1 SPIRIT QUEST TOURS BOOKING TERMS & CONDITIONS Please note: Each guest needs to INITIAL AND SIGN this contract FAX to (877) OR scan and to OR mail to the address below. You may take pictures of each page with your smartphone and , but please make sure ALL pages are legible. PAYMENTS $4,995 per person covers the complete land package as indicated in the online itinerary; Please call for air quotes or book your own air. Main tour is based on a minimum of 20 persons. A payment plan is available. A non-refundable deposit of $1,000 holds your space. All payments are due no later than December 4, 2015 (60 days in advance). If you sign up after that day, (space permitting) payment is due in full at time of booking. Cost is based on double occupancy; single room supplement is $1,533 per person. ROOMMATES/SINGLE SUPPLEMENTS Single rooms are available upon payment of a single room supplement of $1,533 per person. Spirit Quest will provide roommates wherever possible, upon request. If no roommate is available when tour closes, guest will be required to pay a single supplement, so guests are strongly encouraged to sign up with their own roommates. 1 of 4

2 TOUR PRICE INCLUDES Hotel Accommodation - 10 nights room accommodations are provided at the hotels indicated on this itinerary (or similar, depending on availability when deposits are received). Accommodations are per person, based on two persons sharing a twinbedded room or king room with private bath at the Melia, twin-bedded suite or king suite with private bath at the Komaneka Tours and Sightseeing - by private, air-conditioned car, as specified in itinerary All admissions and entrance fees to places visited as per the itinerary Local, English-speaking guides Private airport transfers and transfers between hotels All breakfasts (10), 5 lunches, and 4 dinners (including Welcome dinner and Farewell dinner) Taxes - all local taxes and hotel taxes of 21% are included in the cost of the tour TOUR PRICE DOES NOT INCLUDE International Airfare Gratuities to local guides and drivers of $180 (to be paid along with your final payment) Documentation costs (if any) Entrance and exit visa charges of $45 total (paid at airport individually) Travel insurance - highly recommended Excess baggage charges Beverages (with exceptions noted) Any tours and activities not specifically included, laundry, or any other item of a personal nature not specifically mentioned herein. $ admin fee (if paying by credit card) 2 of 4

3 CANCELLATION DEPOSIT IS NON-REFUNDABLE IN THE CASE OF CANCELLATION, EXCEPT AS NOTED. GUESTS CANCELING PRIOR TO JUNE 4, 2015 RECEIVE A FULL REFUND LESS THE INITIAL $1,000 DEPOSIT; PRIOR TO DECEMBER 4, 2015 RECEIVE A 50% REFUND OF ALL MONIES ABOVE THE INITIAL DEPOSIT, ON OR AFTER DECEMBER 4, 2015, NO REFUND WE STRONGLY ENCOURAGE ALL TRAVELERS TO BUY TRIP INSURANCE IN CASE OF CANCELLATION. TRAVEL DOCUMENTS All travelers need a passport valid for at least six months AFTER the date of entering the country. Please note that this rule is enforced by Indonesia. Holders of American Passports are required to obtain visas to visit Indonesia. Visas will be obtained at the airport upon arrival for US $25 per person, which you must carry in cash. You will also be required to purchase an exit visa on departure, for US$15 per person. These costs are subject to change without notice, per Indonesian Government regulations. TRAVEL INSURANCE Spirit Quest Tours offers travel insurance through our partner SquareMouth, which gives you unparalleled access to a variety of different travel insurance options. Please check with your airline for specifics on the size and weight of your allowed baggage. RESPONSIBILITY Spirit Quest Tours has made arrangements for the tour services described in this brochure. Hotels, airlines, and other suppliers providing tour services are independent contractors and are not the agents, employees or servants of, or joint venturers with Spirit Quest Tours. All certificates and other travel documents for tour services issued by us or our booking agent, Exo Travel/Exotissimo, are subject to the terms and conditions specified by the supplier and the laws of the countries is which the services are supplied. 3 of 4

4 By agreeing to utilize the services of Spirit Quest Tours, the tour participant agrees that neither Spirit Quest Tours, Exo Travel/Exotissimo, Mary Morrissey, Howard Caesar, Life Soulutions That Work, LLC, Unity of Houston, nor their affiliates nor its or their employees or agents (collectively, The Parties ), shall be liable for any delay, inconvenience, loss of employment, upset, disappointment, distress or frustration, whether physical or mental, resulting from the act or omission of The Parties, or shall be liable for any damage, loss, claim, cost or expense arising out of personal injury, accident, or death, loss, damage or delay of baggage or other property, resulting from: 1. the act or omission on the part of any person other than The Parties; 2. acts of God, sickness, theft, labor disputes, mechanical break-down, government actions, weather, or any cause beyond The Parties direct control. No person other than Mary Morrissey, Howard Caesar, or a representative of Spirit Quest Tours or Exo Travel/Exotissimo, authorized by a document in writing, may vary, add or waive any term or condition in this brochure, including any term or condition set forth in the preceding provisions. Written notice of any claim against The Parties must be received no later than fourteen (14) days after the date that services were provided to a tour participant. Although every effort will be made to deliver the trip exactly as in the itinerary, all terms and conditions, including fares, schedules, locations and rates are subject to change without notice. Quality and class will remain the same. By signing this Agreement, I agree to accept and abide by all the terms and conditions outlined above.
