2 Tender No: - BMHRC/Radiology Services/ /04/3 rd Call Director, BMHRC, Bhopal invites tenders in sealed envelope from the Registered Hospitals/Radiological Diagnostic Centers/Firms for 3 Hours daily onsite services as outsourcing regarding (i) Diagnostic Ultrasonography (whole body) (ii) All types of doppler studies and On site reporting on any other radiological investigations in emergency life saving clinical conditions, as required/indicated from time to time, for the period of one year on monthly payment basis further extendable as per requirement for one year on same terms and conditions. 1. Details of machines and equipments available at BMHRC a. CT Scan Machine of Siemens company of Somottom Sensation 32 detector 64 slices b. MRI Machine Magatom 1.5 Tesla, Siemens Model- AERA c. USG and Doppler, 1) Wipro GE Healthcare, Model No. - LOGIQ 5 pro, 2) Fujifilm Sono Site India Pvt. Ltd. Model- M-Turbo, WK32RR d. 800 ma Image Intensifier Sirescop for Routine X-Ray Only (Siemens Ltd.) 2. Obligation of services providers: Providing Radiologist with MCI recognised qualification in Radio-Diagnosis i.e. MD/DNB/DMRD for continuous 3 Hours between 10am to 4pm on Monday, Tuesday, Wednesday, Thursday, Friday and between 10am to 2 pm on Saturday for services regarding a. Diagnostic Ultrasonography (whole body) b. All type of Doppler Studies and On site reporting on any other radiological investigations in emergency life saving clinical conditions, as required / indicated from time to time. c. Plain and contrast CT scan, CT angiographic studies reporting d. Reporting on any other radiological investigations as required/indicated from time to time. 3. Obligation of the BMHRC a. BMHRC shall provide all its cooperation in providing and arranging all required machinery and equipments. b. BMHRC shall provide the services of all concerned technicians and operators for assistance to Radiologist services provider. Page 2 of 8

3 4. General Terms and Conditions / Instructions 1. Tenders should invariably be submitted in single bid system in sealed cover. On sealed covers following information should be indicated:- a) Tender reference number b) Tender regarding Radiology services c) Name of bidder with address 2. All documents should be enclosed with Bid Application Forms. a) List of documents to be submitted along with bid 1. Copy of MCI/MP State Medical Council recognized degree of Radiologist 2. Copy of MCI/MP State Medical Council registration of MD/DNB/DMRD in Radiology 3. Copy of valid registration of bidder i.e. Registered Hospital/Radiological Diagnostic Centre /Firm with competent authority. 3. Conditional bid will not be accepted. 4. A pre bid meeting with interested bidders will be held on Through this meeting all the queries and doubt of the bidders related to bids will be resolved. Any bidder who wishes to participate in the Pre-Bid meeting has to submit the points for consideration of the Pre Bid Committee, 3 days in advance of the meeting i.e., through (scanned copy of letters with sign and seal of authorized person) or hard copy of letter through courier besides this the copy of the document in MS- Word format is to be mailed to purchase@bmhrc.org or po.bmhrc@gmail.com with the tender number in the subject line of the Tenders will be opened on at 3 : 30 p.m. in presence of bidders or their authorized representatives. 6. Any further modification/deletion/additions in the terms and condition of tender will be notified through corrigendum notice on those news papers in which initial NIT was published and also on website of BMHRC or website of CPP portal 7. The Successful Service Provider shall furnish the performance security prior to signing the contract for due performance. The performance security should be for an amount of 5% of the contract value in form of Demand Draft/Bank Guarantee (in prescribed format) from any Commercial Bank in favor of Director, BMHRC, and it shall be valid for 15 months from the date of issue of Contract. The performance security shall be released on satisfactory completion of all contractual obligations. No interest shall be payable on the performance security. Page 3 of 8

