1 Please complete and return pages 2&3 (and page 4 if your employer is paying for this course) SQA Level 1 British Sign Language Course To apply for this course simply complete this booklet and return to Deaf Action This course which is accredited by the Scottish Qualifications Authority (SQA), is designed for students with no previous experience of BSL Course information Cost - The cost of the course is 350, this includes all course fees. If your employer is sponsoring you, please ensure that you complete the Confirmation of Booking section (page 3), which they must sign. For all applications, excluding work sponsored places; a non-refundable deposit of 50 must be received with the application in order to secure a place on the course. Course payment is due by the first night of the course unless a payment plan is in place. Payment plans must be paid off by the end of the course. Skills Development Scotland ITA funding account This course has been approved to participate in the ITA Scotland scheme. ITA Scotland is a Scottish Government scheme that can help pay for the cost of learning. It s for people who are 16 or over and living in Scotland if: Your income is 22,000 a year or less You are on benefits You do not hold a UK degree (or overseas equivalent) You are not undertaking any form of full-time or part-time secondary, further or higher education You are not participating on any National Training Programme (Modern Apprenticeships, Get Ready for Work or Training for Work) Then you can apply for a learner account from ITA Scotland that will give you up to 200 a year towards the cost of a course. You still need to submit this application pack to secure your place on the course. Cancellation Terms & Conditions - Once an application form and deposit are submitted you have committed to a place on a course. You may cancel your place on a course up to 14 working days in advance of the course start date with no cancellation charge; however, your deposit will not be refunded. Any cancellation must be received in writing ( or letter). Cancelling within 14 working days of your course start date will incur 50% of the course fee. This rises to the full amount on the course start date or if you leave after the course has started. Occasionally, if there are insufficient bookings on a particular course we may have to cancel it. In these circumstances we will advise you approximately two weeks prior to the course and issue a refund on any fees paid. Deaf Action is the training provider. Ideally you should send us your proof of income at the same time as your application form. Proof of income is your last 3 payslips or proof of benefits (letter from where you receive them) or the nil income form attached to your ITA confirmation . If you are sending payslips or benefit forms, they must be originals, not photocopies and you must sign and date all three. These can be returned to you once they have been processed. If you receive digital payslips, please these to learn@deafaction.org with a digital signature on them. If you do not supply your proof income to us within 4 weeks of your ITA application, it will expire and you will need to re-apply. Deaf Action will never share your personal or contact information with any 3 rd parties except the Scottish Qualification Authority (SQA) for registration purposes. 5 Payment Plan Deaf Action are committed to making our courses as accessible as we can. We therefore offer payment plans to help spread the cost of the course.
