Privacy statement The Department f Educatin and Training (DET) trading as is cllecting persnal infrmatin n this frm, including names, cntact details and medical infrmatin fr the purpse f administering the EQI Hliday Adventure Prgram, t verify infrmatin prvided and t cntact applicants, parents and schl staff (fr example, t request further infrmatin). If yu live with a hmestay prvider, yur schl will infrm them f yur applicatin and participatin (if yu are accepted). The infrmatin n this frm will be made available t emplyees f the DET invlved in administering the EQI Hliday Adventure Prgram and t the apprved tur prvider delivering the hliday adventure prgram. This infrmatin will be given t supervising staff accmpanying the Hliday Adventure Prgram. Sme f the infrmatin yu prvide will be disclsed t insurance rganisatins, tur peratrs, airlines, travel cmpanies, accmmdatin prviders and ther external parties invlved in prviding services t the grup as part f the Hliday Adventure Prgram. Relevant infrmatin will be given t medical prfessinals in the event yu require medical attentin while n an EQI Hliday Adventure Prgram. Please refer t Medical in the EQI Standard terms and cnditins fr further infrmatin. 3 Instructins Sectin F: Apprval Principal (r delegate) Cnsider the student s schl attendance, discipline and behaviural histry in prviding yur apprval fr the student t participate in an EQI Hliday Adventure Prgram. If the student has an individual health plan, an emergency health plan and/r an authrity t administer medicatin frm, please attach a cpy t this frm. Assess the request taking int accunt: the age, maturity and behaviur f the student; their capabilities and trustwrthiness and the tur itinerary. Parental apprval is required in Sectin E. If a signature is nt btained and cnsent is prvided via ther means (e.g. phne call r ) then the schl must nte the date this ccurred in this sectin. Attach a cpy f any t this frm. 4 Sectin G: Cancellatin plicy All flights are nn-refundable except where Australian Cnsumer Law prvides therwise. The cancellatin plicy is as fllws: 59 t 35 days = lss f 60% f full fare 34 t 14 days = lss f 80% f the full fare 13 days r less = n refund If the tur is cancelled by EQI r the tur prvider, the student will be able t transfer t anther tur f equal value r a full refund will be prvided Befre the tur Sectin H: Tur rules abide by the Travel and Activities clause included in EQI s Standard terms and cnditins, which are published at cmply with all reasnable requests by the rganisers fr any additinal infrmatin r dcumentatin fr the purpses f rganising the Hliday Adventure Prgram (including but nt limited t arranging and bking transprt, turs and accmmdatin); participate in all pre-tur prgram briefings; and have my selectin in the tur revked if: On the tur I am suspended frm schl r my enrlment is cancelled; r I d nt have and maintain a schl attendance recrd and behaviur that is satisfactry t the Department. my parents being given infrmatin abut me, including (but nt limited t) my behaviur n the tur and any health r wellbeing issues r cncerns; the tur supervisrs immediately dealing with any breach f these Tur rules by impsing apprpriate cnsequences, including ntifying my parents. In serius cases, my schl will als be ntified (with additinal cnsequences impsed by the schl under the schl s Behaviur management plan) and I may be sent back t my hmestay (under supervisin) r an emergency placement, if my hmestay is nt available at my wn/my parents expense; fllw all lawful and reasnable directins f the tur supervisrs; be punctual; be curteus t and respectful f tur supervisrs, ther participants, tur guides, transprt and accmmdatin prviders and all ther persns invlved in the tur; be cnsiderate f the general public at all times and respect lcal custms and values; bey all laws f Australia and take respnsibility fr my behaviur; behave in a manner which uphlds and prmtes the highest standards f integrity and dignity; nly depart frm the tur supervisrs if I am in grup f three f mre and prir permissin is btained; take reasnable care t prtect my wn safety and the safety f the thers n the Hliday Adventure Prgram at all times; be respnsible fr the safe keeping f my persnal pssessins and travel dcumentatin whilst in while n the tur. Lst prperty must be reprted at the first available pprtunity t the tur supervisr; nt have sexual cntact, smke, cnsume alchl r take nn-prescriptin drugs at any stage during the Hliday Adventure Prgram; attend and participate in the planned activities thrughut the tur unless I have the express permissin f ne f the tur supervisrs nt t d s. Reasns nt t participate in the planned activities may include, but are nt limited t, injury and illness (after cnsultatin with the tur supervisrs); attend any arranged meetings f the Hliday Adventure Prgram grup (e.g. each mrning, t receive infrmatin and instructins regarding the day s activities); shw respect fr the prperty, facilities and staff (including bus, tur cmpany, accmmdatin sites, turist sites, aerplanes); nt damage, vandalise (including graffiti r putting initials n any bject) r remve any artefacts frm turist sites even if they appear insignificant r wrthless; bey all cnditins f entry at the varius turist sites visited while n tur; Page 4

