MINIMUM ELIGIBILITY CRITERIA for pre-qualification of the Vendors :- The applicants must fulfill of the following eligibility criteria individually as vendor for the captioned job. The Technical Bids of the vendors not fulfilling the said minimum criteria, their Price Bids will be rejected and kept unopened with the Bank. 1. The applicant shall be an established vendor in operation for at least five years as reputed Packers and Movers in PSU/Govt. Deptt/Banks with values as any of the following : i) Three similar works costing not less than INR 2.00 lakh each. Or ii) Two similar works costing not less than INR 2.50 lakh each Or iii) One similar work costing not less than INR 4.00 lakh 2.The Applicant s average Annual Financial Turnover during last 3(Three) years ending 31 st March,2016 should be at least INR 5.00 lakh. 3. The vendor should be duly registered as Transporters conform to all such rules and policies of the State Government of Assam and other government agencies as prescribed from time to time. 4. The vendor should be registered for the purpose of Service Tax and Sales Tax etc, as applicable and should be an income tax assessee with Pan Card. 5. The applicant should possess valid certificates / license for the purpose from concerned department. 6. The vendors should posses the following documents and should submit selfattested photocopies of the following along with Technical Bid: a) Certificate / License for appropriate authority. 4 of 15

Vendors should note that by providing the names of the clients in this form, they are authorizing the Bank to make inquiries about them with those clients. 3.2 Terms and Conditions for the Contract In addition to the terms and conditions mentioned in other documents which form part of this Technical Bids, the following terms & conditions shall also apply: A. GENERAL TERMS & CONDITIONS :- 1) The Technical Bids will be shortlisted after scrutiny and opening of the Technical Bids received by the Bank and only the Price Bids of the shortlisted vendors will be opened for finalization of the tender. 2) The acceptance of Technical Bids/ Price Bid or otherwise will vest with the Assistant General Manager(CPPC), State Bank of India, Centralized Pension Processing Centre, SETHI TRUST BUILDING, 6 th Floor, G.G. Road, Bhangagarh, Guwahati (Assam)., who does not bind himself to accept the lowest bid and reserves to himself the right to reject any or all the applicants / vendors which do not fulfill all or any of the 9 of 15

10 conditions and are incomplete in any respect, without assigning any reason whatsoever. The Bidders shall not be entitled to claim any costs, charges, damages and expenses of and incidental to or incurred by him through or in connection with his submission of Technical Bids / price bids, even though the Bank modify/ withdraw the tender. 3) The applicants are advised to submit the tender based strictly on the general conditions governing the Service Contract, Bank s requirements, scope of the work, Technical Specifications, conditions for Commercial bidding, etc. and not stipulate any deviations. Applications containing deviation from the terms and conditions are liable to be rejected. 4) The Bidders will be required to Earnest Money deposit amounting to INR (Rupees five thousand only) in the form of Demand Draft/Banker Cheque in favour of Asstt. General Manager(CPPC), State Bank of India, Centralized Pension Processing Centre, Guwahati payable at Guwahati drawn on any Bank in India as Earnest Money alongwith the Technical Bid. Earnest Money Deposit shall not carry any interest. If the vendor who quoted lowest amount in the Price Bid, backs out and will not execute the work in time, the Earnest Money Deposited will be forfeited. 5) Bidders will be required to comply with all statutory requirements of the Govt. bodies. In case they fail to comply with any of the statutory requirements, the Bank may pay the full or the unpaid balance due, as the case may be, either by deduction from any amount payable to the vendor under any contract, or as a debt recoverable from the contractor, or by invoking the Earnest Money / Security Deposit to be submitted by the contractor, at the sole discretion of the Bank. 6) The Bidders should visit both the premises i.e. the existing premises and alternative premises of CPPC, Guwahati to assess the work before submitting their Bids. 7) All pages of the Tender Documents duly signed should be completed in all respects with all attachments/ enclosures/ annexure 8) SECURITY DEPOSIT TO BE FURNISHED BY THE SUCCESSFUL BIDDER :- 1. The VENDOR who wins the price bids, will submit Security (including Earnest Money deposit) on contract value on award of the contract for the work. The security deposit shall be retained till expiry of the contract. The Security Deposit will be invoked under any of the following circumstances: (a) The VENDOR fails to complete the contractual obligations as per the tender. (b) VENDOR leaves the job half way. (c) The VENDOR does not undertake the jobs within the stipulated period. (d) The VENDOR does not perform the job to the satisfaction of the Bank. 2. Failure to perform the work and submit the Security Deposit within the stipulated period of time will result in forfeiture of the Earnest Money deposited. 10 of 15

