5 YOUR 2018 BENEFITS WHAT S NEW IN 2018 Effective Jauary 2018, CVS Caremark will be Woodward s ew pharmacy provider for the Athem admiistered health plas, replacig Express Scripts ad Accredo Specialty Pharmacy. You ll be able to choose from more tha 68,000 retail pharmacies atiowide, icludig idepedet pharmacies, ad chai pharmacies such as Walmart, Walgrees, Meijer, Shopko, Kig Sooper, ad 9,600 CVS locatios (icludig those iside Target stores). We thik you ll fid the ehaced member service level to be worth the chage. For example, you ll be able to pick up your 90-day supply of maiteace medicatios (such as blood pressure) directly from the CVS Pharmacy If you choose to do so. O November 1, you ll receive a CVS website lik that will allow you to check prevetive drug, preferred (formulary) drugs, ad costs from home, office, etc. You ll also fid a pharmacy locator to fid the locatio closest to you! CVS Caremark will serve as your mail order ad specialty pharmacy. I December, you ll receive a Welcome Kit from CVS Caremark that icludes more iformatio about your ew prescriptio beefit program. 6 WHO IS ELIGIBLE TO ENROLL You are eligible to eroll if you are a active full-time or beefit-eligible part-time member who works 20 hours or more per week ad has completed 30 days of employmet. Eligible depedets may iclude your: Lawful spouse; Child (up to age 26); or Certified domestic parter. Note: The term child meas a child bor to you, a child legally adopted by you, a stepchild, or a child related to you by blood or marriage ad you are the legal guardia. A depedet child who is o loger eligible due to attaimet of the limitig age of 26 is eligible to cotiue beefits uder COBRA. DEPENDENTS UP TO AGE 26 Our health care plas will exted coverage for adult childre through the etire caledar moth i which he or she attais age 26. Chagig Beefit Electios Durig the aual Ope Erollmet period, you ca chage coverages for the ext caledar year. After the aual Ope Erollmet period, you may be able to chage your coverage if you have a qualifyig evet. MARRIED WOODWARD COUPLES If you ad your spouse both work at Woodward: You are covered as either a member or spouse, but ot both; ad Childre are covered by oe paret oly. For health isurace coverage: Married members have the optio to cover the etire family uder oe health care pla; Oly oe member elects health coverage isurig the other Woodward member as a spouse ; ad The spouse waives health coverage as a member. Here is what you eed to kow about this ew pla: IT S CONVENIENT Oe stop for most care. The majority of your care is provided at oe of our Woodward Health Ceters. With just oe stop, you ca see the doctor, have routie testig performed (such as labs, mammograms, etc.), ad receive basic, oe-time prescriptios (such as atibiotics). Same-day or ext-day appoitmets. No claim forms. There are o claim forms to complete ad less paperwork overall. Welless programs ad services. You ca access our welless programs ad services at the WHC, makig it easier to stay healthy. IT S COST-EFFECTIVE No cost to you for care at a WHC. No outof-pocket expeses for care provided at the Woodward Health Ceters. IT S PERSONALIZED Persoalized service. As soo as you walk through the WHC door, you receive persoalized service to make your health care visits as comfortable as possible. Patiet Support Specialists/Care Navigators. They help you trasitio ay records ad schedule your appoitmets at the WHC or with ay eeded referrals. Care Coordiators. Your Care Coordiator helps you maage all of your health care eeds, whether with your Primary Care Physicia at the WHC or with ay eeded specialists. 9 YOUR 2018 BENEFITS OeWoodward Health Pla The OeWoodward Health Pla is a cosumer-drive pla admiistered through Athem with first dollar coverage provided by the compay usig a Health Reimbursemet Accout (HRA). Here is how the OeWoodward Health Pla works: For 2018, Woodward will credit your HRA accout $750 for sigle coverage ad $1,500 for family coverage that will be applied to your ad other out-of-pocket medical ad prescriptio drug costs. At year ed, ay uused dollars i the HRA will roll over ad be available i the ext pla year. I-etwork prevetive care is covered at 100% o out-of-pocket costs. This ecourages you ad your covered family members to get regular check-ups. Whe you eed care: First you meet your (HRA balace will be applied). Oce you meet the, you ad the pla share expeses (referred to as coisurace). Oce you meet the pla s out-of-pocket maximum, the pla pays 100% of your eligible expeses for the remaider of the pla year. Coordiatio of Beefits The OeWoodward Health Pla adheres to the followig Coordiatio of Beefits (COB) rules whe payig claims: Whe a workig spouse has other coverage through his or her employer, ad is also covered uder the OeWoodward Health Pla, his or her employer s pla is primary ad pays first. If a child is covered by both parets, the pla of the paret whose birthday falls first durig the caledar year is the primary pla for the child. COB does ot apply to prescriptios. 7

CVS Caremark will provide a lik for Ope Erollmet so members ca look up specific drugs, costs ad pharmacy locatios. You ca also cotact CVS Caremark s Customer Care Lie at with questios about trasitio, actual cost of specific drugs, etc. Most curret mail order prescriptios will be automatically trasferred. There is o actio required by you. Expired prescriptios will ot trasfer. If your curret prescriptio expires before Jauary 1, you must get a ew prescriptio from your doctor ad submit it for mail order service. If your curret medicatio is ot o CVS Caremark s Preferred (Formulary) Drug List, you will receive a letter from CVS i late November. If you take Prevetative Drug prescriptios, you will wat to check CVS Caremark s Prevetative Drug List to see if your prescriptio is listed. CVS will provide a lik to this list later this year.
