1 Bank of Melbourne Gold and Platinum Credit Card Insurance Conditions of Use Changes to the policy effective for purchases on and after October
2 The complimentary insurance cover available on Bank of Melbourne Gold or Platinum credit card is changing. The following changes to your policy apply for all purchases made on and after October To help you understand these changes, please read through this document which provides a summary. The summary of the changes set out below are of a general nature only and should not be relied upon as setting out the full terms of your cover. You should read the full terms and conditions of insurance cover as set out in the Bank of Melbourne Gold and Platinum Credit Card Insurance Conditions of Use available on your credit cards page at bankofmelbourne.com.au/credit-cards. If you have any questions about your complimentary credit card insurance cover, please contact our insurance provider, Zurich Australian Insurance Limited, directly on Summary of changes to Bank of Melbourne Gold and Platinum Credit Card Insurance Conditions of Use. All italicised words have the same meaning as in the Bank of Melbourne Gold and Platinum Credit Card Insurance Conditions of Use for purchases made on and after 21 October Changes to eligibility for cover: The following changes may affect your eligibility for cover under your credit card s complimentary insurances: introduction of eligibility for cover for 457 Visa holders, their spouse and dependent children introduction of eligibility for cover for any of your dependents of any age who are physically or mentally incapable of self-support changes to the definition of dependent child/children changes to eligibility for overseas travel insurance policy (see below) 2
3 Excess Excess is payable for each claim you make on certain insurance covers and benefits. The amount of excess on specified covers and benefits is changing to $250 Repairing or replacing damaged property or personal goods If an item is damaged, lost or stolen Zurich may choose to either repair, replace or pay you the amount it would cost Zurich to replace the item, less an amount which takes into consideration it s age. The amounts deducted in this section of the policy has been updated. Automatic Cover for Some Pre-existing Medical Conditions Some pre-existing medical conditions, including asthma, diabetes, epilepsy and high blood pressure will be automatically covered, subject to the conditions of the policy. This means you no longer need to pre-apply to Zurich for cover for these conditions prior to travelling. 3
4 Changes to Overseas Travel Insurance Policy This benefit is available to you, your spouse and dependent children provided that all persons are up to and including 80 years of age when they become eligible for cover. You become eligible for Overseas Travel Insurance when you have a return overseas travel ticket before leaving Australia and you have charged at least A$500 of your prepaid travel costs to your eligible credit card account. Prepaid travel costs can include: flights prepaid overseas accommodation airport or departure taxes any other prepaid overseas itinerary items such as tour costs Your spouse and dependent child/children, who are travelling with you for the entire journey can also become eligible for cover when they also have a return overseas travel ticket before leaving Australia and at least A$500 of their prepaid travel costs have been charged to your eligible credit card account. More cover for Platinum - Overseas travel insurance cover is extended to a maximum period of 6 consecutive months for Platinum cardholders. The maximum period of cover for Gold cardholders remains 3 consecutive months. The maximum claim limits on a wide range of benefits are increasing, providing more cover and security while you travel. The following new benefits have been added to the overseas travel insurance policy, giving you extra peace of mind: Assault Hijack and detention Kidnap & ransom Domestic pets boarding 4
5 Purchase Security Insurance Policy Enjoy higher maximum claim limits on purchase security insurance Extended cover for Platinum - Platinum cardholders can now enjoy 4 months cover against loss, theft, or accidental damage over a wide range of new personal goods purchased anywhere in the world, provided the entire purchase is charged to your Platinum credit card account. Transit Accident Insurance Policy The maximum benefit amounts paid to cardholders for certain accidental death and injury sustained as a passenger (not as a pilot, driver or crew member), on a plane, bus, train or ferry as outlined in the policy have increased. Introducing Rental vehicle excess insurance in Australia cover for Platinum cardholders Platinum cardholders can enjoy cover up to A$5,500 for any insurance excess or deductible in the event you have to pay a claim under the comprehensive insurance policy of a rental vehicle you lease in Australia. Refer to the new policy wording for full conditions and exclusions. 5
