5 FAQs: MACRA Quality Payment Prgram 1. What is the Quality Payment Prgram (QPP)? The QPP imprves Medicare by helping yu fcus n care quality and the ne thing that matters mst making patients healthier. The Medicare Access and CHIP Reauthrizatin Act f 2015 (MACRA) ended the Sustainable Grwth Rate frmula, which threatened clinicians participating in Medicare with ptential payment cliffs fr 13 years. If yu participate in Medicare Part B, yu are part f the dedicated team f clinicians wh serve mre than 55 millin f the cuntry s mst vulnerable Americans, and the QPP will prvide new tls and resurces t help yu give yur patients the best pssible care. Yu can chse hw yu want t participate based n yur practice size, specialty, lcatin, r patient ppulatin. 2. Is the QPP the same as the MACRA? On Octber 14, 2016, the Department f Health and Human Services (HHS) issued its final rule with cmment perid implementing the QPP that is part f MACRA. The QPP plicy will refrm Medicare payments fr mre than 600,000 clinicians acrss the cuntry, and is a majr step in imprving care acrss the entire healthcare delivery system. Yu can chse hw yu want t participate in the QPP based n yur practice size, specialty, lcatin, r patient ppulatin. 3. What is the Merit-based Incentive Payment System (MIPS)? MACRA replaced three Medicare reprting prgrams with MIPS (Medicare Meaningful Use, the Physician Quality Reprting System, and the Value-Based Payment Mdifier). Under the cmbinatin f the previus prgrams, yu wuld have faced a negative payment adjustment as high as 9% ttal in 2019, but the MACRA ended thse prgrams, reduced the ptential negative payment adjustments in the early years, and streamlined the verall requirements. While these 3 prgrams will end in 2018, if yu have participated in these prgrams in the past, yu will have an advantage in MIPS because many f the requirements shuld be familiar. 4. An EC may use different 90 day perfrmance perids fr each MIPS perfrmance categry; hwever, all measures within that categry must use the same 90-day perfrmance perid. 14. 7 16. What are the ways I can Reprt MIPS / participate in the QPP? Hw much incentive am I ging t earn? ECs can receive a 4% negative payment adjustment t their Medicare claims if they chse t nt participate. ECs can earn up t a 4% psitive payment adjustment fr cmpleting the QPP requirements (percentage will depend n measure percentiles) ECs may als earn a prtin f the high perfrmer bnus ($500 millin set aside t be split amng all ECs wh scre greater than 70%). 19. Can I use different reprting methds fr each perfrmance categry? Yes, ECs can use different reprting methds fr each perfrmance categry. ECs cannt use multiple submissin methds fr a single perfrmance categry. Example: reprting sme quality measures by attestatin and reprting ther quality measures by QCDR. 20. Am I a grup r an individual? An individual is defined as a single clinician, identified by a single Natinal Prvider Identifier (NPI) number tied t a single Tax Identificatin Number (TIN). Each EC participating in MIPS as a grup will receive a payment adjustment based n the grup's perfrmance. Under MIPS, a grup is defined as a single TIN with 2 r mre ECs (including at least ne MIPS EC), as identified by their Natinal Prvider Identifiers (NPI), wh have reassigned their Medicare billing rights t the TIN.

8 21. Can we reprt n ther measures nt available in Quanum EHR? Mst Qualified Registries and QCDRs require interfaces t an electrnic health recrd. Quanum EHR des nt have plans t interface with Qualified Registries and Qualified Clinical Data Registries (QCDR), ther than the current QCDR ffering, in Hw d we reprt n measures if we chse t use a qualified registry r using ur specialized registry frm Quanum EHR? ECs will need t verify what data frmat requirements a qualified registry has. Mst qualified registries require an interface t an electrnic health recrd. Quanum EHR des nt have plans t interface with qualified registries in Cntracting the specialized registry thrugh Quest Diagnstics and Quanum EHR des nt qualify the prvider t use the registry fr MIPS reprting. The cnnectin is fr the ACI bnus measure, specialized registry. 23. Wh is respnsible fr charting in the EHR fr MIPS? Each ffice determines wh will be adding infrmatin t the chart. Each ffice will be different based n their wrkflw, size, specialty, etc. 24. Wh is respnsible fr tracking the prgress f the MIPS categries (ACI, IA, Quality)? The practice designates a persn within their practice t generate reprts and track prgress Quanum EHR des have a MACRA/MIPS Premium service available 25. Hw will I run my reprts if I chse t d grup reprting? Quality Quanum EHR needs the grup tax identificatin number (TIN) and the prviders t whm it applies. The Quality Measure Reprting mdule cannt aggregate quality data acrss separate Quanum EHR rganizatins. Once the grup TIN is added the entire practice patient data will need t be prcessed. ECs, (Quanum EHR prvider rle) can create a Quality Reprting Dcument Architecture III (QRDA III) xml fr the grup. The QRDA III can be submitted by the EC. The grup can use Quanum EHR s QCDR fr quality. ACI ECs generate ACI reprts and aggregate the reprt data fr their grup Reprting via attestatin IA ECs dcument and track IA fr the grup Reprting via attestatin 26. D I have t reach 100% t earn any incentive mney? An exceptinal perfrmance bnus f $500 millin will be split between Prviders wh meet the 70% threshld. There is currently n histrical data available t determine a break-even scre. But CMS has planned fr prviders earning 3 pints f the 100-pint cmpsite scre t avid a penalty in 2017, but nt earn an incentive. This is als knwn as a neutral payment adjustment and is likely t be the lwest bar in the beginning. Thse with zer pints fr nt participating will see a negative payment adjustment f 4%. Thse earning 4 t 69 pints will see a psitive payment adjustment, but are nt

