2 Payment Related Instructions for Bidders All bidders/contractors are required to procure Class-IIIB Digital Signature Certificate (DSC) with Both DSC Components i.e. Signing & Encryption to participate in the E- Tenders. Bidder should get Registered at Bidder needs to submit Bid Processing Fee charges of Rs. 2495/- (non-refundable) in the form of Demand Draft from any scheduled bank, in favour of M/s. C1 India Pvt. Ltd. payable at New Delhi for participating in the Tender. 4 ELIGIBILITY CONDITIONS FOR TENDERER 1. Following documents to be uploaded on website: - a) Photocopy of Sales Tax/VAT/Service Tax Registration No. b) Photocopy of PAN Card of Firm/Proprietor/Managing Partner/Managing Director. c) Draft / Banker's Cheque of Earnest Money Deposit. d) The tenderer must have a Registered/Branch Office in Jaipur with a functional telephone for round the clock booking of the requirements. e) The bidder should have not been blacklisted by any Centre/State Govt. agency in the past three years (Attach self-certificate). f) The taxi should be of commercial registration number and valid permit to operate in Rajasthan, Haryana and Delhi. g) Undertaking as per Annexure -I. GENERAL TERMS AND CONDITIONS OF CONTRACT: - 1. The specifications of the car/taxi to be provided should be as under: - (i) Air conditioned sedan model car like Ciaz, Etios, Swift Dzire, Verna, Honda Amaze or similar type car (ii) The Vehicle must have a commercial permit (iii) The vehicle must be in very good condition and not older than the year The car/taxi would be required for Regional Office, ESI Corporation, Jaipur from 09:00 Hours to 18:30 Hours on all working days of the month excluding Saturday, Sunday and Holidays. Normally reporting time will be started at 09:00 hrs from Regional Office, if otherwise will be informed to the agency. However, as and when required the vehicle shall have to be provided on Saturday/Sunday/Holiday for which no extra charges will be paid. 3. The contract will be awarded for a period of one year extendable by another one year on the same terms and conditions after receipt of satisfactory performance. 4. Incomplete/conditional quotation will be summarily rejected. 5. The complete quotation must be submitted online on the on or before the date as specified above and any bid received after the prescribed time limit shall not be considered. The bids will be opened on the same dates i.e. on the date as specified above in the presence of the representative of the bidders present, if any. 6. The Bid should be accompanied by an Earnest Money Deposit (EMD) of rupees as specified above in the form of Demand Draft/Pay Order from a commercial bank drawn in favour of ESI Fund A/c No.1, payable at Jaipur. In the absence of EMD, the tender shall be rejected summarily. The earnest money shall be refunded to the unsuccessful Tenderers after finalization of the contract. It will be refunded to the successful Tenderer on receipt of performance security deposit. No interest is payable on the EMD. 8. Payment will be released within a month on receipt of bill and after certification by an authorised officer of ESIC that the services provided during the month is satisfactory. 9. The liability on account of fuel, driver and all expenses relating to maintenance, insurance, fire, loss/damages of property or life of passenger or third party etc. would, solely and wholly, be the responsibility of the contractor. Corporation will not bear any liability apart from the hiring charges. Only parking and toll taxes will be payable separately on production of receipts on actual basis. 10. The rate quoted by the vendor shall inclusive of fuel, oil, repair and maintenance, all taxes, fines, insurance, other legal dues, driver's salaries and payments. However, service tax, if applicable, will be reimbursed on actual basis on production of documentary evidences. 11. Extra Kilometers beyond 2000 kms per month and Extra Time beyond 9 ½ hrs per day will be paid as per accepted rate. 12. The calculation of mileage should be from the reporting point to the relieving point and will not be calculated from garage to garage basis. 13. The car/taxi deputed should carry all relevant papers duly updated and should fulfill the norms prescribed by the appropriate Govt. Department. The car/taxi should be properly and comprehensively insured and should carry necessary permit/clearance from the Transport Authority and/or any other Authority including pollution under control certificates. 14. The driver deputed on duty should not be involved in more than two punch or challan by transport / traffic authorities for negligence driving. 15. The contractor / agency should ensure that they have arrangement for establishing contact and round the clock service. The drivers employed should carry mobile and valid Driving License. 6 16. The contract between the Corporation and Contractor can be canceled with a notice period of one month from either side, without assigning any reason. 17. The Regional Director, Regional Office, Employees' State Insurance Corporation, Jaipur reserves the right to accept/reject any bid and to cancel the bidding process at any time and reject all bids at any time prior to placement of order, without thereby incurring any liability. 18. The agency/contractor shall bear the expenditure for challan by traffic police caused due to lack of statutory document of the vehicle ESIC will not be liable for such expenditure. Compliance of all taxes, dues, Labour Laws as per existing/prevalent Govt. Rules and Regulations applicable will be the responsibility of the Contractor/Agency. 19. TDS shall be deducted from the monthly bill of the Agency as per Govt. of India Rules. 20. The monthly bill shall be submitted by the Service Provider in triplicate duly supported by the copy of logbook and mark one copy for payment and other two as duplicate. 21. The driver should maintain a proper log book, which can be verified and signed by the Branch Officer (General Branch) or his nominee from time to time. The Driver should wear proper neat and clean dress. His behaviour with the officials must be polite. 22. Bidders who are willing to offer new vehicle may also bid. In case such a bidder is successful, he shall provide the vehicle within 14 days from the date of award failing which his EMD shall be forfeited. 23. No tenderer will be allowed to withdraw after submission of the tenders within the bid validity period of 90 days from date of opening of tender otherwise the EMD submitted by tenderer would stand forfeited. 24. In case the successful Tenderer declines the offer of Contract, for whatsoever reasons(s), his EMD will be forfeited. 25. The successful bidder shall have to pay Security Deposit as specified above, payable through Demand Draft/ Bankers Cheque in favour of ESIC Fund Account No.1 payable at Jaipur. I, the undersigned, hereby declare and affirm that I have gone through the terms and conditions mentioned in the tender document and undertake to comply with all the terms and conditions. 2. That the rates quoted by me are valid and binding upon me for the entire period of contract. 3. That the earnest money of Rs.../- deposited by me vide Banker Cheque /Demand Draft No. dated. is attached herewith. 4. That I/We authorize The Regional Director to forfeit the security deposited money by me/us, if any delay or failure to provide vehicle to the satisfaction of the E.S.I. Corporation within the stipulated time. 5. That I will be in a position to provide contract as per work explained to me to the satisfaction of the E.S.I Corporation. 6. That there is no vigilance/cbi case or court case pending against me/my firm debarring me/my firm to undertake contract work/supply of items quoted. 7. That I hereby undertake to carry out the work as he has been explained to me to the satisfaction of E.S.I. Corporation within the stipulated period. 8. I have been informed that the Regional Director has the right to accept or reject any or all the tenders without assigning any reason thereof. 9.