6 Details of the changes to the policy: Eligibility for cover The following changes apply to eligibility for cover under your credit card s complimentary insurance policy: The definition of cardholder is replaced with: a person who permanently resides in Australia or a person who holds a current and valid 457 Visa and resides in Australia and to whom Westpac has issued an eligible credit card for an eligible credit card account. This includes additional cardholders. If a family is travelling together, only one person can claim the benefits payable to the cardholder. The others can only claim as a spouse or dependent child/children. In relation to the Purchase security insurance policy this definition is also extended to include any Australian resident who, by way of a gift from the cardholder, receives any personal goods, purchased by the cardholder. The definition of dependant child/children is replaced with: all children up to and including the age of 18, who the cardholder has sole custody of and who live with the cardholder; all children up to and including the age of 18 who the cardholder has shared custody of; all children from the age of 19, up to and including the age of 25 who are full-time students attending an accredited institution of higher learning in Australia and who the cardholder has/had sole or shared custody of, and who are dependent upon the cardholder for their maintenance and financial support; all unmarried persons who are physically or mentally incapable of self-support who the cardholder has custody of and who live with the cardholder; however, dependent child/children never means an infant born on the journey. 6
7 Excess Excesses on the following covers have changed and the new excess amounts are set out below: Insurance Policy Overseas travel insurance policy for Benefits 1 to 7: 1. 8 Repairing or replacing damaged property or personal goods This section of the policy has been updated with the following schedule. 10 Automatic cover for some pre-existing medical conditions The following pre-existing medical conditions are automatically covered subject to the conditions of the policy. Condition Acne Allergies Asthma Cataracts/ Glaucoma Coeliac Disease Diabetes/Glucose Intolerance Ear Grommets Epilepsy Gastric Reflux Requirements If you have not required treatment by a medical practitioner in the last 3 months. If the condition has not required treatment by a medical practitioner in the last 9 months and you have no known respiratory conditions e.g. Asthma. If no exacerbation requiring treatment by a medical practitioner in the last 12 months. If you have no ongoing complications, are not on a waiting list for an operation and have not been operated on in the last 2 months. If the condition has not required treatment by a medical practitioner in the last 6 months. If you were diagnosed over 12 months ago and have not had any complications in the last 12 months. You must also have a Blood Sugar Level reading between 4 and 12 or a HbA1C score of 9% or less. You must also not currently be undergoing treatment for kidney, eye or nerve complications. With no current infection. If there are no underlying medical conditions (e.g. Previous head trauma, stroke) and you have not required treatment by a medical practitioner for a seizure in the last 2 years. If the condition does not relate to another underlying diagnosis (e.g. Hernia / Gastric Ulcer). 10
11 Condition Gout Hiatus Hernia Hip/Knee Replacement Hypertension (High Blood Pressure) Requirements If the gout has remained stable for the last 9 months. If no surgery is planned. If performed more than 9 months ago and less than 10 years ago. If you have no known heart conditions and your current blood pressure reading is lower than 165/95. 11
12 Overseas travel insurance policy Increases to Overseas Travel Insurance claims limits Maximum claim amounts on the following benefits have increased as set out below: Benefit 1. Medical and hospital expenses incurred overseas 2. Loss or damage to personal property. Bed care patient allowance (e.g. miscellaneous expenses such as phone calls and TV rental): Emergency dental Property/personal goods left unattended in a motor vehicle for Platinum cards Portable electrical equipment and binoculars Cameras and associated equipment/ accessories for Platinum cards Laptop computers and associated equipment/ accessories for Gold cards Travel documents, travellers cheques, credit cards, cash etc. Maximum limit on and from 21 October 2012 Up to a total of $15,000 ($100 per day) for Gold and Platinum cards Up to a total of $2,000 per person for Gold and Platinum cards Up to a total of $20,000 per person up to a maximum of $30,000 per family for Platinum cards Up to a total of $15,000 per person up to a maximum of $20,000 per family for Gold cards Up to $5,000 per item Up to A$5,500 per item Up to A$5,500 in total Up to A$550 per person up to a maximum of $1,250 per family for Gold and Platinum cards 12
