Kyrene Employee Benefit Trust (KEBT) O p e n E n r o l l m e n t I s s u e Benefits Open Enrollment Window April 25 - May 6, 2016 The online enrollment begins on April 25 and will close at 4:30 pm on May 6, I n s i d e t h i s i s s u e : NO HEALTH PLAN COST INCREASES for 2016/17 School Year! 2 All employees who work 30 hours or more per week in all positions are required to log-in to the i-visions Self Service portal to complete the enrollment, even if you waive all benefit plans. No benefits roll over from the prior year. No contract yet for 2016/17? If you are eligible this year, you will need to complete this online enrollment process to ensure that you re-elect the benefit plans you wish to have for next school year. Your Enrollment Checklist Read all benefits s, they may contain specific instructions for you. Review the plans & pricing posted at Departments, Human Resource Services, Benefit Information. WOW Rally 2 Log in to ivisions to review your plans and re-enroll between April 25 May 6, Earn $250 with WOW Rally at if you have a Kyrene medical plan; complete before June 30, Medical Plan Option- Health Savings Account Voluntary Benefit Options FSA / Voya 403b/457b EAP Benefit Contacts and Plan Materials Important Enrollment Details Return any required hardcopy forms by May 6, 2016 by 4:30 pm. Print out your online confirmation statement from the ivisions system. Have a safe, healthy and relaxing summer break! KEBT has auto-enrollment. If you do not complete the required online process, you will be enrolled in the Base PPO medical plan for employee only coverage. This plan has a $5,000 annual deductible before the plan pays. Benefit Changes Snapshot Eff. 7/1 NO COST INCREASES on any medical / vision insurance plans! NEW DENTAL PLAN CIGNA! Cigna is replacing our current carrier for our two dental plan options and the premiums are decreasing slightly. If you carry a Prepaid plan check the Cigna dentist directory to be sure your dentist is in-network in the equivalent DHMO plan or choose a new dentist or the Cigna PPO plan. 2 P a g e 2 Medical Plans No Cost Increases for 2016/2017! The KEBT is pleased to announce No Cost Increases for our medical plans. Standard Medical plan ($500 annual deductible, 80% in-network coverage) Free for employee only coverage. Health Savings Account medical plan (see below for details) Free for employee only coverage. Working together KEBT and the District are able to continue high quality health plans for our employees in 16/17 Base Medical plan ($5,000 annual deductible, 80% in-network coverage) - Free for employee only coverage. Premium Medical plan (no deductible, 90% in-network coverage) will still be approximately $99 per month to buy-up for employee only coverage. You may purchase dependent coverage for your family on any of the four medical plans. Go to Departments, Human Resource Services, Benefit Information for all plan changes and details, including Summaries of Benefit Coverage (SBC s) and plan changes required to comply with the Affordable Care Act. Be sure to congratulate her on this statewide recognition that was well earned! WOW/ Kyrene School District was awarded the Good Healthkeeping Seal of Approval the highest honor by the statewide Wellness Council of AZ. With the UnitedHealthcare (UHC) wellness budget, KEBT has been able to keep our health plan costs in control, by keeping employees well. Current WOW Programs MOM, September & March Fall Health Expo annually in October Maintain Don t Gain/ WOW Biggest Loser and Comeback Kyrene Diabetes Prevention & Control UHC links those at risk with a support program. Rally is online within your account where you can earn your $250 just for making wellness a priority by taking a health survey, getting your annual wellness full physical and completing online missions. Go to Rally. Begin in May to complete Rally by June 30, Health Savings Account Medical Plan Considering this plan? Meeting Attendance is Strongly Encouraged! Health Savings Account Medical Plan KEBT will continue to offer the Health Savings Account (H.S.A.) medical plan option. Features of the plan: Free employee only coverage Lower employee premium costs for dependent coverage $1,008 deposit into a Take it With You health savings account by Kyrene over the course of the school year. Annual Employee deductible, $1,500 Annual Family deductible, $3,000 Deductible = amount you must pay out of pocket, or from the health savings account before the plan pays 80% of covered services. Interested in this lower premium cost option? Be sure to attend an enrollment meeting to more fully understand this health plan.

3 O p e n E n r o l l m e n t I s s u e P a g e 3 Vision Plan Avesis NEW Dental Carrier Cigna! restorative services. NEW DENTAL Carrier: Cigna The new dental plans offer a monthly premium price decrease. On the PPO dental plan you can see almost any dental provider. VISION Plan: We are continuing to offer our vision plan through Avesis with no premium cost increase. Avesis offers access to retail chains and independent vision providers. You may use Avesis at COSTCO*, Wal-Mart, Nationwide Vision, and Visionworks among others. The Cigna DHMO has replaced the Prepaid plan. You will need to choose a dentist from their DHMO list. The plan features a set copay for each service. The Cigna PPO dental plan is designed the same as our current PPO dental plan, with a $50 annual deductible. Services are paid at 100% preventative, 80% for fillings, etc., and 50% for major * Please be aware that COST- CO will not file the claims with Avesis for you. You will pay COSTCO directly and Avesis will reimburse you for eligible vision expenses if you submit the Avesis completed claim form. Group life is one of the best products Group Life Insurance, A Good Investment The Hartford for your money in the life insurance Did you know that as a benefits eligible employee with Kyrene School District you have a district paid life/ accidental death and dismemberment insurance policy in the coverage amount of $50,000? Kyrene pays for this policy on your behalf as part of your employee benefits package. Open enrollment is a good time to consider adding additional life insurance to your benefit plans for yourself, your spouse and children. To enroll, complete the online enrollment through ivisions. Please follow the instructions and print and complete the hardcopy enrollment application and the evidence of insurability forms. These forms can be obtained on the life insurance screen as you are enrolling. These hardcopy forms must be received by May 6th by 4:30 pm or the additional life insurance request will be denied. Policy information is posted online at Departments, Human Resource Services, Benefit Information. If you leave employment with Kyrene, you can request to convert or port your life insurance. Please notify the employee Benefits department within 30 days of your last work day to obtain the necessary forms. market. Short-term Disability Insurance Insurance for the unexpected Many of us know stories about colleagues who have had to take an unexpected leave of absence from work for an extended period of time due to an accident, illness or maternity. Unless the person has a large number of saved sick, personal or ACA hours to apply, much of the leave may be unpaid. Short -term disability (STD) is an insurance plan that can pay you a monthly sum of money up to 66 2/3% of your base earnings for your personal health related absences which exceed 7 consecutive calendar days for up to 6 months with doctor verification. If you have not elected STD insurance in the past, you may wish to consider adding this important benefit. A hardcopy application is required in addi- tion to the online enrollment. Question: Do you have enough accumulated time to cover your health related absence for up to 6 months? If the answer is no, STD insurance may be a good investment for you. TIP: STD has a pre-existing condition exclusion with a 12 month look back from the plan approval date. This means that conditions that began or were treated before the plan is approved can be denied for coverage for up to 12 months. Purchase STD insurance now before you need it!

