1 Basic Health Care/Health Coverage Initiative Application Overview Client Locator/Search Window The first screen after signing on to the system is the Client Locator window, shown below. It is used to search the database for clients by entering either full or partial information into the search fields. If only partial information is used, the screen displays a list of clients matching the search criteria. If the client does not exist, a new client can be added from this screen. Existing clients are those who have applied for one of the programs in the past. Figure 1: Client Locator Window This application is used for four programs in Contra Costa County (Basic Health Care, Health Coverage Initiative, Charity Care, and Discount). Page 1 of 16
2 Client Demographics Window The Client Information window is accessed by selecting a client from the search list in the Client Locator window. Residence Status of client is filled in by the system. Figure 2: Client Demographics Window The Client Information window contains demographic information about the client (home address, phone number, languages spoken, date of birth, SSN, Medical Record Number, Marital status, mother s maiden name, and residence status). The information on this page can be edited and saved as necessary. Page 2 of 16
3 Screening Window Figure 3: Screening Window The first step for applicants is the Screening window. The questions in this window are designed to screen the applicant for eligibility to other programs, such as Medi-Cal, Medicare, or commercial health insurance. Certain questions will bring up additional dialog boxes that require additional information (see following screens). Answering Y to the question Were you born in the USA? will cause the Additional Demographics window to appear, as shown in Figure 4. Figure 4: Additional Demographics Window This screen is used to enter in information that would be found on a client s birth certificate. We use this information to search the California Birth Index (CBI) for persons who are born in California and to help those who cannot purchase a birth certificate to do so. Page 3 of 16
4 Answering N to the question will bring up the Immigration Information window, as shown in Figure 5. Selecting Permanent Resident brings up collection fields for Green Card # & Dates. Select country of birth from list. Figure 5: Immigration Information Window This window collects information about where the client was born (if they are not a US citizen) and their status (Naturalized Citizen, Permanent Resident, Refugee/Asylee, or Unqualified/Undocumented Alien). Selecting one of the options (Naturalized Citizen, Permanent Resident, Refugee/Asylee, or Unqualified/Undoc Alien) may bring up additional data entry fields that will need to be filled out (as shown for Permanent Residents above). This information is collected to determine which program the patient/client is eligible to receive. Page 4 of 16
5 Answering Y to the question Do you have children at home under 21 years of age? will cause the Parental Information window to appear, as shown in Figure 6. Figure 6: Parental Information Window The questions on the Parental Information screen are answered with Yes or No; these questions are used to help determine possible eligibility for Medi-Cal instead of BHC/HCI. Answering Y to the question Are you disabled now? will cause the Disability Information window to appear, as shown in Figure 7. Figure 7: Disability Information Window The questions on the Disability Information screen are used to help determine possible eligibility for Social Security Disability/Medi-Cal instead of BHC/HCI. 6 Answering Y to the question Do you have medical insurance? will cause the Insurance Information window to appear, as shown in Figure 8 Figure 8: Insurance Information Window This window allows you to enter information about the client s medical insurance coverage (Company name, the person insured, policy number, group number, effective dates, address & phone number of insurance company, and whether it is an HMO or PPO type plan). The answers are used to determine BHC/HCI eligibility for the client. Page 6 of 16
7 Answering Y to the question Have you had health insurance within the past 3 months (not including Medi-CAL)? will cause the Insurance Information window to appear, as shown in Figure 9. Select reason from dropdown list. Figure 9: Insurance Detail Window Select the reason that the client no longer has insurance from the Reason Insurance no longer valid list, as shown above. Enter the Insurance coverage End Date and the Name of Insurance. The answers are used to determine eligibility to HCI. Answering Y to the question, Restricted Medi-CAL? will cause the Medi-Cal Information window to appear, as shown in Figure 10. Figure 10: Medi-Cal Information Window Enter the Medi-Cal Application Date and select the Application Status (from the three button choices). Page 7 of 16
8 Income Information Window After all of the questions have been answered in the Demographics window, the [ Income ] button is enabled. Clicking this button brings up the Income Information window, as shown in Figure 11. Figure 11: Income Information Window, Employment Tab There are seven tabs on this screen that need to be completed in order. Each tab represents a specific portion of the client s financial status. The first tab, Emp Inc., is used to record the client s (and client s spouse s) current employment records. Page 8 of 16
9 Entering Y for the question Do you have any income from employment or self employment? will cause the screen to change to that shown in Figure 12. Figure 12: Add Employment Information This screen is designed to keep a record of all employment incomes of the client and their spouse (or parent, if the client is a child). The system has the ability to accept an unlimited number of employer, income, and asset entries. All entries will appear in the table at the bottom of the screen. Page 9 of 16
10 When all of the jobs are entered, proceed to the next tab, Other. Select the Type of income from the list. Figure 13: Income Information Window, Other Tab This screen is designed to keep a record of all other sources of income that a client might have. If the client has other sources of income, enter Y for the question Do you have other sources of income?. The screen changes to that shown in Figure 13. The source of income can be selected from the Type drop-down list, as shown above. The system can accommodate an unlimited number of income types. A list of all income types entered will appear in the table at the bottom of the screen. Page 10 of 16
11 The next tab is RB-Home (for Room & Board information). Figure 14: Income Information Window, RB-Home Tab This screen is designed to keep a record of whether or not the client is receiving free room and board. It also keeps track of whether or not the person is homeless. If the client is receiving free room and board, enter Y for the question Do you receive free Room and Board?. The screen changes to that shown in Figure 14. Page 11 of 16
12 The next tab is the Assets tab. Select the Type of Asset from the list. Figure 15: Income Information Window, Assets Tab This screen is designed to enter the source and amount of the client s Assets. If the client has assets, the screen changes to that shown in Figure 15. Select the asset from the Type list and enter the amount for each type of asset. Every type and amount entered will appear in the table at the bottom of the screen. Page 12 of 16
13 The next tab is the Vehicle tab. Figure 16: Income Information Window, Vehicle Tab This screen is designed to record the value of vehicles owned by the patient/client (in excess of one vehicle each, for the client and their spouse). The screen changes to that shown in Figure 16. All vehicles listed will appear in the table at the bottom of the screen. The next tab is the Property tab. Figure 17: Income Information Window, Property Tab This screen is designed to record the value of real property (in addition the house the client is living in) that the client may own. If the client has real property, the screen changes to that shown in Figure 17. All real property entered will appear in the table at the bottom of the screen. Page 13 of 16
14 The next tab is the Supt-Pov tab. Select the Type of support from the list. Figure 18: Income Information Window, Supt-Pov Tab This screen is designed to record child and spousal support that the client may be paying. If the client is paying child and/or spousal support, the screen changes to that shown in Figure 18. All amounts entered in this screen reduce the patient/client s total adjusted income. All child and spousal support information entered will appear in the table at the bottom of the screen. Page 14 of 16
15 As information is entered, consolidated financial data is stored in the database and is displayed in real-time along the right edge of the window, as shown in Figure 19. Figure 19: Financial Data Verifications Window The Verifications window, as shown in Figure 20, displays all documents required from the patient/client based on their answers to all of the questions in this application. Figure 20: Verifications Window This screen allows you to enter, edit, or delete these verifications to prove eligibility for the BHC/HCI/Charity Care/Discount program and confirm the information provided by the client. 16 Summary Window The final step is to view the Summary window, as shown in Figure 21. The verifications are listed in the Verifications table; a list of letters that have been sent to the client are listed in the Letters table.