11 B. OTHER TERMS AND CONDITIONS :- 1.Exclusive vehicle to be provided for transportation of the goods / documents of the Bank as mentioned in the tender. 2. The rate quoted by the Bidders will include pre-survey, supply of all packing materials, packing, complete documentation, loading transportation, unloading, unpacking, removal of debris, placing at the new office etc. 3. All risk insurance during transportation will be arranged by the Bidders through reputed insurance company on the basis of value of the goods/documents. 4. All payments will be made by the Bidder for any octroi, Govt. levies, taxes etc. and will be included in the quoted rate. 5. The Bidder will ensure the safe delivery of the goods/documents as specified by the Bank. In case of damages to the Goods/Documents, suitable action/levy of penalty as deemed fit by the Competent Authority will be taken. 6. The bill for payment should be submitted alongwith the proof of delivery duly signed by the authorized official of the Bank and payment will be made by the Bank within 30 days from the date of receipt of the bill. 7.They should have their own office at Guwahati atleast for last 3 years(evidence to submit) with telephone facility. 8. The Contract will be terminated without any notice, if the successful bidder Company/Agency violates any of the terms and conditions of the tender accepted by the bidders at any stage of the contract. 9. Tenderer should acquaint himself with the rules and regulations issued by the local authorities, applicable labour laws, etc., and quote the rates accordingly. No extra charges / increase in the rate shall be considered on any account, under any circumstances. 10.In case any permission, license or approval from any Municipal, Local authorities or any other Govt. Authorities is necessary, it will be vendor s responsibility to obtain the same at his own cost. 11.Bank will not be responsible for any loss to the vendor under any circumstances. 12.In respect of all workmen or employees directly or indirectly employed by the vendor for the performance of the works mentioned herein, the vendor shall at his own expense arrange for all the safety provision as per relevant provision of law. 13.The tenderer shall be responsible for the payment of wages and other dues to their employees employed by him in compliance of Minimum Wages Act and Equal Remuneration Act. 14.The tenderer shall ensure that the hours of work and other service conditions of employment of the contract labour is in consonance with all applicable laws and rules, including Contract Labour (Regulation and Abolition) Act etc. 15.It will be vendor s responsibility to ensure that each obligation under this contract is duly performed and observed. 16.The Bank will not be responsible for any compensations or otherwise for any injury/death caused to any person / staff/vehicles of vendor/other public vehicles while executing the work under this tender. 17.The tenderer shall be liable to comply with all applicable laws, rules and regulations in respect of all the labour laws and statutory requirements, including fire 11 of 15

12 safety regulations and other regulations, which are in vogue or will become applicable in future. 18.The tenderer shall accept and bear full and exclusive liability for the payments of any or all taxes etc. now in force or hereafter imposed, increased and revised from time to time by the Central or State Govt. or by any other authority with respect to or covered by wages, salaries, or other compensations paid or payable to persons engaged by the vendor. 19.The tenderer shall be responsible for all the claims for its workmen and the said workmen of the Vendor shall not make any claims whatsoever against the bank. The vendor s workmen will not have any right whatsoever to get absorbed in the Bank. 20.No child labor shall be employed by the vendor to carry out any of the works/services covered by this tender. 21.The near relatives of employee or employees of the Bank are prohibited from participation in this tender. The vendor shall have to submit an undertaking to the effect that his/their no near relative or relatives is/are employee(s) of the Bank. 22.The tenderer shall be entirely responsible for any mis-happening, accident to his driver/ worker while performing duty and shall have no claim/binding on Bank with respect to any compensation/monetary benefits etc., whatsoever. 23.The vendor should have adequate insurance coverage for i.