4 Flexible Spending Account (FSA) - UnitedHealthcare A FSA allows you to set aside money from your check on a pre-tax basis to pay for medical or dependent care expenses. You are able to set aside: $2,500 for a medical FSA and $5,000 for a dependent care FSA for child or elder care. With UnitedHealthcare (UHC) administering your medical FSA, you will not have to file as many hardcopy claim forms or receipts, they can verify your medical and Rx expenses. * If you elect the Health Savings Account (H.S.A.) medical plan option, your medical expenses cannot also be reimbursed through the medical flexible spending account. If you have questions please call UHC prior to enrolling for both the H.S.A. and the FSA. Optional Additional Retirement Savings through Voya Kyrene School District, Tempe Elementary and Tempe Union formed the Tempe Tri-District in 2009 to offer a unified 403b/457b and Roth plans to employees through Voya. These plans allow you to invest on a pre-tax or post-tax basis to save money for retirement in addition to your ASRS pension savings. You can elect to set up or change deductions in these plans anytime during the year. Voya documents are posted online. To set up an account or roll over funds from a previous qualified account, contact our Voya representative Jason Smith at (623) Employee Assistance Program (EAP) Available to All Employees! You do not need to be enrolled in a Kyrene medical plan to use these services. Employee Assistance Programs (EAP) are available to all employees who may be experiencing personal, financial, or legal concerns and issues. For employees with a Kyrene medical plan, please call Care24/UHC (888) For employees who do not have a medical plan with Kyrene call (866) Remember Your Additional Hardcopy Forms Needed! Short-term Disability, Life Insurance Plan and Medical Health Savings Accounts and the Domestic Partnership coverage elections may require additional hardcopy forms to be submitted during the open enrollment period to finalize your elections. Any forms that are required in addition to the online benefit enrollment can be found at Departments, Human Resource Services, Benefit Information or in the ivisions Benefits Enrollment portal on the applicable plan page as you are enrolling. Typically forms are required for short-term disability and life insurance increases, decreases or new elections. If you are keeping these coverage levels the same as your current year elections you would not need to submit hardcopy forms, just be sure to confirm your current year elections online. Also, additional forms are required if you are enrolling for the medical plan option, Health Savings Account. While you are in the online enrollment system, please read the details on each screen to determine if you need to submit an additional form for your benefit request. Page 4

Director (480) ; Online Open Enrollment Materials Required annual plan notices, medical and all health plan Summary Plan Documents (SPD s), Certificate Books and Summary of Benefit Coverage (SBC) are posted online at: Departments, Human Resource Services, Benefit Information A very limited supply of hardcopy packets can be obtained by contacting the Benefits Department in Human Resource Services. If you need assistance please contact us! Important Enrollment Details Very Important: No benefits roll over from current year. You are required to log into ivisions and complete the enrollment to either waive/decline or elect employee benefits each year. Eligible employees who do not complete the online enrollment will be auto-enrolled into the new high deductible BASE medical plan and basic life insurance ONLY. If you are auto-enrolled, you will be unable to change this medical plan for the year unless you experience another IRS qualifying life event mid-year. If you are not sure if you should enroll, please call the Benefits Department prior to May 6th, All Elections during open enrollment are FINAL until the next Open Enrollment period, unless you experience a qualifying life event. You must inform the benefits department in writing of a qualifying event within 31 days of the event occurring. To make changes, the change must be consistent with and due to the event. Failure to notify may result in the loss of ability to make changes and certain COBRA rights. Medical Plan Comparison Tool Not sure which medical plan to elect? Compare medical options using your own health scenarios, log into the ivisions self-service portal. In ivisions Under Employee Resources, click on Benefits, then Plan Comparison tool. The user ID and password to access the Plan Comparison tool will be posted on the page as you are logging in. Premium Deductions Premium deductions for coverage may begin prior to our receiving approval from the various carriers, or depending on the benefit plan, may not go into effect until we receive approval from the carrier. If your request is denied by the carrier you will be refunded any premiums taken, or if approved you will be responsible to pay any premiums for any month of approved coverage. Incomplete Enrollments If benefit requests are made online and a required hardcopy additional form is not received in the Benefits Department by May 6th, 2016 by 4:30 pm the request will be denied. Additional forms that are received without the proper online enrollment request will also be denied. Page 5
