See Instructions to Bidders. SCOPE OF THIS ADDENDUM The following becomes a part of the original project manual and drawings, taking precedence over the items that may conflict. The bidder shall note receipt and make acknowledgment of the Addendum on his/her bid form, incorporating its provision in his/her bid. GENERAL A pre-bid meeting was held on Wednesday, May 24 th. Meeting minutes and attendance roster are attached at the end of this addendum. PROJECT MANUAL The following additions, changes and clarifications have been made to the Project Manual. Bid Form: Replace the Bid Form with the revised Form attached at the end of this addendum. Section Summary: Clarification: 1.4 Type of Contract, Page : Add the following paragraph: B. Schedule: Starting date for Work will be established in a Notice to Proceed issued by the Engineering Manager of the CFMO. Work shall be completed within 90 calendar days (AND) on or before December 31, Clarification: 1.4 Type of Contract, Page : Add the following paragraph: C. Work Under Separate Contract: The Entrance Latrines will also be remodeled under separate contract. The Work of this Contract shall be coordinated so one of the two Latrines will remain operational at all times. 4 The above bid includes all applicable State and Municipal Sales and Use Taxes on materials, and State and Municipal Excise Taxes and all other State and Federal Taxes that would affect the amount of the bid. (See Instructions to Bidders-SD Sales and Use Tax Information for Public Contracts.) In addition, any material furnished by the State for use in this project is subject to Use Tax and Excise Tax. The total taxable value of materials furnished by the State for this project is $ A Performance and Payment Bond as required by General Conditions will not be required on contracts which do not exceed Fifty Thousand Dollars ($50,000). (See SDCL as amended). If discrepancies remain at the time of substantial completion, a value will be assigned to each of the discrepancies and two (2) times their estimated value will be retained from payment to the Contractor until completed and accepted. (See SDCL as amended). Within ten (10) days after Contractor's receipt of the Agreement for Construction, the Contractor shall submit to the South Dakota Army National Guard, the executed Agreement for Construction, Performance and Payment Bond, Certificates of Insurance and Affirmative Action Plan (if applicable). Work shall be commenced within ten (10) consecutive calendar days after written Notice to Proceed by the Engineering Manager and shall be substantially completed within 90 calendar days (AND) on or before December 31, The undersigned acknowledges receipt of the following addenda to the drawings and/or specifications (give number and date of each): Addenda Nos. dated respectively. The undersigned acknowledges that they have read and understand the Asbestos-Containing Materials Statement contained in the project manual. Accompanying this proposal is a certified check, cashier's check or draft in the amount of 5% of the base bid and all add alternates, and drawn on a State or National Bank in the amount of $ or a 10% bid bond issued by a surety authorized to do business in the State of South Dakota, in the amount of $. (Not applicable if Bid is under $50,000.) In submitting this bid, it is understood that the right is reserved by the Owner to reject any and all bids and to waive any irregularities. It is further understood by the Bidder that he may not withdraw his Bid within 30 days after the actual opening thereof. In submitting this bid, bidder asserts it has reviewed all provisions of the General Conditions including the provision for assessment of liquidated delay damages found in Article 10 of the General Conditions. Bidder agrees that the damages anticipated by the Owner in the event of delay in completion of the project are uncertain in amount and difficult to prove; the amount stipulated in Article III of the Agreement for Construction is a reasonable amount in light of the anticipated loss and injury; and the Owner s actual damages in the event of delay would be impracticable or extremely difficult to fix. 6 ELECTRICAL ADDENDUM E01 SDARNG Armory Entrance/Main Latrines May 30, 2017 Brookings, South Dakota Associated Consulting Engineering, Inc. 340 South Phillips Avenue Sioux Falls SD GENERAL: 2. Reference the Main Locker Rooms/Latrines, Women & Men. a. Remove light switches controlling the fixtures in each of these spaces and replace with a blank cover. Wire such that switches can be added in the future. b. Add four (4) ceiling mount occupancy sensors in each of these spaces for controlling the lights in each space. Mount occupancy sensors at approximate locations shown below. Provide power packs as necessary. Power packs shall be mounted accessible above the ceiling in the Exercise Room located between the locker rooms/latrines. c. Occupancy sensors shall be of the low voltage dual technology type equal to WattStopper #DT300- U series. Connect such that all lights in each space are controlled similar to the switching shown. d. Power packs shall be equal to WattStopper #BZ-200-U series. 1

8 2823 West Main Street Rapid City, South Dakota (605) FAX: (605) May 2017 SUBJECT: Optional Pre-bid meeting on May 24, 2017 for Latrines Projects The following questions and italicized responses were raised at this meeting held at the Brookings South Dakota National Guard Armory on 24 May A sign-in sheet was distributed among the members of the meeting. 1. What are the hours to work? Rebecca will be there T-F 6:30-5, and Steve Siemonsma will be there M-TH 6: When is the Substantial completion for these projects? We advertised December 31; Banner Associates will clarify and re-issue bid form by Addendum. 3. Can both bathrooms be done at the same time? We need to have at least one set open since those latrines are the only ones we have at this facility. Would like if they were phased as that was the original plan. Contractors indicated that it would be difficult if there were two different contractors doing the different bathrooms. Banner Associates will clarify and re-issue bid form by Addendum. 4. Can the bathroom projects be combined? Originally the projects were split up because of budget constraints and that troop labor might be doing one of the renovations. 5. At this point, the questions asked above were repeated in the staff room, and the meeting adjourned to tour the facility and gather more information on the existing site conditions. 6. Will the rough-ins be changing in the latrines? They will be changing in the Entrance Latrines due to ADA accessibility requirements; dimensions for new locations are shown on architectural plans. 7. Will we be salvaging the stall partitions and mirrors? Salvage the plastic partitions and mirrors in the main latrines only. 8. Can we enter the walls to get to the shower plumbing? It is impossible to work on the fixtures without getting into the wall somehow. Yes, that is acceptable to open the walls in the showers to gain access to the plumbing. 9. Are the shower seats provided for the handicap stalls? The shower seats are specified in Section Toilet and Shower Accessories. 1
