Blackman High School Choral Boosters shall act only as agent for the group in making arrangements for hotels, transportation, restaurants, or any other service, therefore, shall not be responsible for breach of contract, failure to comply with any laws such as the Americans with Disabilities Act (ADA), or any intentional or negligent actions or omissions on the part of such suppliers, and does not assume any liability whatsoever for any injury, damage, death, loss, accident or delay to person or property due to an act of negligence of, or default of, any hotel, carrier, restaurant, company or person rendering any of the services included in the tour, or by act of God. Further, no responsibilities are accepted for any damage or delay due to terrorism, sickness, pilferage, labor disputes, machinery breakdown, quarantine, government restraints, weather or other causes beyond their personal control. No responsibility is accepted for any additional expense, omissions, delays, re-routing or acts of any government or authority. Blackman High School Choral Boosters is not responsible for lost or stolen baggage, or other personal items brought on the trip. MEDICAL CONSENT FORM It is the parents responsibility to provide a Student Medical Consent Form giving authorized school personnel permission to act in a medical emergency for the duration of this trip. The student s parents / guardians are to complete the school s Student Medical Consent Form and return it to the school. It is understood that Blackman High School Choral Boosters does not have the authority to administer, provide, or assist in first aid or medical care. CHANGES TO A TOUR The right is reserved to withdraw or add any inclusion and/or make such changes as may be found desirable for the convenience of the parties & the proper carrying out of the tour. INCREASE IN YOUR COSTS The costs of any additional fuel surcharges/taxes imposed by the government, airlines, cruise lines, or bus companies will be the responsibility of the each individual and/or each group. All monies received for travel services are forwarded to the supplier, except for the agent s commission. CANCELLATION Any Cancellation at any time is subject to charges by the suppliers and vendors: to include tickets, transportation, accommodations, and any expense incurred in relationship to your trip. The group is responsible for all collection and charges. TRAVELER RESPONSIBILITY By embarking upon his/her travel, with or without signing and returning this form, the traveler, minor traveler, and parents/legal guardians of minor traveler voluntarily assumes all risks involved in such travel, whether expected or unexpected. In the case of a minor traveler, the minor traveler and his/her parents/legal guardian voluntarily assumes all risks involved in such travel, whether expected or unexpected. Traveler, minor traveler, and parents/legal guardians of minor traveler are hereby notified of the above stipulations, as well as, assuming risk for terrorism, civil unrest, war, act of war, medical, or climatic disruptions, and the possibility traveler may be unable to travel as scheduled because of personal situations. Traveler, minor traveler, and parents/legal guardians of minor traveler are responsible for any damages to the aircraft, bus, hotel, boat, or any transportation, accommodations, and facilities used in relationship to this trip. Any student or adult creating or exhibiting disruptive behavior may be removed from this trip and will not receive a refund of any type. 6 Blackman High School Chorus CODE OF CONDUCT PUNCTUALITY Students are expected to be on time for each event and the departure of each event. PARTICIPATION Students are expected to be prepared to participate in each event unless otherwise approved by the teacher and escort. RESPECT FOR SELF AND OTHERS Students are expected to be honest, behave with dignity, and treat others with respect and courtesy. Behavior of the individual should not interfere with the rights of others. This includes the use of appropriate language, actions, and attire. Students are expected not to harass others verbally or physically. Students are expected remain free from the influence of tobacco products, alcohol, or drugs. Students are expected not to use or possess such substances. RESPECT FOR AUTHORITY Students are expected to comply with all school rules and to obey all laws and any other rules established by the escort, teacher, or chaperones. Students are expected to respond in a respectful manner to all adults. Any violation of these rules may result in disciplinary action, which included but is not limited to sending the participant home at the parents expense. Chaperones are present at each site visited, but at selected sites participants may have the freedom to explore on their own. It is important for each student to exhibit responsible behavior. RESPECT FOR PROPERTY Students are expected to treat all property including motor coaches, hotels rooms, and all other property that is in contact with the students with care. FREEDOM FROM FEAR Students are expected to contribute to a safe trip environment free from fear.
