2 Incentive Guide Invest in your health Your chance to earn Amplify rewards is here! All Vitality participants can win big for investing in their health, regardless of whether you elect AmTrustprovided health insurance. Once you ve reviewed this guide, log in to Vitality and start earning! What s new in? Below are a few enhancements that impact the Vitality program beginning January 1, : AmTrust-insured spouses will join Vitality beginning January 1,. The tobacco-free discount is no longer a stand-alone discount. Tobacco-free incentive requirements will now be included in the Amplify discount. Access your Vitality account by visiting amtrustamplify.com and clicking the Vitality Login button in the top right. Complete incentive requirements in your Vitality account by clicking the Rewards tab> Employer Incentive Plan. AmTrust-insured spouses join Vitality! Employees that elect employee + spouse or family coverage will have their AmTrustinsured spouse added to their Vitality account and will participate as a two adult team beginning January 1,. Both the employee and insuredspouse must complete incentive requirements in Vitality by October 1, to earn the insurance discount in

3 Key dates for your calendar Your Amplify incentive timeline Mark your calendar with these important dates, and follow along to qualify for Amplify incentives in the year ahead. JANUARY 1 JANUARY 2 FEBRUARY 20 MARCH 6-9 New Vitality year begins. AmTrust-insured spouses can now register for Vitality. Schedule your Vitality Check with Quest Diagnostics between now and October 1,. Last chance to schedule a Vitality Check for an on-site Quest Diagnostics event. On-site Quest Diagnostics events occur at large locations across U.S. OCTOBER 1 DECEMBER 31 JANUARY Deadline to qualify for discounts on 2019 AmTrustprovided health insurance. Last day to redeem $30 Fitbit coupons earned in. First 2019 bi-weekly pay day with earned discounts. Last day to complete a Quest Diagnostics screening in Visit amtrustamplify.com and click the Vitality Login button in the top right to access Vitality. Click the Rewards tab> Employer Incentive Plan to view your incentive requirements and checklist. 3

4 Tier I Rewards for all Vitality participants Who can participate? All Vitality participants, regardless of whether or not you elect AmTrust-provided health insurance, including insured-spouses. What rewards can I earn? As a Vitality participant, you can earn rewards throughout. Here s a look at what s up for grabs: Vitality Bucks Every Vitality Point you earn equals one Vitality Buck. When you increase your status, Vitality rewards you with Bonus Bucks! Redeem your bucks for gift cards and fitness devices in the Vitality Mall. You don t lose your bucks until you spend them.* How to Redeem Your Vitality Bucks Rewards tab> Redeem your Vitality Bucks BRONZE SILVER GOLD PLATINUM One Adult 2,500 Points 6,000 Points 10,000 Points +500 Bonus Bucks +2,000 Bonus Bucks +4,000 Bonus Bucks Two Adults 3,500 Points 9,000 Points 15,000 Points +750 Bonus Bucks Bonus Bucks +6,000 Bonus Bucks $30 Coupon Earn a $30 coupon toward the Fitbit device of your choice. Employees must complete the Vitality Health Review, Vitality Check and achieve Silver Status. Insured-spouses must complete the Vitality Health Review and achieve Silver Status only. Once you qualify, your coupon is valid until December 31,. How to Redeem Your $30 Coupon Rewards tab> Fitness Devices. Select any Fitbit with a green promo code. Then, apply the coupon at checkout to view your savings in Vitality Bucks or U.S. dollars. Only one in five U.S. adults meets the CDC physical activity guidelines of 150 minutes of aerobic activity and 2 days of muscle strengthening activity per week. Lack of exercise is a primary cause of most chronic diseases. Making physical activity part of your work day, like tracking your steps, helps reduce your health risks and costs. Employees who redeem Vitality Bucks for fitness devices or gift cards should be aware that the value of the redeemed item is considered taxable income. Employees are responsible for all taxes due as required by IRS regulations. More information can be found by visiting Payroll will automatically calculate the tax owed on the item you redeemed and this amount will be deducted from your pay slip. This deduction will shows a taxable gifts category on the pay slip along with the associated tax amount. 4

5 Tier 2 Amplify insurance discount in 2019 Who can participate? Employees, as well as, AmTrust-insured spouses, who currently elect AmTrust-provided health insurance, plan to elect it for 2019, or may need to elect it during 2019 due to a qualifying life event (QLE). How it works Each year, you have the opportunity to qualify for discounts on AmTrust-provided health insurance by completing incentive requirements in the Vitality program between January 1 and October 1. These incentive requirements must be completed annually by all participants to continue earning discounts in the following year Insurance discount overview Employees that elect employee only or employee + child(ren) coverage will continue to participate in Vitality as a one adult account, the same way you always have. You must complete the incentive requirements listed under the Employee section by October 1, to qualify. Employees that elect employee + spouse or family coverage will have their insured-spouse added to their Vitality account and will participate as a two adult team beginning January 1,. Both you and your insured-spouse must complete incentive requirements by October 1, to qualify. Employees should complete the incentive requirements listed under the Employee section and the insured-spouse should complete the incentive requirements listed under the AmTrustinsured spouse section. What employees need to complete to qualify Employee The employee s incentive requirements (req.) include: Req. #1: Consent yes to the Employer Incentive Settings consent box to be rewards-eligible. Req. #2: Complete your annual non-tobacco user affidavit (non-tobacco users) or 8-week Living Smoke free program (tobacco users). Req. #3: Complete your annual Vitality Health Review (VHR). Req. #4: Complete your annual Vitality Check (VC). Req. #5: Achieve Silver Status or higher. What insured-spouses need to complete to qualify AmTrust-insured spouse The AmTrust-insured spouse s incentive requirement (req.) include: Req. #1: Complete your annual Vitality Health Review (VHR). Req. #2: Achieve Silver Status or higher as a team. Don t Forget! Employees who do not complete all incentive requirements and achieve Silver Status or higher by October 1, will no longer receive the health insurance premium discount starting January

Vitality Customer Care (VCC) Customer service: or Hours of operation: Monday Friday from 8-5 p.m. CST When to use VCC: Use VCC each time you have a Vitality-specific account question. Amplify will refer you to a thirdparty Vitality Specialist at VCC for account-specific inquires to protect your privacy. Incentive Checklist in Vitality How to access: Click the Rewards tab> Employer Incentive Plan in Vitality. Then, scroll to the bottom and click get started. When to use your checklist: Vitality automatically keeps tabs on the progress you make towards completing incentive requirements. Be sure that all items have a green checkmark and complete under them by October 1,, to earn discounts on your AmTrust-provided health insurance 7