9 eligible fr an exceptinal perfrmance bnus. Thse earning 70 pints and ver will receive a psitive payment adjustment and are eligible fr an exceptinal perfrmance bnus. 27. What s the threshld I need t have t get sme f the $500 millin? An exceptinal perfrmance bnus f $500 millin will be split between prviders wh meet the 70% threshld. Quanum EHR currently supprts 2017 Transitinal Advancing Care Infrmatin Objectives. The naming cnventin f the reprts currently reflects their rigin frm Meaningful Use. The reprts calculate the denminatr, numeratr, and percentage fr each bjective crrectly based n the data entered int Quanum EHR by the users. 33. Perfrmance pints are the pints yu earn fr having a percentage fr that measure. If yur percentage fr that measure is between 1% and 10%, yu will receive an additinal 1% twards yur ACI categry scre. Add this t yur base pints and yu nw have 13.5% f the 25% pssible fr the ACI categry. Bnus pints are awarded fr participating in specific registries r IA. The default cmpressed file applicatin prvided by Micrsft perating systems des nt supprt encryptin. There are free and paid cmpressed file prgrams that will pen the encrypted cmpressed file. 7- Zip, WinRAR, WinZip are examples f cmpressed file prgrams that will pen the encrypted cmpressed file.

12 38. Hw ften shuld I run my quality measure reprts? Running quality measure reprts is unnecessary just t see Where are we tday? Prir t Octber 21, 2017 the Guidelines by Cnditin page displays current Quality Measure perfrmance. Reprts > Guidelines. After Octber 21, 2017 users view measure percentages in the Quality Measure Reprting mdule. 39. Hw will I knw when a quality measure is satisfied fr a patient? Run the reprt using Quality Measure Reprting mdule then review the data the next day (refreshes every 24 hurs) 40. Which quality measures shuld I chse? We recmmend fcusing n 9 ttal quality measures ECs need ne utcme measure; if n utcme measure is available, select ne high-pririty measure ECs will submit a minimum f 6 quality measures 41. Will I submit my quality measures by attestatin r Registry? If ne f the quality measures yu wish t submit is nly pssible t reprt thrugh EHR attestatin, all f yur measures will need t be submitted thrugh EHR attestatin. If all f the quality measures yu chse can be submitted via a registry, yu can d s. Submitting data via a registry requires an extra fee fr the service. 42. Des reprting quality measures cst any mney? The Quality Measure Reprting mdule is free t all Quanum EHR practices. Wh will have access t the Quality Measure Reprting mdule? Prviders (Quanum EHR rle) and the practice s delegated administratrs (Quanum EHR rle) have access t the mdule Prviders have access t the Quality Measures Dashbard, the Physician Screcard and its tls, and Data Submissin. Delegated Administratrs have access t the Quality Measures Dashbard, Measure Settings and the Physician Screcard and its tls Prviders may request additinal staff be given access if needed. 44. What is the minimum number f patients I need t have fr any quality measure? Class 1 Measures can be scred based n perfrmance measures that were submitted r calculated that met the fllwing criteria: The measure has a benchmark and, Has at least 20 cases and, Meets the data cmpleteness standard (generally 50 percent) Receive 3 t 10 pints based n perfrmance cmpared t the benchmark. Class 2 Measures cannt be scred based n perfrmance and instead are assigned a 3-pint scre. Measures that were submitted, but fail t meet ne f the class 1 criteria.

13 Measures either: D nt have a benchmark, D nt have at least 20 cases, r D nt meet data cmpleteness criteria. Receive 3 pints. 50% f data cmpleteness Imprvement Activities 45. What are Imprvement Activities (IA)? In this new perfrmance categry fr 2017, clinicians are rewarded fr care fcused n care crdinatin, beneficiary engagement, and patient safety. The IA perfrmance categry within MIPS assesses hw much yu participate in activities that imprve clinical practice. Examples include nging care crdinatin, clinician and patient shared decisin-making, regular implementatin f patient safety practices, and expanding practice access. Under this perfrmance categry, yu ll be able t chse frm many activities t demnstrate yur perfrmance. This perfrmance categry als includes incentives that help drive participatin in certified patient-centered medical hmes and APMs. Mst participants: Attest that yu cmpleted up t 4 IA fr a minimum f 90 days. Grups with fewer than 15 participants r if yu are in a rural r health prfessinal shrtage area: Attest that yu cmpleted up t 2 activities fr a minimum f 90 days. 46. D ECs need t perfrm IA if their practice is a certified Patient Centered Medical Hme (PCMH)? N. Yu will autmatically earn full credit. 47. Additinal infrmatin n IA Reprted by attestatin. EC tracks and dcuments their IA. Yu must attest by indicating "Yes" t each activity that meets the 90-day requirement (activities that yu perfrmed fr at least 90 cnsecutive days during the current perfrmance perid). Yu may reprt activities using a qualified registry, via certified EHR Technlgy), QCDR, the CMS Web Interface (fr grups f 25 r mre), r attestatin. These intermediaries will need t certify that yu perfrmed the activities as indicated. Yu can chse t attest t the set f activities that are mst meaningful t yur practice since there are n subcategry reprting requirements. That is, yu dn't have t select activities in each subcategry r frm a certain number f subcategries. 48. Hw des Quanum EHR submit the reprts t CMS? The EC will submit by the fllwing ptins: ACI attestatin IA attestatin Quality attestatin r QCDR The QCDR is a premium service. The cst t each participating EC is $ The service is limited t 29 f the 34 available quality measures. Quanum EHR MIPS Service Offering 49. Des Quanum EHR have a MIPS attestatin service? Yes. The service prvides guidance and directin t help enable an EC t meet the requirements f the CMS MIPS under the MACRA.

14 The service is delivered remtely. On-site services are available. EC agrees t pay the service fee plus all related travel expenses.