13 Benefit 2. Loss or damage to personal property. (cont.) Emergency replacement of your clothes and toiletries, Maximum limit on and from 21 October 2012 Up to A$800/ person to a maximum of A$1,600 for a family for Platinum cards Up to A$550/ person to a maximum of A$1,000 for a family Gold cards 4. Resumption of journey following the death of a relative 5. Special event - for reasonable cost of arranging alternative transport in order to attend a special event. 6. Rental vehicle excess 7. Travel delay - For reasonable additional meal and accommodation costs after a six hour delay 8. Funeral expenses. 9. Accidental death - In the event of accidental death which is not covered under the Transit accident insurance policy included in this document: 11. Loss of income - Cover for loss of income if you or your spouse is unable to resume work solely as a result of injuries whilst overseas on a journey. The income lost during the first month after the cardholder or spouse has planned to return to work is not covered. Up to a maximum of $12,500 for a family Up to $3,750 Platinum cards Up to $3,250 for Gold cards Up to $5,500 Platinum cards Up to $5,000 for Gold cards Up to A$500/person up to a maximum of A$1,100 for a family for Gold and Platinum cards Unlimited for Gold and Platinum cards Up to A$50,000 per cardholder and A$25,000 for a spouse or dependent child/children to a maximum $150,000 for Platinum cards Up to A$25,000 per cardholder and A$25,000 for a spouse or dependent child/children to a maximum $75,000 for Gold cards Up to 12 consecutive weeks cover up to A$1,000 per person per week, up to a maximum of $12,000 for Platinum cards A$750 per person per week, up to a maximum of $9,000 for Gold cards 13
14 New Benefits The following benefits are included in the overseas travel insurance policy: Benefit 12. Assault 13. Hijack and detention 14. Kidnap and Ransom 15. Domestic pets boarding Cover if you re assaulted and require hospitalisation as a result of your injuries while you are overseas. Any claim must be supported by a police report of the incident and medical certificate given by a registered medical practitioner. Cover if whilst overseas the control of the plane, bus, train,ferry or taxi you are travelling in is seized by force or threat of force by unauthorised persona and you are detained for more than 12 continuous hours. The cover will compensate you for each 24 hours you are held captive. Cover if whilst overseas on the journey you are illegally abducted and held hostage for the purpose of extortion or ransom. The cover will reimburse for any extortion or ransom monies paid to your abductors which results in your release. The policy sets out in which circumstances the cover will apply. The cover does not extend to kidnapping that occurs in countries or territorial waters of Mexico, the Philippines, Somalia or any country located in Central or South America. Cover for additional pet boarding fees for domestic cats or dogs, if your return to Australia is delayed because of events covered under the policy or your transport is delayed for reasons beyond your control. 15 Purchase security insurance policy Maximum claim limits on purchase security insurance have increased as below: Up to A$3,500 per claim for Platinum cards and A$3,000 for Gold cards for jewellery, watches and fine arts; and In any 12 month period, up to A$135,000 for Platinum cards and A$125,000 for Gold cards per eligible credit card account. Transit accident insurance policy The schedule of benefits in this section is replaced with: Injury Benefit Amount Platinum Gold cards Cards Loss of Life. A$750,000 A$250,000 Loss of both hands or both feet. A$500,000 A$250,000 Loss of one hand and one foot. Loss of the entire sight of both eyes. Reverse charge calls to this number can be made from many countries via Australia Direct, Telstra s international reverse charge service. Zurich s team of medical professionals are available 24 hours a day, 7 days a week for advice and assistance for medical emergencies and any associated problems for traveller, including arranging emergency transportation to the nearest suitable hospital, at your request advising your family in Australia of your medical condition and keeping them informed of the situation; and arranging urgent messages and emergency travel planning. 17 Documents to take with you when travelling If you need to make a claim while you re overseas, we will need to confirm your eligibility for insurance. So that Zurich can do this for you as quickly as possible, please make sure you take these documents with you when travelling: A copy of the Bank of Melbourne Gold and Platinum Credit Card Conditions of Use: it contains the details of the cover provided and important phone numbers. See above for how to obtain a copy if you don t already have one A copy of your return overseas travel ticket Proof of your eligibility for insurance, for example a copy of your credit card statement or card receipts showing prepaid travel costs of $500 the ticket using eligible credit card and other documents that confirm your eligibility 17

Zurich is liable for separate risks insured and the benefits of the covers are extended to Bank of Melbourne cardholders (as third party beneficiaries) as complimentary benefits.
