3 Travel insurance An integral part of Citibank VISA business payment cards is travel insurance, included by default and provided by Citibank in alliance with Colonnade Insurance S. A., organizační složka, the provider of the insurance. This type of insurance accompanying payment cards is taken out for the period of validity of the payment card. The insurance applies by default to the cardholder during business as well as private travel, together with up to three other individuals who are travelling with the cardholder to the same location, at the same time, and using the same transport vehicle as the cardholder. The maximum insurance benefits per insured event for one payment card is ed to the sum insured specified in table Scope of Travel Insurance below. Insurance validity The insurance is valid during a foreign business trip for up to 90 days, 24 hours per day worldwide outside the countries of the insured person s permanent residence (according to the insurance conditions = A country where the insured lives or has been living for at least six months or a country where the insured is a participant in the public health insurance system.). Insurance coverage commences at the time of departure from home or the place of performance of work in the country of permanent residence, whichever occurs later, until return home or to the place of performance of work in the country of permanent residence, whichever occurs earlier, but not later than the 90th day from the date of commencement of the trip, and only during the period of validity of the payment card. Who is insured? Citibank VISA business payment cardholder and up to 3 cardholder s fellow travelers whose insurance has been activated. Insurance activation The insurance does not need to be activated for the payment card holder. The insurance is active automatically from 00:00 a.m. on the day following the date of takeover of the payment card by its holder but not earlier than on the first day of the card s validity. Insurance for fellow travelers is only valid if, prior to commencement of the foreign trip, it is activated by paying travel expenses using the payment card and who travel together with the cardholder to the same place, at the same time and using the same means of transport as the cardholder. Payment of travel expenses means: Payment of air tickets, bus and train tickets, boat tickets etc. made before commencement of the foreign trip. Payment or booking of accommodation abroad made before commencement of the foreign trip. Payment or booking of car hire abroad made before commencement of the foreign trip. Travel insurance accompanying payment cards is established within the following scope: Medical expenses and transport. Assistance Services. Costs of transport of the Insured Person. Insurance of legal aid and bail in case of a traffic accident. Liability insurance. Luggage and travel documents. Luggage delay. Trip cancellation. Return and sending a replacement employee. 4 The insurance does not cover damage occurring in countries to which the Ministry of Foreign Affairs of the Czech Republic recommended not to travel, in connection with war, terrorism, explosion or radiation originating from nuclear facilities, the insured person s illegal conduct, engagement in high-risk sports, as a result of AIDS and some other diseases or other circumstances that existed before commencement of the trip, etc. Other examples of exclusions from travel insurance Doing high-risk sports. Carrying out professional sporting activities. Doing any kind of sports in organized sport competitions. Read these conditions carefully before travelling abroad. Assistance service (assistance service provided within the travel insurance accompanying the payment cards of Colonnade Insurance S. A., organizační složka) Together with your payment card, you will receive the insured person s assistance card with the insurance policy number and assistance service contact numbers indicated on the reverse side. Please have the card on you always when you travel abroad. Assistance insurance The assistance service is prepared to provide you with organizational and consulting assistance in remediating the consequences of an insured event at the place of your foreign trip. The service is available 24 hours a day, 7 days a week. The operators speak Czech and all major world languages. You can contact the assistance service by telephone (also through a fellow traveler or attending physician) at the number indicated on your assistance card. You will be required to provide the following information after contacting the assistance service: Name and surname of the insured person. Bank name and insurance policy number. Your payment card number and period of its validity. Information about the insured event. Contact details of the attending physician or hospital where you are hospitalized. Insurance claim form Claims for the insured event process must be made by completing the Insurance claim form which is available on our website at in section Corporate banking / Large companies / Documents / Payment cards. An insurance claim must be raised not later than 30 days after the occurrence of the insured event or as soon as practicable. 