4 Educational Tours, Inc PO Box 257 Holt, MI Friday, March 22, :00 PM: Three 56 passenger motorcoaches (maximum seats available to group are 166) arrives at Kenston High School, 9500 Bainbridge Rd., Chagrin Falls, OH ( ) for loading. NOTE: In keeping with Educational Tours, Inc. s commitment to safety and security, we prepare each itinerary in accordance with the DOT rules and regulations, giving drivers a minimum of 9 hours off each night and a maximum of 15 hours on duty during any 24-hour period. We reserve the right to revise the itinerary due to unforeseen circumstances such as traffic, road closures, site closures, weather, etc. Your Tour Manager will consult with the Group Leader and drivers on any changes. Midnight: Depart from Kenston High School on first class, air-conditioned, DVD equipped MOTORCOACHES provided by PRECIOUS CARGO ( / ). (7 hours have been allotted for the drive; plus one hour for rest stop on your own enroute)

5 Saturday, March 23, :00 AM: Arrive in New Jersey; meet your TOUR MANAGER at the DOUBLETREE NEWARK, 128 Frontage Rd., Newark, NJ ( ) for CHANGING ROOMS and BUFFET BREAKFAST 9:30 AM: Depart by coach for your workshop 10:30 AM: 12:00 PM: Participate in a BROADWAY CLASSROOM S BROADWAY 101 WORKSHOP Depart by coach for Rockefeller Center 12:30 PM: Arrive for lunch on your own at Rockefeller Center Explore Fifth Avenue Home to some of the best-known stores and buildings in the world including Tiffany's, Saks Fifth Avenue, the Lego Store, the NBC Studio Store, the Plaza Hotel and more. Also view St. Patrick's Cathedral One of the largest churches in the U.S. Twin spires 300-feet-high grace the 13th Century Gothic-style structure. ($5 pp donation is suggested at your own expense if you wish to enter the cathedral)

7 Sunday, March 24, :30 AM: BUFFET BREAKFAST at the hotel 9:30 AM: Depart by coach for the cruise 10:30 AM: Arrive at South Street Seaport and board your ONE HOUR ZEPHYR STATUE OF LIBERTY EXPRESS CRUISE Climb aboard this fun express ride to view the Statue of Liberty from the unique perspective of the waterfront and learn more about New York s historic skyline - all narrated by knowledgeable and entertaining guides! (11am cruise time, subject to availability) 12:00 PM: Walk to Brookfield Place 12:30 PM: Arrive for lunch on your own at Brookfield Place 1:45 PM: Meet at the 9/11 MEMORIAL MUSEUM- This memorial museum tells the individual stories of the 2,977 people killed in the 9/11 terrorist attacks at the World Trade Center, the Pentagon, and on Flight 93, as well as the six people who perished in the February 26, 1993 World Trade Center bombing. The historical exhibition presents the story of those attacks and particularly, the events as they unfolded on and after 9/11. Also visit the 9/11 Memorial- Experience a moving tribute of remembrance and honor to the victims of 9/11 in this beautiful open-air plaza.

Strawberry Fields In Central Park, across the street from the famous Dakota House, where legendary Beatle John Lennon once lived. 11:30 AM: Walk to Times Square 12:00 PM: Arrive at Times Square The heart of the Theater District; where New Yorkers celebrate New Year's Eve. View the skyscrapers and glittering marquees. Lunch on your own 3:00 PM: Depart for home. (7.5 hours have been allotted for the drive; plus 1.5 hours for rest stop/dinner on your own enroute.)

10 TRIP NAME: KENSTON HIGH SCHOOL BAND AND CHOIR TRIP NUMBER: TRIP PARTICIPANT INFORMATION FOR INDIVIDUAL PAYMENTS Payments made individually may be made in the following ways: 1. Personal check 2. Credit or Debit card with MasterCard, Visa, or American Express 3. Money order or cashier s check All payments are NON-REFUNDABLE and NON-TRANSFERABLE after 10/19/18. Therefore, Travel Protection will be purchased on your behalf, and includes Cancel for Any Reason (CFAR), Accident and Sickness, Trip Interruption, and Travel Delay Protection. This plan follows Educational Tours, Inc. s cancellation policy. Details of this plan are available from your Group Leader. The total cost of the trip is estimated to be $ per participant at this time for quad occupancy. Up to 12 adults/chaperones upgraded to double rooms at quad occupancy rate.

13 01. Register yourself with your billing address for your Visa, MasterCard, or American Express, and password. Save. *This is the billing address for the card & the receipt. If this does not match your issuing bank address your payment will not process. 02. Log in using your & your password that you just created. 03. Add your Trip Number (format example: ) to add your trip. You will then see the number of the trip and the name of the trip. 04. Add as many participants as you are paying for. At the end of each participants name you will see a pay button. Click this. The next screen will be where you fill in the amount of the trip payment. Please note that if you enter your name only with no payment you will be put on the Waiting List. Making a scheduled payment amount will enroll your name on the Official Trip Roster. You will need to refresh the screen after making the first payment to change from Waiting List to Traveling. (Refresh by either changing to a different screen and then go back to this screen to update it or use the F5 button at the top of your keyboard to refresh.)

14 01. Fill in the amount of your travel protection payment in the Travel Protection Amount field. If travel protection has already been purchased or provided on your behalf, leave 0.00 in the field. Fill in the amount of your trip payment in the Trip Payment field. 02. Enter your Visa, MasterCard or American Express number, expiration date and the 3 or 4-digit security number (last number on the backside of your card). Click Submit. You will see a green Thank you and receive an confirmation for that purchase. If you see a Sorry, your payment cannot be processed notice, it is either due to a typing error or the issuing card company rejecting it. It is not ETI or the website determining the rejection. Please retype and submit again to see if you had typing errors or check with your issuing card company to resolve the problem. A confirmation will state this too for reference. (Refer to #01 above.) 03. Log out until next time when you will log in with your & password. If you have forgotten your password the link to recover that is in the lower left-hand corner of the Log in Screen. You will receive an with a link to reset your password. For your security ETI does not have access to your password. 01. REMEMBER, all payments are non-refundable and non-transferable after 10/19/18.
