The Vendors shall enter into rate contract for Two Years. 2. Vendors shall supply Medicines to Officers/Supervisors/Employees/CISF and GSL dispensary requirement on GSL Medical Officers recommendation/endorsement on credit basis at discount on MRP rates. 3. The Vendor shall have valid drug license for retail/ wholesale chemist as per drug and cosmetics act and must have Shop within 8 KMs radius from GSL Office at Vaddem, Vasco Da Gama, Goa. 4. Vendors shall have minimum 01 year experience in the same field and also Average sales of 10 Lakhs per annum (To be supported by a certificate from a

2 Chartered Accountant. As well as supporting documents like GST/ VAT registration copy, IT Returns are to be submitted along with offer.) 5. Vendors shall supply only specified medicines/ Tablet/capsules/ampoules/syrups/ dressing materials as per the prescription with MRP, Expiry date and batch Number, clearly mentioned in the bills. 6. Payment (NEFT) will be made within 30 days (after submission of all relevant documents), from the date of supply. The Vendors shall ensure to obtain the signature of persons taking the Medicine on the bills, which shall be enclosed with the Vendors Bills for payment. 7. The performance of the Vendor is under periodical review and can be terminated, if found not satisfactory. 8. GSL reserves the right to cancel the rate contract at any time at its discretion without assigning any reason. 9. There will be no assured business from GSL and Officers/Supervisors/Employees/CISF are at a liberty to procure medicines from any of the empanelled Chemists. There should be no cash transaction against the GSL prescription holders as well as no substitution of the prescribed medicine as well as exchange of other items available with the chemist without authorization from CMO / MO, GSL. 10. The Chemist and Druggist should organize medication that are not available at the time of prescription / dispensing. 11. The estimated value of the medicines under the contract will be around 50 Lakhs. It may increase or decrease depending upon actual requirement. 12. Specific confirmation to the above points from 1 to 10 is mandatory; otherwise your offer is liable for rejection. 13. Interested Vendors may download the tender form from the GSL web site ( 14. SUBMISSION OF OFFER IN TWO BID SYSTEM: Offer must be submitted in two parts, each offer securely closed ( sealed ) separately in two envelopes Part I & Part II as stated below: Part I: (i) This Envelope super scribing Techno-Commercial Offer should contain all relevant statutory papers and registration certificates with Annexure including all pages of tender document are to be signed by the Authorized signatory.

3 (ii) It should also have a copy of signed blank price bid (after erasing price). (iii) It should be sealed in an envelope bearing "Subject Tender, enquiry number and due date, (iv) Prequalification documents as Qualified Criteria as described under clause 1-10 as above. (v) EMD of Rs.5000 (Rupees Five Thousand Only) in the form of crossed Pay order/demand draft drawn on Nationalized/Scheduled Bank, in favour of Goa Shipyard Ltd, payable at Vasco da Gama. EMD amount is not interest bearing and is refundable after the finalization of the Contract. Part II: This securely closed (sealed) Envelope super scribing PRICE BID, Tender Enquiry No., due date and time and should contain only the PRICES in the prescribed format at Annexure II provided with the tender. Offer in any other format shall not be considered. Both these securely closed Envelopes i.e. Part I & Part II should be put in a third Bigger Envelope securely closed (Sealed), super scribed with Tender No., due date, time and addressed to, Dy. General Manager ( HSE), Goa Shipyard Ltd. Vasco da Gama, Goa, India and the sealed tenders shall be submitted in the Tender Box placed at the GSL reception marked (Tender for Bi Annual Rate Contract for supply of Medicine) or may be sent by post to the DGM (HSE), Goa Shipyard Ltd, Vasco Da Gama, Goa be submitted or reach the above mentioned person on or before 15:00 hrs., on 12/02/2019. If the third bigger envelope is not sealed and marked as above, GSL will assume no responsibility for the misplacement or premature opening of the bid. Any Bid received by the Employer after the deadline for submission of Bids prescribed as above will be returned unopened to the bidder. In the first stage the Techno-Commercial Offers shall be opened at 1600 hrs. on 12/02/2019 in the presence of Tenderers or their authorized representatives present at the time of tender Opening.

4 The Price Offers of only those parties who are techno commercially cleared shall be opened at time and date to be notified separately. GSL reserves the right to accept or reject any or all the tenders without assigning any reason. ii) Price Bid (Part-II), containing ONLY the maximum discount on MRP which shall be uniform and applicable all medicines surgical items. Offers received in single bid may be ignored. 15. Your quoted rate should remain firm till execution of order. 16. No enhancement of rate for whatever cause is allowed once the offer is accepted and the order is placed. Withdrawal from the quotation after it is accepted or failure to make the supply within the scheduled period will entail cancellation of order. 17. GSL reserves the right to accept or reject all or a part of any tender at its discretion following GSL purchase policy & Government guidelines. 18. Offers from bidders who do not meet our qualifying criteria will be rejected. 19. For any other clarification, please contact CMO, GSL 20. Basis of Price:- Overall Percentage (%) discount on printed prices is to be quoted in the place meant for the overall discount of the price schedule & shall remain firm for entire contract period of Two (02) years. Evaluation shall be based on overall maximum percentage (%) discount on printed price/mrp (inclusive of all taxes & duties) offered by the bidders. 21. Certification of Price :- Supplier must certify on the bills that the rates charged, are as per company's printed Price allowing agreed overall percentage of discount (on total Price i.e. Price inclusive of Taxes & duties as applicable,) 22. Supplier will have to submit two copies of bills along with the certification. 23. DISTRIBUTION OF CONTRACT :- The Rate contract may be finalized with Vendors, who can match H1 discount rate. The L1 supplier shall be decided, based on maximum discount percent given on all medicines (on total price i.e. Price inclusive of Taxes & duties as applicable). To get more supplier counter offer of L1 discount rate shall be given to L2,L3,L4 et al., who has quoted against this Tender and are Techno Commercially cleared. If they accept L-l rate then they will be empanelled.

5 24. VALIDITY OF CONTRACT: - The contract will be initially for 24 (Twenty Four) months which may be extended further at the discretion of GSL Management on the mutually agreed terms and conditions. However GSL reserves the right to terminate the contract at any time by giving 15 days notice. 25. NOTE: - It is confirmed that all the terms and conditions stipulated in the Tender Enquiry have been fully understood by us and acceptable to us.

We hereby convey our unqualified acceptance to the terms and conditions as stipulated in the tender. It is also confirmed that the price has been quoted in the format received with the enquiry. In the event of observance of any deviation in any part of our offer at a later date, the deviations shall stand null and void.
