2 Williamsburg, Yorktown & Jamestown Itinerary May 30, 2018 Wednesday 6:00AM Bus arrives at North Brandywine Middle School 6:15AM Depart for Williamsburg 12:00PM Lunch at the Jamestown Café (included) 1:00PM Jamestown Settlement Guided Tour 3:30PM Visit Historic Jamestown 5:00PM Arrival at the Williamsburg Woodlands Hotel Treat your hotel room with respect and keep it clean while you use it. Remember you will be held responsible for any damage caused to your room. All pay-per-view channels will be turned off. Outgoing phone calls will not be permitted however you will be able to receive incoming calls. Please contact us and we will be happy to set up a plan that fits your budget Cancellation Policy & Trip Protection: If you wish to cancel your tour, you must provide written notice to Grand View Tours, Inc. If you cancel WITHIN 90 DAYS BEFORE your tour departs, you will incur a $150 fee IN ADDITION to any monies that may have been advanced on your behalf and do not cover a tour cost increase due to your cancellation. Once your final payment is received, your payment is nonrefundable. Trip protection is available for an additional $15 or $22.50 per person depending on the coverage you choose. This information will be provided with your child s confirmation and may be purchased up to the final payment. It is also available at Conduct of Tour Participants: Good conduct is the responsibility of everyone. Students must remain with their chaperons and are responsible for maintaining appropriate behavior. All buildings, museums and tours reserve the right to dismiss any group or individual who is disruptive. No refunds will be given if a person or group is asked to leave an attraction or any part of the tour early because of misconduct. Student is required to abide by the Student Code of Conduct for School Trips included with this contract. Damages: Any damage to the buses, hotel or attractions caused by anyone on the tour becomes the responsibility of that person and must be paid prior to the end of the tour. Gratuities: Gratuities for the included meals, bus driver and tour guides are included in your tour cost. Liability of Grand View Tours, Inc.: Grand View Tours, Inc. acts solely as an agent for arranging transportation, hotel accommodations and other services and does not assume any liability for injury, loss, expenses, accidents or delay due to occurrences outside its control including, but not limited to, an act of God, war, riot, sovereign conduct, or conduct of third parties. Grand View Tours, Inc. reserves the right to revise the tour itinerary/program or cancel a tour if circumstances warrant it. If changes are necessary, they will be kept to a minimum. In the unlikely event, it is necessary for Grand View Tours, Inc. to cancel a tour it shall have no liability beyond the refund of any funds paid. Grand View Tours, Inc. will not guarantee arrival at or departure from any point at a specific time due to circumstances beyond its control such as traffic, accident, breakdown, road conditions, storms or acts of God. Grand View Tours, Inc. cannot guarantee the operation of any electrical amenities such as the PA system, radio and DVD player on a coach or school bus that we charter.

5 STUDENT CODE OF CONDUCT FOR SCHOOL TRIPS SCOTT, NORTH & SOUTH BRANDYWINE MIDDLE SCHOOLS WILLIAMSBURG TRIP MAY 30 to JUNE 1, 2018 Your school-sponsored trip is planned with an educational objective. The success of this trip depends upon the conduct of all students who participate. Participation in a school trip is a privilege. It should be noted that attendance is voluntary, not mandatory, and, as such, the student agrees to abide by the rules and regulations or forfeit his/her personal rights to participate in the trip. In order that everyone receives the maximum benefit from the participation on this trip, the Student Code of Conduct must be adhered to at all times. There are standards for behavior that students are expected to uphold at all times. These include: 1. All scheduled tour activities are obligatory. 2. Respecting public and private property. 3. Obeying al policies of Coatesville Area School District and Grand View Tours, Inc. for the duration of the trip. 4. Conducting oneself in a courteous and respectful manner at all times. 5. Remaining in the presence of adult supervision at all times, unless explicit consent has been given by an adult supervisor. 6. Following directions and instructions of chaperones without fail. 7. ALCOHOL, DRUGS & TOBACCO USE IS PROHIBTED. Understanding that choosing to bring, consume, or be in the presence of others who are consuming will lead to immediate termination of the trip privilege. Consequences will be applied regardless of whether the student has brought or consumed alcohol, tobacco or drugs, or has associate with, or is in the presence of others who are consuming drugs, alcohol or tobacco. 8. Shoplifting, possession of illegal drugs and all other illegal activities will not be tolerated and are punishable by immediate dismissal from the tour. If your child is involved in any illegal activities, all costs to return home are at his or her expense. If the local authorities are involved, your child will be subject to the laws of the country he or she is visiting. 9. Acquiring a physician s written order to carry and take any prescription medication. 10. Adhering to trip dress code as set forth by your school. 11. Abiding by the curfew established. 12. Spending each night in the assigned room. a. No student may leave his/her room for any reasons after lights are out without a chaperone s permission and presence. The only exception to this rule is when all hotel guests are to respond immediately to a fire alarm or other emergency situation. b. No student shall be in another student s room when the other student is of the opposite gender, unless a chaperone is present. Page 1 of 2

