Arrival: Sunday, June 30 th arrival at Lisbon airport, after going through customs and collecting your luggage you will go to the bus for transportation to your accommodation evening: team leaders meeting. Participation at the IberCup Estoril Participation at the IberCup. Jamor Sports Complex located 5 miles from Lisbon center. Fields and accommodations near the beach. Approx teams. 25+ different countries. Accommodations IberCup Estoril Hotel/Full board - daily breakfast, lunch and dinner is included. Accommodation in a 3-star hotel located in the center of the village of Estoril, less than a mile from the beach and 15 miles from Lisbon. Rooms (and some apartments) for 3 people include private bathroom, central A/C, phone, TV, hairdryer, and Wi-Fi. The hotel includes a restaurant/cafeteria, lounge area, and swimming pool. Breakfast is served in the hotel, lunch and dinner at the tournament center. Single and doubles are available as well; $ supplement per person per stay sharing a double and $ supplement per person per stay staying in a single room. 2 Participation at the San Marino Cup 2019 International Tour Itinerary 100 international youth soccer teams. 18 Countries, 4 Continents and more than 3000 participants. Matches played on 12 grass and astroturf fields in Rimini, Riccione (Italian Riviera) and the San Marino Republic. Accommodation San Marino Cup Hotel accommodation in a 3-star (Italian classification) hotel in or around Rimini. We offer rooms for 1, 2, 3 and 4 people. Daily breakfast and dinner is included. Front desk, bar, lounge, restaurant and outdoor pool. All rooms are equipped with a private bathroom with shower, TV, desk and a chair. Participant prices are based on 3 or 4 people to a room. Supplement $ per person per stay for a single room, $ per person per stay for sharing a double. Return flight to Chicago Christine Richardson Memorial Fund Earlier this year Christine Richardson passed away. Christine was one of the initiators of the first Sockers F.C. International Tour in 1984 to the Holland Cup. In her memory Premier International Tours will donate $1, total to one of the 2019 participants. The management of Sockers F.C. will determine which player will receive this scholarship. Age categories Sockers F.C. will enter teams in the boy s categories born in 2003, 2004 and Ground transportation During the San Marino Cup we have included a private bus for your group. This bus will arrange airport transfers, transfers from the hotel to the fields and is available for excursions. Price per person - US$3, The participant prices is based on a minimum of 35 people traveling. We reserve the right to increase the prices if we don t reach the 35 travelers. Parents - Sockers F.C. welcomes parents and family members as well to join. The same participant prices will apply. We have only limited space available.

4 General release form I hereby acknowledge that participation in international travel and soccer carries a potential hazard. I therefore release Sockers F.C. Chicago, its staff and chaperones, Premier International Tours its staff and chaperons of all liability in the event of injury during the 2019 international tour. We understand that the Sockers F.C. 6 Tour conditions General conditions apply to all services offered by L&J Group DBA Premier International Tours (L&J) and the Sockers F.C. Chicago. In these conditions Sockers F.C. Chicago and L&J include its offices, its branches abroad and agents, local representatives and assistants. Application An application will be binding after the confirmation of acceptance by L&J. Upon confirmation of acceptance, all arrangements are under proviso of availability. L&J withholds the right to substitute at any moment the housing accommodation, and/or to alternate the program for circumstances which are beyond control. The substitute housing accommodation, and/or the alternate program, should meet the same criteria as established in the confirmation of acceptance. Tour payments US$ non-refundable deposit per person upon booking. The remaining amount will be paid according to the schedule as mentioned in the information. L&J reserves the right to void the booking in case of serious delay of payment. In this case, L&J will refund the received payments minus the cancellation fees + deposit. Cancellation All cancellations must be received in writing. The day that the notification is received by L&J is the effective cancellation date. If any individual (or the entire group) cancels their tour anytime after booking the following cancellation fees will be charged: If a person cancels after paying $ , the cancellation fee will be $ and the other $ will be refunded if the Chicago finds a replacement. If a person cancels after paying $ 1,000.00, the cancelation fee will be $ and the other $ will be refunded if the Chicago finds a replacement. Suppliers present these regulations to L&J/PIT. This policy is in place to protect all members in the group. If your total number of participants changes due to additions/ deletions your participant s price may be adjusted accordingly. In case L&J/PIT uses a supplier with a different cancellation policy we will mention the different conditions on the tour confirmation. Insurance The general conditions of the insurance company apply. Passport All participants must be able to present a valid passport. The fee for the passport is not included in the tour costs.

7 Exclusion from participation L&J and Sockers F.C. Chicago withhold the right to exclude any participant from further participation due to seriously bad behavior. There will be no refund for the unused portion of the tour. Serious bad behavior includes, but is not limited to violation of regulations of the tour management, housing accommodations and transportation companies. If any participant is found to be responsible for any damage while on the tour, he/she will be required to pay immediately for such damages. Sports program L&J acts as an intermediary and will prepare the desired sports program with great care. Neither L&J nor its vendors accept any responsibility for the omission or partial omission of the program due to circumstances beyond control, such as, but not limited to, bad weather conditions, power failure, etc. Rate of exchange and participant prices The tour costs are based on the exchange rate of the US$ towards the Euro as of July, Fluctuations of the exchange rate can affect the tour costs. We reserve the right to increase the participant s prices in case of an increase in airport (security) taxes and/or fuel surcharges for the airlines. Responsibility and liability The responsibility of L&J and Sockers F.C. Chicago and program hosts is limited. L&J acts in the capacity of an agent for the participants in all matters pertaining to the tours, such as, but not limited to, the arrangement of transportation, housing accommodation, meals, sight-seeing, other social and cultural activities, and sport programs. L&J, Sockers F.C. Chicago and the program hosts are not responsible for any personal injury, illness or death, for any loss, damage or theft of belongings (such as baggage, possessions or money), for any error, accident, delay, deviation or curtailment of any kind caused directly or indirectly by any actions, omissions, acts of neglect of failure or facilities by: a) Any person or company retained for the benefit of the participant, including, but not limited to: housing accommodation, sports organizations, transportation companies and tour guides; b) Acts of God, acts of public enemy, arrest or restraint by any government, seizure under legal process, quarantine restrictions, riots or civil commotions, strikes, lockouts or labor stoppages, war hazards or dangers incident to the state of war; c) Any defect or failure to comply with any part of local rules or regulations of any airplane, automobile, motor coach, ship, train or other vehicles; d) Improper or insufficient passports or other documents; e) Participation in program activities. Additional expenses incurred under any of these circumstances are to be borne by the participant. General L&J and Sockers F.C. Chicago will assume that the participant and the parents or legal guardians are in agreement with the conditions above, as well as the conditions mentioned in the documentation, and that all engagements with L&J have been undertaken by an authorized person.
