2 1. EXECUTIVE SUMMARY FOR DECEMBER ODT SMT MEETING DECEASED DONORS AND TRANSPLANTS November was a strong month for deceased organ donation activity. There were 148 deceased (89 DBD and 59 DCD) in November, bringing the total to 148. This is a 5% increase on the same eight months last year and the number of deceased per million population (pmp) remains on track. (For information: there were 153 fewer eligible when comparing the eight-month periods.) o With a target of 1632 deceased for the whole year and 148 proceeding, 584 are needed for the rest of the year to achieve the target, equating to an average of 146 per month for the remaining 4 months of. Although we continue to see an increase in deceased when compared with the equivalent period last year, the same cannot be said for deceased donor transplants. There were 357 such transplants in November, meaning that 2577 patients have received a deceased donor transplant in the eight months from April to November 18. This is 9 fewer transplant recipients than the same period last year (<1% decrease). The number of deceased donor transplants per million population is now red. MISSED OPPORTUNITIES AND IMPACT ON CONSENT/AUTHORISATION In the eight-month period from April to November there were 256 missed referral opportunities. This is 197 fewer (-194 DCD) than the same period last year, equating to a 43% decrease. The DBD referral rate remains at 99% and the DCD referral rate has increased by 4 percentage points (pp) from 89% to 93% when comparing the eight-month periods. There were 196 occasions where a SNOD was not present for the formal organ donation discussion. This is 34 fewer (+2 DBD, - 36 DCD) than the same period last year, equating to a 15% decrease. The DBD SNOD presence rate remains at 95% and the DCD SNOD presence rate has increased by 2 pp from 85% to 87% when comparing the eight-month periods. o From the 196 occasions where a SNOD was not present, there were just 59 consents, equating to a 3 consent rate. When looking at the consent rates for the various combinations of ODR status and SNOD presence : Opt-in and SNOD: 95%; Opt-in, no SNOD: 71%; No opt-in, SNOD: 58%; No opt-in, no SNOD: 22%. IT DISRUPTION AND IMPACT ON ALLOCATION There was eleven hours of unplanned IT system disruption in November. This resulted in an occasion where the cardiac and liver allocation could not be followed as offers had to be made as per the unplanned outage SOP. The incident has been reported to IT to investigate. ABSENCE AND TURNOVER Absence is now at 2.8%, the lowest rate for over a year. The majority is short term absence. Annual turnover is now 1.9%. This is 34 fewer (+2 DBD, - 36 DCD) than the same period last year, equating to a 15% decrease. The DBD SNOD presence rate remains at 95% and the DCD SNOD presence rate has increased by 2 pp from 85% to 87% when comparing the eight month periods. Four Trusts/Boards account for nearly a third of occasions. A supplementary paper has been provided with this month's performance report. Although this is 54 fewer occasions than the same period last year, there have been six more consents. This means we achieved a 2 pp increase in the overall consent rate when comparing the eight month periods, from 65% to 67%. When breaking this down, there have been 4 more DBD approaches and 25 more DBD consents. However, there have been 94 fewer DCD approaches and 19 fewer consents. The DBD consent rate remains at 73% when comparing the eight month periods and the DCD consent rate has increased by 3 pp from 59% to 62%. (For information: there were 153 fewer eligible when comparing the eight month periods (+69 DBD, -222 DCD). The regions that have experienced a reduction are working with local units to identify the reasons.) From the 196 occasions where a SNOD was not present, there were just 59 consents, equating to a 3 consent rate. Although this is higher than the 17% consent rate in the same period last year when a SNOD was not present, it is far from the 71% consent rate achieved when a SNOD is present. For the remaining 4 months of the year, the new monthly averages needed for various milestones are as follows: With 1574 deceased in 17/18 and 148 in Apr-Nov 18, 526 are needed for the rest of the year to match last year, equating to an average of 131 to 132 per month for the remaining 4 months of. To achieve the 1618 double the taskforce baseline goal, with 148 in Apr- Nov 18, 57 are needed for the rest of the year, equating to an average of 142 to 143 per month for the remaining 4 months of. With a target of 1632 deceased and 148 in Apr-Nov 18, 584 are needed for the rest of the year to achieve the target, equating to an average of 146 per month for the remaining 4 months of. Consent rate for patients from black and Asian communities Target During April to September 18 the consent rate for patients from black and Asian communities was 38% (78/6). The ideal donor kidney project continues and plans to increase across all abdominal groups are in place. The kidney imaging project has been delayed in its implementation due to IT issues and also the volume of change planned for the SNODs this winter. The pancreas imaging project will be developed shortly and will either accompany or follow the kidney imaging project training. The first London Kidney and Pancreas collaborative meeting was held in November, positive feedback was received from all attendees, it was agreed following discussions that 3 working groups would be set up to achieve 3 potential quick wins: sharing, declines and staffing. A steering group has been convened to lead this work and the next London Kidney and Pancreas collaborative meeting will be held in Spring 19. The UK will host the 19 European Day for Organ Donation and Transplantation (EODD); the UK will also host the 23rd meeting of the European Committee (Partial Agreement) on Transplantation (CD- P- TO). The incidents are under investigation to identify preventative actions to avoid recurrence. Nine out of 25 departments within ODT are achieving 95% or over. The lowest teams are Eastern with 81% and Yorkshire with 86.3%, however they have experienced levels of absence. Eleven out of the 23 MT courses are at 95% or over. Safeguarding has a low compliance rate of 78.6% however this is due to 3 out of 14 individuals being non compliant in OD Services only. Adult Basic Life Support again shows the lowest compliance rate of 72.8% overall, with Yorkshire at 47.4% (1 out of 19 individuals non compliant), and South Wales at 38.5% (8 out of 13 individuals non compliant). Thirteen departments out of 25 have 1 compliance. Yorkshire team have a low compliance rate at 77.3% (5 out of 22 not completed) however there has been absence on the team. South East team is at 81% completion (4 individuals out of 22 not completed). Support Services has the lowest PDPR completion rate at 69.2% (4 out of 13 individuals not completed). Focus on PDPR completion is expected in the early part of 19 once more information is available on completion requirement for pay point progression. There are 15 supervised SNODs from Cohort 9 who commenced in teams on 15th October and have completed Module 1. South East, South Central and Yorkshire each have three Cohort SNODs which may be difficult to manage in terms of adequate exposure.