4 Failure to furnish performance security in time would entail the cancellation of the contract and black listing of the firm. 8. Offer will be valid for 180 days from the opening of the tenders quoted. The tender rates should be kept open/ valid for a period of one year from the date of signing the agreement which is further extendable for further one year if mutually agreed between Director, BMHRC and service provider firm on same terms and conditions. 9. In case of default, institute will have the right to obtain services from open market/another party at Service providers risk and expenses. Liability of the vendor will be limited to the amount of performance guarantee. 10. Handwritten quotations shall be accepted at the bidder s risk. In case of any discrepancy in the figures, the rate mentioned in words will only be considered. 11. The Service Provider should have not been blacklisted during last three years by any government/private institution and there should be no vigilance/cbi case pending against the Hospitals/Diagnostic Centers /Radiologist. 12. In case of any change in formation of Hospitals/Radiological Services provider Centers/Firm it should be immediately informed to the BMHRC. If any change has been made by the service provider such as providing the services of new Radiologist, it is mandatory to submit by the service provider to BMHRC the copy of his/her degree recognised with MCI along with copy of registration certificate with MCI. 13. Selection will be made purely on the basis of lowest financial bid meeting the required eligibility criteria. L1 shall be decided based on the total lowest monthly cost of services. In case of same L-1quote by two or more bidders, bidder with higher experience in number of years (Post Qualification) shall be considered as L The Service Provider should enclose the copy of Registration of Radiologist with MCI/State Medical Councils. Copy of degree in Radiologist i.e. MD/DNB/DMRD Recognised with MCI. In case of Hospitals/Service provider Firms names of engaged Radiologist along with copy of their registration with MCI/State Medical Councils should be furnished. 15. The images and all other related details provided to service provider shall be the property of the BMHRC and shall not be used and /or shared with anybody other than the BMHRC except with the prior written approval of the BMHRC. 5 16. In case of any Medico-Legal case concerning or relating to the read report given by service provider as the registered radiologist, he shall provide full cooperation. 17. Payment to the Service Provider shall be made within 60 days from the submission of the bill. Service provider is responsible to submit a monthly bill for professional charges in duplicate after the 5 th of following each month. 18. In case of failure in providing outsourced services not only will the prorata payment not be made but a penalty equivalent to twice of the daily payment based on sanctioned normal monthly rate (excluding emergency services) may also be imposed. 19. The Director has full power to terminate the contract at any time during the period of contract without assigning any reason. 20. Arbitration clause: i. Arbitration the COMPANY and CUSTOMER shall make every effort to resolve amicably by direct informal negotiations any disagreement or dispute arising between them under or in connection with contract. ii. Deputy Director Finance is authorized to sign the agreement on behalf of BMHRC after approval and in event of any disagreement of dispute remaining unresolved, the same shall be referred to the Director BMHRC for arbitration who will act as arbitrator or will appoint an arbitrator whose decision shall be final and binding on all parties. iii. Venue of Arbitration shall be BMHRC at Bhopal. 21. Force Majeure: Any failure of omission or commission to carry out the provisions of this contract by the successful Bidder shall not give rise to any claim by any party, one against the other if such failure of omission or commission arises from an act of God which shall include an acts of natural calamities such as fire, flood, earthquake, hurricane or any pestilence or from civil strikes, compliance with any statute and/or regulation of the government, lockout and strikes, riots, embargoes or from any political or other reasons beyond the successful Bidder s control including war (whether declared or not), civil war or state of insurrection, provided that notice of the occurrence of any event by either party to the other shall be given within two weeks from the date of occurrence of such an event which could be attributed to force majeure conditions. 22. Any dispute is subject to jurisdiction of Bhopal court. Page 5 of 8

It is here by submitted that there is no ongoing police case/vigilance inquiry against the tenderer or/and its partners/directors of the company and he/she has never been convicted or punished by the Hon ble court of Law. 3. It is here by submitted that if I / We withdraw or modify the bids during period of validity, or if I / We are awarded the contract and I / We fail to sign the contract or to submit a performance security before the signing the contract.