5 remain as a grup, r with a tur supervisr, while turing; carry a cpy f the prgram itinerary at all times and ensure I have a cpy f the current accmmdatin address and phne numbers. If I becme lst I am t telephne the tur supervisr by mbile phne, if this is unsuccessful, I am t g by taxi t the accmmdatin site and ntify staff f my return; and be respnsible fr bringing my wn spending mney and cvering the cst f persnal expenses (including meals that are nt included in the itinerary), excess baggage fees and any excess payable fr claims made against my travel insurance plicy (if applicable). Digital devices I agree: that taking digital devices n the trip is at my wn risk and I am respnsible fr insuring fr lss/theft; t fllw instructins, related t the use f digital devices, given by peple f authrity (i.e. tur supervisrs, airline and airprt staff); (preference) r text family members at an apprpriate time and refrain frm ing r texting during meal times /attractin visits; t be respnsible fr glbal raming, Wi-Fi, r data charges; t use digital devices in an apprpriate manner that des nt disrespect ther members f the grup r the brader cmmunity; and t seek permissin befre taking clse up phts f any individuals r small grups. Transprt seek permissin frm a tur supervisr befre leaving the grup at the airprt. If leaving the grup, I must be in a grup f three r mre, have a watch r phne set n the time at the airprt and check in with tur supervisrs at the departure gate at the designated time; be respnsible fr my wn luggage and nt leave bags unattended unless specifically directed by the tur supervisrs r airprt authrities; ensure that my luggage cnfrms t the dimensins and weight in line with their ticketing/airline arrangements r the limits impsed by the tur prvider; wear seatbelts, life jackets and safety helmets when/if prvided; and remain n the bus I am allcated t and ntify the tur supervisr at the first available pprtunity if any student/s are missing frm the grup. Accmmdatin nt venture utside the precincts f the accmmdatin unless as part f the fficial tur r with the permissin f tur supervisrs; scialise in cmmn areas (dressed apprpriately e.g. n pyjamas r bare feet) f the accmmdatin site as lng as tur supervisrs are present; nt enter the rms f students f the ppsite sex at any time; keep nise and music t a minimum; nt rder rm service, take items frm the mini bar (if staying in a htel) r use the in-rm phnes ther than t cntact the tur supervisrs, if required; nt take items frm the accmmdatin such as twels r cat hangers; nt damage r destry any prperty in the accmmdatin belnging t smene else (e.g. the accmmdatin prvider r anther guest; nt invite any visitrs (utside the tur grup) t accmmdatin, unless prir apprval has been given by a tur supervisr; check fr any damage t the premises n arrival and ntify supervising staff; attend tilets and shwer facilities in pairs where they are lcated away frm sleeping areas; and keep my rm tidy. Meals be respnsible fr managing any fd allergies at meal times; sit adjacent t a tur supervisr at meal times, if I have a serius fd allergy; take spare fd with me if I cannt eat meals supplied; and Page 5

6 travel with an Epipen, if required. Health/wellbeing/safety travel with required medicatin, such as an Epipen, asthma medicatin etc.; being mnitred by tur supervisrs in the use f medicatin; disclse t the tur supervisrs any matter which might adversely affect my health, wellbeing r safety at any pint during the Hliday Adventure Prgram. This includes immediately reprting any accidents, illness, security issues r risk t tur supervisrs; and lk ut fr the health, wellbeing and safety f fellw students (and in particular rm-mate/buddy) and reprt any cncerns immediately t the tur supervisrs. Cnsequences fr breaching tur rules I agree: that a breach f these tur rules may result in my participatin in the prgram being cancelled and being returned t my hmestay prvider r an emergency placement if my hmestay prvider isn t available; and that my parent/legal custdian and hst schl will be ntified and any additinal expenses incurred will be the respnsibility f my parent/legal custdian.
