1 Information, Requirements and forms for DofE Silver Expedition Dear Parent/Guardian, The purpose of this letter is to provide you and your child with important information about the Silver expeditions of Duke of Edinburgh Award (also known internationally as International Award for Young People) coming up in January-February, The main details of the trip are as follows: Dates: Practice Expedition: Thursday, 19 th January Friday, 20 th January, 2017 Qualifying expedition: Wednesday, 8 th February- Friday, 10 th February, 2017 Activities: Accommodation: Forms: Cost: Hiking, Navigation and Camping Tents supplied by EIS-Meadows All forms needed are included in this letter- the last 3 pages 900Dhs Cost is per student for both trips and includes the following: Transport from EIS and back Water Qualified instructors from North Star Public liability insurance NOT included in the price:- food, backpack, sleeping bag & hiking shoes, eating utensils, sleeping mat, pots and pans Tents, maps, compasses, gas cookers and selected camp equipment School chaperones Support vehicles supplied by North Star Personal medical (health) insurance Some additional camp equipment could be borrowed from Ms. Ouzounova at EISM but the stocks are limited and it will on first come first served bases. If you are interested in sending your son/daughter please complete the forms below and return it with a payment of 900 AED by Thursday, 20 th October,2016 this is a non-refundable payment. Payment and signed forms are to be returned to Ms. Daniela Ouzounova in the PE office. Payments for the trip via bank cheques are to be addressed to EIS-Meadows. Training Programme It is essential that these sessions are attended as they will introduce the basic skills needed to have safe and enjoyable expeditions. During each session students will be assessed on their readiness and ability to complete the expedition in a safe manner. This will be judged by the DofE teacher leader running that activity. A student may be denied the opportunity to participate in the expedition if the teacher feels he/she is not ready or incapable of completing the expedition safely. The expeditions are physically challenging and so they also require certain level of fitness. Below is the training schedule until the Practice trip.

Monday, 17 th October, 3-4pm Packing and kit lists Monday,24 th October, 3-4pm Tents Monday, 31 st October, 3-4pm Building a campfire Monday, 7 th November, 3-4pm Map & compass basics Monday, 14 th November, 3-4pm Camp cooking Monday, 21 st November, 3-4pm Map & compass- maps, checkpoints, route planning Monday, December 5 th, 3-4pm First Aid Monday, 9 th January, 3-4pm Route planning-actual trip Monday, 16 th January, 3-4pm Trip meeting-all going to the trips must be present Note: This training schedule could be a subject to change due to unforeseen circumstances at the time of issue of this letter. In the case of a change everyone involved will be informed accordingly in advance. Transport January Practice Trip: We meet at EIS-Meadows reception area at 7:50am on Thursday 19 th January, 2017 and return to EIS-M at 5:00pm on Friday 20 th January, February Qualifying trip: We meet at EIS-Meadows at 8:30am on Wednesday, 8 th February, 2017 We will return to EIS-M at 5:00pm on Friday, 10 th February, Transport will be provided by Diamondlease. Medical History Please complete the attached form regarding your son/daughters medical history. Accident and Health Insurance Each student is covered for any illness or accident that occurs during the trip. The insurance for all DofE trips is provided by North Star and gets approved by EISM management. The policy will be send to you as soon as the approval process is complete. If you do not consider this policy enough and would like to arrange your own for your child that is fine. Please provide a signed and dated copy of the policy in full by the 12 th January, 2017.

3 Suggested Equipment List - Medical pack (per hiking group) - Expedition clothing -2 days (warm for night) - Expedition food (snacks, lunch x 2, dinner (x 2 for Silver), and breakfast (x 2 for Silver)) - Walking shoes (good broken in trainers will be fine) - Sleeping bag + sleeping mat - Torch - Sunscreen & hat - Toiletries toothpaste, wipes/sanitizer - Toilet paper - Small amount of money for snack on our return - Rucksack - Lighter/matches - Compass & whistle - Water bottle - Tents and gas cookers will be provided by EIS-M (to be collected from Ms. Daniela Ouzounova no later than 12 th January, 2017) - Cooking equipment (cutlery, utensils and pots) Emergency contact The mobile number for emergency during the trips is , Ms Daniela Ouzounova. DofE Registration All students attending this trip must be registered DofE members with a completed registration from and fee. Information and questions All information regarding DofE has been distributed to parents via , DofE parent information evening, School newsletter and to students via Managebac. DofE registration and trip payment do not guarantee your son/daughter a place on the trip. Students may be prevented from attending excursions due to poor behavior, breaches of uniform dress code or other reasons. PARENTS OR GUARDIAN S DECLARATION To the best of my knowledge, the information I have given is correct and the child named below has my permission to engage in all the trip activities, unless otherwise noted and indicated in an enclosed written doctor s report. I accept full responsibility for any claims, losses, expenses or liabilities arising out of, or related to, my child s participation in the Silver/ Silver expeditions. I hereby give permission to North Star and/or EIS-M staff to dispense the above medication on my behalf, according to the dosage and frequency indicated. I understand that all medications should be sent in an appropriate container, clearly marked with the child s name and medication details. I hereby give permission to North Star and/or EIS-M staff to authorize, on my behalf, any emergency medical care (including surgery) which, on the recommendation of qualified medical personnel, may be deemed necessary. In such circumstances, I understand that North Star and/or EIS-M staff will seek to advise me or the emergency contact person designated at the earliest possible convenience. I understand and accept that should my child possess or consume any drugs or alcohol, smoke, or in the opinion of North Star and/or EIS-M staff behave in a way incompatible with the expeditions at any time, that I will make immediate arrangements for him/her to leave the expedition at my expense and return home. 6 Insurance Policy Group Coverage I have access to a copy of the group s insurance policy and agree to use this coverage for my child on the Silver expedition.
