Detal treatmet must be received from the Member s participatig detal office uless exceptio is specifically authorized i writig by the Pla. Routie ad periodic examiatios are limited to oce every 6 moths per erolled Member. Extractio for orthodotic purposes. Detal procedures ad charges icurred as part of implats (placemet or removal) ad prosthetic devices placed o implats (fixed or removable). Example: bridges, crows, detures. Prophylaxis procedures are limited to oce every 6 moths. Replacemet of lost or stole detures, crow ad bridgework or other detal appliaces. Bitewig radiographs (x-rays) i cojuctio with periodic examiatios are limited to oe series films i ay 12 cosecutive moth period. Full mouth radiographs (x-rays) i cojuctio with periodic examiatios are limited to oce every 3 years. Paoramic films are limited to oce every 3 years. Fluoride treatmet is limited to erolled Members uder the age of 18 years oce every 6 moths. Periodotal scalig ad root plaig, ad/ or sub-gigival curettage, ad periodotal maiteace procedures are limited to oe course of therapy durig ay 12 moth period.
