6 Who is Eligible for Beefits? Thik Smart... It is importat to esure that you (ad your family) have the coverage you eed whe you eed it most. Pla Ahead... Because we all have differet eeds, AARP offers you a choice of beefits to fit your lifestyle. From medical plas to life isurace, you select the programs ad beefit levels that meet your eeds. Employees You are eligible for beefits if you are a full-time employee regularly scheduled to work forty hours a week. Part-time employees must work at least 21 hours per week or at least 14 hours per week i a approved job share arragemet. To be eligible for Log-Term Disability, you must be regularly scheduled to work at least hours per week. Depedets Medical, Visio, Detal, ad/or Life Isurace If you are eligible, your spouse, AARP registered domestic parter, ad childre are eligible. Childre are eligible for coverage util the ed of the moth i which they tur age 26. They must be your biological, adopted, or step-childre; the child of your AARP registered domestic parter; or childre for whom you have bee grated guardiaship by a court order. Your child does ot have to be a studet, a depedet for tax purposes, or umarried. You should oly eroll legitimate depedets who are eligible uder the pla. AARP has a obligatio to esure that oly eligible depedets are covered uder the pla. AARP may, at ay time, request verificatio of a depedet s relatioship status through supportig documetatio. Examples of this documetatio may iclude, but are ot limited to marriage certificates, birth certificates or court ordered guardiaship support documets. Childre beyod Idicated Coverage Ages Coverage for a erolled depedet child who is ot able to be self-supportig because of metal or a physical challege will ot ed just because the erolled depedet has reached age 26. AARP will exted the coverage for that erolled depedet beyod age 26 if both of the followig are true regardig the erolled depedet: Is ot able to be self-supportig because of metal or physical challege. Depeds maily o the employee for support. Coverage will cotiue as log as the erolled depedet is icapacitated ad depedet uless coverage is otherwise termiated i accordace with the terms of the pla. You must furish the Admiistrator with proof of the erolled depedet s icapacity ad depedecy withi 31 days of the date coverage would otherwise have eded because the erolled depedet reached age 26. 4

7 Domestic Parters Ay employee eligible to participate i medical, visio, detal, ad/or life isurace coverage offered through AARP may elect to cover a AARP registered domestic parter same or opposite sex ad/or childre of a AARP registered domestic parter uder those plas. The value of premium cotributios made by AARP toward a AARP registered domestic parter is cosidered taxable icome for the employee. Employee cotributios toward a AARP registered domestic parter coverage must be made o a post-tax basis. If a covered AARP registered domestic parter loses eligibility, he/she will be eligible for beefits through a COBRA-like cotiuatio pla. These beefits are idetical to those offered through stadard COBRA. Details regardig AARP registered domestic parter eligibility ad the erollmet process are foud o IfoNet. Cotiget Workers To the extet permitted by applicable law, cotiget workers are ot eligible to participate i ay of AARP s employee beefits, icludig, without limitatio, pesio, 401(k), retiremet, deferred compesatio, welfare, medical, health, group isurace, disability, bous, vacatio pay, leave policies, severace pay ad other similar plas, programs, ad policies. Rehired Employees If a employee separates from AARP ad is later re-hired by AARP, eligibility for beefit plas will vary depedig o the Origial Start Date, Rehire Date ad Service Aiversary Date. These optios cover both i-etwork ad out-of-etwork coverage. Your costs are geerally less whe you receive care from i-etwork providers. I-Network Beefits Visit ay provider i the UHC Choice Plus POS etwork ad receive i-etwork beefits. I-etwork providers, physicias or other medical professioals, hoor your membership card ad bill UHC directly for services redered based upo cotracted rates or discouts. Deductible, co-pays, ad coisurace apply. Out-of-Network Beefits Visit providers who do ot participate i a UHC etwork ad you are still covered, but your out-of-pocket costs will be higher. Deductible ad coisurace apply. After meetig deductible, reimbursemet for a eligible charge is based o a maximum that is tied to reasoable ad customary charges (R&C). Out-of-etwork providers ca bill for their charges that exceed the UHC s R&C charges. Out-of-etwork providers may require paymet i full at time of service. You must submit claims to UHC to receive reimbursemet. 10 Fid a Doctor To fid a doctor or to see if your doctor participates i UitedHealthcare's POS provider etwork, go to ad select UitedHealthcare Choice Plus as the health pla. UHC Preferred Provider Orgaizatio (PPO) UHC offers a PPO etwork istead of a POS etwork i certai areas due to cotractual limitatios i the states Idaho, Motaa, Pesylvaia (Harrisburg oly), ad the U.S. Virgi Islads. The PPO beefits are the same as POS beefits; however you use the PPO etwork istead of a POS etwork i these areas. Employees livig i Hawaii have a separate PPO Pla that complies with state regulatios. Prior otificatio may be required before you receive certai services. Refer to the Summary Pla Descriptios o IfoNet for further iformatio. UHC Idemity/Out-of-Area Pla The UHC Idemity/Out-of-Area Pla is available oly to those employees who live i areas where the UHC POS/PPO etworks are limited. Traditioal pla that does ot iclude a etwork. Visit ay provider you wish. Pla beefits are subject to R&C charges for covered services. Providers ca bill you for charges i excess of the reimbursemet maximums. Eve i areas where this pla is offered ad o etwork is available, some providers may hoor UHC cotracted rates. If the actual charge is more tha the R&C amout, you pay 100% of the differece betwee the actual charge ad the R&C amout, i additio to ay applicable deductible ad coisurace amouts. R&C charges are based o what most providers i your area charge for a particular service. 11 UHC Deductibles, Coisurace, ad Out-of-Pocket Maximums Whe deductibles ad coisurace apply, i order for beefits to be paid, you must first satisfy a aual deductible, based o eligible charges. Deductibles: Aual out-of-pocket amout that a idividual or family must pay before coisurace is applied ad the pla begis to pay. Applies to ay service that requires coisurace but ot a co-pay (welless visits do ot require coisurace). Example 1: Family of 5 idividuals - Whe employee satisfies his or her deductible, coisurace is applied ad his or her beefits begi. - Whe employee ad spouse satisfy their respective deductibles, coisurace is applied ad their beefits begi. - Whe employee, spouse ad child satisfy their deductibles, coisurace is applied ad beefits begi for the etire family (family deductible is capped at 3 times a idividual). Example 2: Family of 5 idividuals - Whe employee ad spouse (or child depedet) satisfy their deductibles, ad two childre collectively satisfy a idividual deductible amout, coisurace is applied ad beefits begi for the etire family. Coisurace: Oce the deductible is satisfied, the pla pays a percetage of eligible cotracted rate or R&C charges ad you pay a percetage called coisurace. Co-pays: A set amout you pay for a specific service (typically a office visit). Co-pays are ot subject to the deductible. Out-of-Pocket Maximum: Whe your deductible, coisurace, ad co-pay amouts reach the out-of-pocket maximum (based o eligible charges), the pla pays 100% of eligible i-etwork ad out-of-etwork charges for the remaider of the year. At erollmet, choose a Kaiser facility at which you wish to receive care. Select a doctor withi that ceter who will coordiate your care. Most care will be provided withi your chose Kaiser facility.
