FOR DUVAL COUNTY HEALTH DEPARTMNENT TB UNT EXPANSON THS CONTRACT is executed as of this JL day of.! ~ ~_L,, 2014, by and between the CTY OF JACKSONVLLE, FLORDA, a municipal corporation in Duval County, Florida (hereinafter the "Owner" or the "City"), and CORE CONSTRUCTON COMPANY OF JACKSONVLLE, NC. D/B/A CORE CONSTRUCTON CO., NC., a Florida profit corporation with principal office at 4940 Emerson Street, Suite 205, Jacksonville, Florida (hereinafter the "Contractor"). WTNESSETH, that for the consideration and under the provisions hereinafter stated and referred to moving from each to the other of said parties respectively, it is mutually understood and agreed as follows:. That Contractor is the lowest and best responsible bidder for furnishing all labor, materials, and equipment and performing all operations necessary for the TB Unit Expansion at the Duval County Department of Health, located at 515 West 6th Street, Jacksonville, Florida 32206, including, but not limited to, removal of plumbing tixtures, lavatory, chase, a portion of the chase wall, a portion of the wing wall, aluminum door, frame, sidelight, ceiling, tolding door ceiling track, ceiling boards and grid, HV AC equipment, duct, fixtures, resilient sheet floor, wall covering, wainscot, selected roof areas, toilet, urinal partitions, and resilient sheet and base t1oor, modification of doors, anct the construction of new partitions, wainscot, liquid roof tinish, liquid epoxy base and floors, ceramic wall tile, roof top air handler, electric duct heater, urinal, lavatory, water closet, doors, new gypsum boards, insulation, air diffusers, air return grille, exhanst tim, acoustical ceiling board, light fixtnres, exit fixture, occupancy sensor, room number signs, plumbing work, mechanical work, electrical work, and all other improvements and repairs required to complete the work, all in accordance with plans and specifications hereafter refe1ted

2 to, and has been awarded this Contract for said work pursuant to award made September 16, The Contractor will at its own cost and expense do the work required to be done and furnish the materials required to be furnished on said work in accordance with plans and specifications prepared by the Engineering Division of the City of Jacksonville's Public Works Department and VRL Architects, nc., bid numbered JCF , bid date August 6, 2014, designated as "Bid Specifications for Duval County Department of Health TB Unit Expansion," and strictly in accordance with the advertisement calling for bids, plans, specifications, blueprints, addenda, requirements of the City of Jacksonville, proposal of said Contractor, and award therefor (hereinafter collectively the "Contract Documents") now on file in the Office of the Chief of the Procurement Division of the City of Jacksonville, all of which are hereby specifically made a part hereof by reference to the same extent as if fully set out herein, for a total maximmn indebtedness not-to -~xceed THREE HuNDRED TWENTY-SEVEN THOUSAND AND 00/100 DOLLARS (327,000.00), at and for the prices and on the terms contained in the Contract Documents..3. On the faithful performance of this contract by Contractor, Owner will pay Contractor in accordance with the. terms arid on the conditions stated in the Contract Documents. 4., This Contract and all amenchnents thereto may be executed in several counterparts, each of which shall be deemed to be an original and all of such counterparts together shall constitute one and the same instrument.
