ELIGIBILITY CRITERIA Applications are invited for empanelment of House keeping, Cleaning & Maintenance contractors/ service providers. Only those contractors/ service providers who possess the following eligibility criteria should apply. Applicants shall submit documentary proof of eligibility criteria along with the Application Form, duly filled in, signed and sealed. The empanelment will be done for a period of three years for the Bank s branches and offices in Pune and Pimpri Chinchwad cities and districts. The applications should receive on or before by pm at above address. Applications received after the date will not be accepted. Applications not meeting the eligibility criteria will be rejected. Applications are invited under following two categories: 1. For annual work value up to Rs.5 lacs. 2. For annual work value up to Rs.20 lacs The contractor must be a Proprietorship firm/ Registered partnership firm Or a Registered Company (private or public) or a Registered Co-operative society and having its registered office/ branch office in Maharashtra. The applicant must have its branch office/ franchise in Pune/ Pimpri Chinchwad city/ district. A copy of the same with address proof is to be enclosed. The contractor must have a minimum of three years experience in providing such house keeping, cleaning & maintenance services in reputed private sector organizations, Govt./ Semi Govt./ PSU organizations, Banks, Institutions etc and also having head office/ branch/ franchise in Pune/ Pimpri Chinchwad. The tender papers in the enclosed prescribed form must be signed by the proprietor/ all partners of the firm/ authorize designator of company/ society or by a person holding a power of attorney authorizing him/ her to do so. Copy of such power of attorney should be attached along with the tender. A copy of certificate of registration/ certificate of incorporation should be attached with the application. All papers should be duly signed.

2 Contractors should have in hand or satisfactorily completed cleaning, House keeping, maintenance/ facility management jobs, etc. in Maharashtra in Central Government/ state Government, PSU s/ reputed Private Sectors which should have been completed/ undertaken in last 3 (three) financial years. Tenderor should have at least one eligible work, in hand Or completed in last one year, at Pune/ Pimpri Chinchwad cities/ districts For category (a) Minimum 3 (Three) single works each having completed annual contract value not less than Rs.2.00 lacs (two lacs) i.e. 40% of maximum annual work value Minimum 2 (Two) single works each having completed annual contract value not less than Rs. 2.5 lacs (two lacs fifty thousand) i.e. 50% of maximum annual work value. Minimum 1 (One) single work having completed annual contract value not less than Rs.4.00 lacs (four lacs) i.e.80% of maximum annual work value. For category (b) Minimum 3 (Three) single works each having completed annual contract value not less than Rs.8.00 lacs (eighty Lacs) i.e. 40% of maximum annual work value Minimum 2 (Two) single works each having completed annual contract value not less than Rs. 10 lacs (ten lacs) i.e. 50% of maximum annual work value. Minimum 1 (One) single work having completed annual contract value not less than Rs.16 lacs (sixteen lacs) i.e. 80% of maximum annual work value. Total average annual turnover of the contractor for Housekeeping, cleaning and maintenance jobs, during the last 3 (three) completed years ending 31st March should be at least for category (a) minimum Rs lacs (four lacs) and for category (b) minimum Rs. 16 lacs (sixteen lacs). Certified/ Audited copies of last three years Balance Sheet, Profit & Loss A/c and yearly turnover should be submitted. Tenderor shall submit Solvency Certificate from a nationalized bank or other scheduled commercial bank to the extent of Rs lacs (four lacs) for category (a) and Rs lacs (sixteen lacs) for category (b). To show that the contractor undertakes such jobs on regular basis, he should own the adequate cleaning equipments/

3 machineries (enclose list) Tenderor has to enclose Certified copy of registration certificate under Employees Provident Fund Act and also latest proof of the same (paid challans) to be attached. Tenderor has to enclose certified copy of registration certificate under Employees State Insurance Act and also latest proof of the same (paid challans) to be attached. Certified copy of License from Labour Commissioner to employ contract labour under Contract Labour Act to be enclosed. Certified copy of last 3 years Income Tax Returns with PAN card No and copy of PAN card of the tenderor to be enclosed. Tenderor to submit Certified Copy of GST Registration which should be in the name of the tenderor. Latest paid challans to be attached. To show that the contractor undertakes such jobs on regular basis and of same magnitude, the contractor should have adequate number of employees on his pay rolls (sufficient proof/ muster/ pay roll copies to be given). NOTES: Information has to be filled up specifically in this format. Please do not write remark As indicated in the Brochure. Information shall be limited to the Applicant. If any relevant data concerning the Group of Companies, to which the Applicant belongs, is desired to be given, the same shall be given separately in a supplementary sheet. Application not providing details or with insufficient details or not signed shall be rejected. All pages shall be signed and stamped with the rubber seal of the establishment. The person/persons who will be signing the tender documents on behalf of the organization must have necessary authorization/ Power of Attorney to do so. If the space in the Proforma is insufficient for furnishing full details, such information shall be supplemented on separate sheet of paper, stating therein the part of the Proforma and serial number. Separate sheet shall be used for each part. Bank reserves its right to accept any or reject any application/ applications without assigning any reason therefore.
