1 JMJ PILGRIMS, CORP. Terms and Conditions YOUR CONTRACT IS WITH JMJ PILGRIMS, CORP. The registration conditions, information and terms in this literature form the basis of our contract with you, the participant. In these registration conditions, you and your means all persons named on the registration, or any of them, as applicable. We, us and our means JMJ Pilgrims, Corp. OUR PRICES. The published price includes: Roundtrip airfare between Chicago, Illinois and Medjugorje. Land transportation and taxes Room accommodations provided by visionary Mirjana 2 Meals a day provided by host family Pilgrimage led by English speaking tour guide Miki Musa or other coordinated by Miki when he is not available Daily Mass in English Scheduled meeting with Mirjana Soldo and other visionaries (when available) International evening Mass Service at St. James Church Confession available daily Daily adoration and healing services Opportunity for Mirjana's apparition (schedule dependent) Opportunity to climb Apparition Hill & Cross Mountain Opportunity to visit Community Cenacolo and Mother s Village The published price does not include: Additional fare from your local airport to and from Chicago Travel insurance Items of a personal nature such as telephone calls, internet, laundry Beverages or alcohol Optional activities or side trips Excess baggage fees and passport fees. REGISTRATION AND CONFIRMATION. Registration is confirmed upon receipt of a $ deposit per person and the completed registration form. Please review the confirmation information and any additional information in relation to your registration as soon as you receive it. Contact us immediately if any information appears to be incorrect or incomplete as it may not be possible to make changes later. PAYMENT. Once you have received your confirmation of registration, the balance of payment due must be received by us 60 days prior to departure. Payment may be made by check or money order. If registration is received within 60 days of departure, full payment must be made on your receipt of the confirmation. 2 are entitled to assume that you wish to cancel your registration. In this case, we will be entitled to keep all deposits paid or due at that date. Documents are not guaranteed if final payment is not received on time! If expediting of documents is required due to late registration or payment, the passenger will be liable for extra costs incurred for rush delivery of documents. All documents will be sent approximately three weeks prior to your departure. Reservations made after price guarantee deadline will be charged an increased rate due to airfare increases. You may pay by check or money order. YOUR CONTRACT. A binding contract between us comes into existence when we send our confirmation to you. We both agree that Illinois law will govern pursuant to the arbitration provision below. SPECIAL REQUESTS AND MEDICAL CONDITIONS / DISABILITIES. We cannot accept any booking which is specified to be conditional on the fulfillment of a particular request. All such bookings will be treated as standard bookings unless otherwise agreed to in writing. We cannot guarantee special facilities or assistance for disabled passengers, and it is your responsibility to arrange for such assistance or special needs you may require. We are always willing to provide assistance and information whenever possible. JMJ Pilgrims, Corp. is not responsible for any medical condition that occurs prior, during or after the pilgrimage. If you are undergoing medical treatment you must obtain written permission to travel from your doctor. Any certificates you may have should be taken with you along with your medication and should be carried in your carry-on luggage. INSURANCE. We consider adequate travel insurance to be essential and every person travelling with us is recommended to purchase travel insurance which is available separately. PASSPORTS. A valid US passport is required for US citizens. If you do not have a valid passport please contact your closest passport office. No visa is required for travel to Croatia and Bosnia & Herzegovina for US and Canadian citizens. Cancellation due to lack of proper documents is the responsibility of the passenger and subject to penalty. If you are not a US citizen, please consult your local Embassy or Consulate for visa requirements. It is your responsibility to ensure that you are in possession of all necessary travel and health documents before departure. All costs incurred in obtaining such documentation must be paid by you. We regret we cannot accept any liability if you are refused entry onto any transport or into any country due to failure on your part to carry all required documentation. If failure to have any necessary travel or other documents results in fines, surcharges or other financial penalty being imposed on us, you will be responsible for reimbursing us accordingly. 