7 Fraudulent Misuse Insurance 50 Fraudulent Misuse Insurance 50 will protect you against the consequences of loss or theft of your VISA Gold Business card. It covers the following: Fraudulent misuse of your VISA Gold Business Card as a result of its loss or theft. Fraudulent misuse of your VISA Gold Business Card as a result of its loss or theft of the PIN code. compensation in case of loss or theft of keys if they were lost or stolen together with the VISA Gold Business card. compensation in case of loss or theft of personal documents provided that they were lost or stolen together with the VISA Gold Business card. Fraudulent Misuse Insurance 50 is included by default in the VISA Gold Business card and is offered by Citibank Europe plc, organizační složka in alliance with BNP Paribas Cardiff Pojišťovna, a. s., the provider of the insurance. Insurance benefits will be provided up to a maximum amount of CZK per misused VISA Gold Business card and no more than three times per calendar year. The insured persons are entitled to insurance benefits in the event of fraudulent misuse of the VISA Gold Business card as a consequence of its loss or theft involving PIN code use provided that the VISA Gold Business card was misused no more than 72 hours before a phone call is made to report the loss/theft of the VISA Gold Business card and to request that the VISA Gold Business card be blocked. Insurance benefits will be provided up to a maximum amount of CZK 50,000 per misused VISA Gold Business card and no more than three times per calendar year. Insurance against the loss or theft of keys if the insured s keys were lost or stolen together with the VISA Gold Business card. Insurance benefits as a result of loss or theft of keys will be provided up to a maximum amount of CZK for each insured event and no more than three times per calendar year. Insurance against the loss or theft of personal documents if they were lost or stolen together with the VISA Gold Business card. Insurance benefits as a result of the loss or theft of personal documents will be provided up to a maximum amount of CZK for each insured event and no more than three times per calendar year. Who is insured? All holders of VISA Gold Business cards under 75 years are insured by default. When does the insurance become valid? The insurance becomes valid from 00:00 a.m. on the day following the day on which the VISA Gold Business card is approved. Monthly premium Page 7 (of 13)

The policyholder, the insured or the entitled person also has the possibility to address the complaint to the supervisory authority in the insurance sector, which is the Czech National Bank, with its seat at Na Příkopě 28, Prague 1. If the insurer is called upon by the Czech National Bank to comment on the complaint, it will handle the complaint in the same way as in the previous paragraph and, at the instruction of the Czech National Bank, it will send the information on the result of the investigation directly to the complainant or back to the Czech National Bank. Insurance termination The insurance terminates at p.m. of the last day on which the VISA Gold Business card is valid. The insurance terminates at p.m. of the day on which you report the loss or theft of the VISA Gold Business card to the bank, unless a new card has been issued. The insurance ceases to exist at the latest when the insured person reaches the age of 80 years. Nowithstanding the above, the above insurance shall terminate as of 31 December 2018 at the latest due to termination of the cooperation between Citibank Europe plc, organizační složka and BNP Paribas Cardif Pojišťovna, a. s. as of that day. 9 Secured Insurance with Your Card Secured Insurance with Your Card will protect you against 9 risks that can arise as a consequence of theft of your Citibank VISA payment card: Insurance against fraudulent misuse of the Citibank VISA business payment card as a result of loss or theft of the card. Insurance against fraudulent misuse of the Citibank VISA business payment card as a result of its theft or loss involving misuse of the Citibank VISA business payment card involving PIN code use. Insurance against fraudulent misuse of the Citibank VISA business payment card in an online transaction as a result of its theft or loss involving misuse of the Citibank VISA business payment card. Insurance against theft or loss of keys, personal documents, wallet and bag if these are stolen or lost together with the Citibank VISA business payment card. Reimbursement of forced cash withdrawals from an ATM under threat of physical violence, and theft of cash withdrawn from an ATM in an assault or under the threat of physical violence, if the theft occurs during cash withdrawal or within 2 hours thereof. Insurance against theft of mobile phone and against fraudulent misuse of SIM card if the mobile phone is stolen together with the Citibank VISA business payment card. Insurance benefits can be paid out no more than three times per calendar year. Insurance benefits in the event of theft or loss of keys, personal identification, wallet, and bag will be paid out if those items were stolen or lost together with the credit card. Insurance benefits can be paid out no more than three times per calendar year. Insurance benefits in the event of theft of withdrawn cash in an assault or after a forced cash withdrawal from an ATM under threat of physical violence that occurred in direct relation to the cash withdrawal from the ATM using the insured person s Citibank VISA business payment card or that occurred under threat of physical violence, or in the event of theft of withdrawn cash in an assault or under threat of physical violence Page 9 (of 13)