6 13. Consenting to a luggage check by appropriate school staff prior to leaving for the field trip and upon return. 14. Allowing to a search of the room and personal belongings at any time deemed necessary by the appropriate school staff based on reasonable suspicion. 15. Hitchhiking and the driving or renting of any motor vehicle is strictly forbidden for all travelers. 16. Your child is required to pay for any phone calls or incidental personal expenses incurred at hotels. These will be payable the evening before departure at each hotel. Compensation for damage done to hotel rooms or to buses is your child s responsibility. If your child notices any damage upon arrival at a hotel, he or she should notify the group leader immediately. I agree that if for any reason I am in violation of the rules of the school trip, I may be brought before the appropriate school personnel for disciplinary action. I further agree to accept the penalty imposed on me, with the understanding that all such actions will be explained to me. I further realize that the severity of the penalty may increase with the severity of the violation, even to the extent of being sent home immediately at my own expense or being arrested by law enforcement officers. I understand the behavior expectations and the consequences of violating the Student Code of Conduct. I have read this code and agree to comply with all of the rules and regulations outlined by the staff supervisor. 8 DETAILS OF COVERAGE Restrictions apply - see Plan Documents for complete coverage details. TRAVEL PROTECTION TRIP CANCELLATION/TRIP INTERRUPTION Provides reimbursement up to your full, prepaid, non-refundable trip cost when you are forced to cancel or interrupt due to: Unforeseen sickness, accidental injury or death, which occurs before departure. (Certain exclusions apply) Being Hijacked, quarantined, or having to serve on a jury or appear as a witness in court; Fire, flood, burglary or other Natural Disaster at your Primary Place of Residence or Destination; A documented theft of passports or visas; Being directly involved in a traffic accident while en route to Your scheduled point of departure; Bankruptcy or Default of an airline or cruise line Strike that causes a complete stop of services for at least 18 consecutive hours Inclement Weather that causes a complete stop of services for at least 18 consecutive hours Being the victim of a Felonious assault within 10 days of the Scheduled Departure Date; A Terrorist Incident* Your host being unable to accommodate you due to their death or life threatening illness/injury Trip Delay Reimburses up to $150 per day when you are delayed en route to or from the covered Trip for 6 or more hours. Covered expenses include: Prepaid, unused, non-refundable land and water accommodations. Local transportation to join the Trip. Reasonable additional expenses incurred for meals and lodging. Emergency Medical Evacuation/Repatriation: Up to $100,000 to transport you to nearest treatment by U.S. standards and return you home when able to fly. All transportation must be authorized and arranged by OnCall International. Non-Insurance Worldwide Emergency Assistance Services The non-insurance Travel Assistance feature provides a variety of travel related services. Some of the services offered include: Medical or legal referral Hospital admission guarantee Translation service Lost Baggage retrieval Inoculation information Passport / visa information Emergency cash advance Prescription drug / eyeglass replacement Bail bond * Terrorist Incident must occur within 30 days of Your Scheduled Departure Date in a city listed on the itinerary of Your Trip. This same city must not have experienced a Terrorist Incident within the 90 days prior to the Terrorist Incident that is causing Your cancellation of Your Trip. Benefits are not provided if the Travel Supplier offers a substitute itinerary; GENERAL LIMITATIONS AND EXCLUSIONS Insurance benefits are not payable for any loss due to, arising or resulting from: 1. suicide, attempted suicide or any intentionally self- inflicted injury of You, a Traveling Companion, Family Member or Business Partner booked to travel with You, while sane or insane; 2. an act of declared or undeclared war; 3. participating in maneuvers or training exercises of an armed service, except while participating in weekend or summer training for the reserve forces of the United States, including the National Guard; 4. riding or driving in races, or speed or endurance competitions or events; 5. mountaineering (engaging in the sport of scaling mountains generally requiring the use of picks, ropes, or other special equipment); 6. participating as a member of a team in an organized sporting competition(does not apply to Trip Cancellation); 7. participating in bodily contact sports, skydiving or parachuting, hang gliding or bungee cord jumping; 8. piloting or learning to pilot or acting as a member of the crew of any aircraft; 9. being Intoxicated, or under the influence of any controlled substance unless as administered or prescribed by a Legally Qualified Physician; 10. the commission of or attempt to commit a felony or being engaged in an illegal occupation; 11. normal childbirth or pregnancy (except Complications of Pregnancy) or voluntarily induced abortion; 12. dental treatment (except as coverage is otherwise specifically provided); 13. amounts which exceed the Maximum Benefit Amount for each coverage as shown in the Schedule of Benefits; 14. due to a Pre-Existing Condition, as defined in the Policy. The Pre-Existing Condition Limitation does not apply to the Emergency Medical Evacuation or return of remains coverage; 15. medical treatment during or arising from a Trip undertaken for the purpose or intent of securing medical treatment; 16. a mental or nervous condition, unless hospitalized for that condition while the Policy is in effect for You; 17. due to loss or damage (including death or injury) and any associated cost or expense resulting directly from the discharge, explosion or use of any device, weapon or material employing or involving chemical, biological, radiological or similar agents, whether in time of peace or war, and regardless of who commits the act and regardless of any other sequence thereto. The following limitation applies to Trip Cancellation: All cancellations must be reported directly to the Travel Supplier within 72 hours of the event causing the need to cancel, unless the event prevents it, and then as soon as is reasonably possible. If the cancellation is not reported within the specified 72 hour period, the Company will not pay for additional charges, which would not have, been incurred had You notified the Travel Supplier in the specified period. If the event prevents You from reporting the cancellation, the 72-hour notice requirement does not apply; however, You must, if requested, provide proof that said event prevented him or her from reporting the cancellation within the specified period. Cancel For Any Reason Protection: Optional Coverage applies only when requested on the application and the appropriate additional plan cost has been paid. CFAR must be purchased at the time of plan purchase and with or prior to your final trip payment. If You purchase the Cancel For Any Reason protection and You cancel Your Trip for any reason not otherwise covered by this plan, the Insurer will reimburse You for up to 75% of the prepaid, forfeited, non-refundable payments or deposits You paid for Your Trip provided You cancel Your Trip more than 48 hours prior to Your Scheduled Departure Date.