3 FLIGHTS. Flight times are outside our control. They are set by airlines and are subject to various factors including weather conditions, potential technical problems and the ability of passengers to check in on time. Any change in the identity of the carrier or flight times will not entitle you to cancel or change to other arrangements without paying our normal charges except where specified in these conditions. In the unlikely event of a delay, we cannot be held liable for any costs incurred or inconvenience suffered. BAGGAGE. Free baggage allowance is one checked piece to which the total sum of the dimensions (length + width + height) does not exceed 62 inches for check-in. You may have one 22 inch carry-on (22"x 14" x 9"). JMJ Pilgrims, Corp. is not responsible for the loss, theft, or delay of participant s belongings. CANCELLATION BY YOU. If you wish to cancel or revise a reservation (a revision being any change whatsoever in the reservation as booked) it must be made in writing. The following charges will be assessed after payment of any account: AIRFARE: Once tickets have been issued, they cannot be canceled as specified in the ticketing agreement with the carrier. LANDCOSTS: Cancel 31 days prior to departure - full refund less $ cancellation fee. Cancel 30 days or less prior to departure - NO REFUND! PILGRIMAGE PRICES: Prices are subject to change due to fluctuation in foreign exchange rates and changes in airfare. After full payment is received, JMJ Pilgrims Corp. will guarantee the pilgrimage price as quoted. No change of names or substitutions are permitted. No refund or cancellation fees will be waived due to death or sickness. SPECIAL NOTICE. We reserve the right to ask any passenger who behaves inappropriately to leave. We cannot guarantee in these circumstances to organize the passenger s return to their joining point. We reserve the right to pass on to the passenger any costs incurred by JMJ Pilgrims, Corp. should inappropriate behavior result in such costs. RESPONSIBILITY CLAUSE. JMJ Pilgrims, Corp., its staff, members, successors, agents and assigns (collectively JMJ Pilgrims Corp. ) do not own or operate any entity which is providing goods or services for your pilgrimage, including, for example, lodging accommodations, transportation companies, sightseeing establishments, food service providers, etc. As a result, JMJ Pilgrims, Corp. is not liable for any negligent or willful act or failure to act of any such person, or of any third party not under its control. Without limiting the foregoing, JMJ Pilgrims, Corp. shall not be responsible for any injuries, death, damage, delay, or loss caused to participants as a result of social or labor unrest, mechanical or construction difficulties, lodging, airline, or other overbooking, health or disease problems, terrorist or criminal activity of any kind, acts of God, weather, local laws or regulations, or any other actions and/or omissions outside JMJ Pilgrims Corp. s direct control. Participants voluntarily assume all such risks. JMJ Pilgrims, Corp. - Page 3

4 JMJ Pilgrims, Corp., acts as the pilgrimage operator. You acknowledge that you are aware that political instability or regional conflicts occur in some areas traveled to. JMJ Pilgrims, Corp. cannot be held responsible for injuries or damages resulting from such instability or conflicts. The suppliers providing transporation and hotel accommodations for the pilgrimage are independent contractors and are not agents or employees of JMJ Pilgrims, Corp. By utilizing the service of the suppliers, you agree that JMJ Pilgrims, Corp. will not be liable for any accident, loss, injury, or damage to you or those traveling with you in connection with any accommodations, transportation or any other services resulting directly or indirectly from any occurrences or conditions beyond its control including: defects in vehicles, breakdown in equipment, thefts, delay or cancellation of or changes in itinerary for any act, omission, or event during the time you are on the vehicle or aircraft. JMJ Pilgrims, Corp. is not responsible for the late arrival of passengers at airports or lodging due to inclement weather, nor are they to be held liable for payment or any refund for unused services. JMJ Pilgrims, Corp. reserves the right to make changes in the itinerary or cancel a pilgrimage at any time whenever the organization deems it necessary for the comfort, convenience and safety of the participants. COMPULSORY ARBITRATION: Any dispute concerning this contract, the brochure, or any other advertising material concerning this program, or the program itself, will be resolved exclusively by arbitration in Illinois pursuant to the then current rules of the American Arbitration Association. Illinois substantive law will apply to all issues concerning any such dispute.