10 within 2 hours of cash withdrawal from the ATM. Insurance benefits can be paid out no more than three times per calendar year. If the insured person s mobile phone is stolen together with his/her Citibank VISA business payment card, insurance benefits will be paid out provided that the stolen mobile phone was not more than three years old at the time of the insured event and provided that the insured person has blocked the mobile phone. The insurance benefit will be paid out as follows. If the same type of mobile phone is sold by a selected vendor, the insured person will be provided with a new mobile phone identical to the stolen device as long as the current sale price of the new mobile phone does not exceed the of the insurance benefit, i.e. a maximum of CZK 10,000. If the sale price of the new mobile phone exceeds: a. The of insurance benefits, the insured person will be provided with a voucher worth CZK redeemable for the purchase of a new mobile phone from a selected vendor. b. If the mobile phone type that was stolen is no longer available at the selected vendor, the insured person will be offered 3 types of comparable mobile phones (in terms of functionality and quality, the appearance of the phone being irrelevant) with a sale price up to the of the insurance benefit, i.e. CZK 10,000. The insured person will choose one such mobile phone at his/her own discretion. If the sale price of the new mobile phone exceeds the of insurance benefits, the insured person will be provided with a voucher worth CZK redeemable for the purchase of a new mobile phone from a selected vendor. The insurance does not extend to mobile phone accessories. If the SIM card in the stolen mobile phone is fraudulently misused, the insured person is entitled to insurance benefits provided that the mobile phone was stolen together with the Citibank VISA business payment card and the SIM card was inside the stolen device. In such a case, the insurance benefit is equal to the sum of the costs of calls made no more than 24 hours before the theft of the mobile phone was reported and before the request was made to block the SIM card or the mobile phone. Insurance benefits, including fees associated with blocking the mobile phone and SIM card, can be provided no more than once per calendar year per insured person. Who is insured? The insurance can be taken out by a person under 75 years of age with his/her Citibank VISA business payment card The insurance ceases to exist at the latest when the insured person reaches the age of 80 years. When does the insurance become valid? At 00:00 a.m. on the first business day following the day on which the Secured Insurance with Your Card was taken out. Monthly insurance Optional Secured Insurance with Your Card costs CZK 89 per month. The monthly insurance premium is paid through your Citibank VISA business payment card How to take out insurance? You can take out the insurance easily, without having to meet with a representative of the insurance company. 12 Optional insurance for payment cards Optional travel insurance can be taken out for payment cards issued by the bank that are by default accompanied by travel insurance. The following optional insurance can be taken out individually with the payment card; the insurance is offered in alliance with Colonnade Insurance S.A., organizační složka, the provider of the insurance. Travel insurance for a long-term stay Accident insurance Travel insurance for a long-term stay This extension is suitable for you if you are planning a trip abroad that will exceed 90 days because you are not covered by the basic travel insurance for long-term stays of this kind. The long-term stay insurance is taken out for a fixed period, for trips lasting more than 90 days. The insurance can only be arranged for persons under the age of 70. The insurance coverage starts at the time when the insured person crosses the Czech border when travelling abroad, but at the earliest on the first day of validity of the long-term stay insurance. The insurance coverage ends when the insured person crosses the border of the Czech Republic on his/her return from abroad, but at the latest on expiry of the 365th day after commencement of the trip and at the latest on expiry of validity of the long-term stay insurance. The long-term stay insurance is taken out in the territory of the Czech Republic before commencement of the journey to the same extent as the travel insurance included by default, unless stated otherwise in the Policy. Monthly premiums are charged for each commenced month of duration of the long-term stay, including the first 90 days of the trip. Accident insurance Accident insurance allows you or your family members to obtain insurance benefits in the event of your death or permanent consequences of an accident. Taking out accident insurance is a good option especially when you have not taken out any other insurance for similar risks. The worldwide accident insurance is taken out for an indefinite period, for the duration of the travel insurance included by default. The insurance can only be taken out for a payment card holder under 70 years of age. Upon taking out the worldwide accident insurance, the coverage of accident insurance taken out within the compulsory travel insurance is extended to the whole world (including the territory of the Czech Republic) and for 24 hours a day. How to take out insurance?
