment period.
