1 Your action is required In order to receive healthcare coverage for 2019 through the Ascend to Wholeness Healthcare Plans, you MUST choose one of these plans. If you do not choose a plan during the open enrollment period, you will not receive healthcare coverage for 2019 on the Ascend to Wholeness Healthcare Plans. Please contact the Office of Human Resources directly with any Open Enrollment or plan questions. Enrollment Period October 1, November 26, 2018 Health Plan & Insurance options Enrollment Highlights Effective January 1, 2019, depending on your 2018 engagement level, you have two health plan choices which are highly competitive in the market. These plans give you full access to wholeperson health and wellness programs to help you avoid preventable illnesses and manage preexisting medical conditions. To receive healthcare coverage for the new plan year through the Ascend to Wholeness Healthcare Plans, you MUST select a healthcare plan. If you do not select a plan during your employer s open enrollment period, you will not receive healthcare coverage. What's New in New Third-Party Administrator (TPA) We are moving from Healthscope to a new vendor called WebTPA. WebTPA will provide Member Services and process claims. You will still call the same toll-free number: WebTPA offers a number of enhancements to customer experience. o Additional dedicated team members for our group o Additional business hours. They will be open 7:00 am 9:00 pm CST o Faster processing time for claim reimbursements 2. Labcorp will be part of the Aetna Signature Administrators network. This is in addition to the Quest labs that are already in network. 3. You can start earning Activity Points for 2020 as early as September 1, 2018 until July 31, The biometric screening and wellness assessment campaign will start January 1, 2019 until July 31, Cardiac Centers of Excellence The Plan has partnered with the Cleveland Clinic to provide services for non-emergency cardiac procedures. The Cleveland Clinic is a leader in providing best outcomes for cardiac procedures. 5. Telehealth with 24/7/365 availability, you can now connect with in-network board-certified doctors without ever leaving your home. Through a partnership with Amwell, you can consult with a doctor via video or phone. Examples of available services are, behavioral health, urgent care, and lactation consulting.

3 Select Your Coverage Step 1 You will need to select your coverage by logging into your Benefits Management Dashboard on the Ascend to Wholeness website at

4 You will want to use the same login credentials that you used to log your activity points and complete your assessment in Ascend to Wholeness. However, you need to make sure you are in the Benefits Management Dashboard by following the steps above.

5 You may be asked to add a secret question and or add a phone number to your account. Select secret questions from the drop-down menu and provide answer. Example Question 1: Where did you meet your spouse/significant other? Answer: La Sierra University Then click Save Changes

6 Enter your cell phone number, then click send verification code. Step 2 After logging in, click Start Your Enrollment. November 26, 2018 DEADLINE to enroll and make changes!

7 Step 3 Complete the Tell me about yourself section. Click Continue when completed.you will be prompted to click I agree that you verify that your personal information is correct. Step 4 Complete the Tell me about yourself Family Section. Click Continue when completed.you will be prompted to click I agree that you verify that your personal information is correct.

8 Step 5 Ask EMMA - Get started with your benefits enrollment audio Step 6 Click Continue to enroll in the different benefits.

9 Step 7 Make benefit selections by clicking on the View Plan Options. You may waive benefit by clicking on the I don't want this benefit (waive). Step 8 Select a particular benefit, and then determine who will be covered under benefit. Then click Continue.

10 Health Insurance Selection Select between the Accelerate and Access plan (if applicable) by clicking Select or Waive Health if you will not be participating in the Health Plan. Step 9 - Complete Enrollment Once you have reviewed all of your selections, and clicked, I agree, and I'm finished with my enrollment, you will then click Complete Enrollment to finish. Your enrollment is complete! Return anytime before 11/26/2018 to change your elections.
