2 Welcome to the University of Florida College of Medicine The College of Medicine provides a broad and comprehensive employee benefit program to faculty members of the University of Florida. The retirement programs are planned to provide maximum financial security during retirement years. Selecting health and retirement benefits is your first step in taking advantage of the benefits provided to you by the College. Please review this booklet carefully to see what best meets your needs. You may also contact the Benefits Manager, Jamie Weinar at jweinar@ufl.edu or (352) Things to Consider: You have 60 days from your date of hire to enroll in your benefits. UFSelect plans are enrolled in using myufl. State of Florida plans are enrolled in using the People First website. Your 403b deductions will start on your first paycheck, however in order to select the funds that you want your money invested in, you must enroll with one of the vendors for each plan. In order to enroll your dependents, you will need their DOB and SSN. If your dependent doesn t have a SSN, please notify the COM Benefits Manager.

6 Dental Insurance As an employee of the University of Florida, you are for the Eagles Select dental reimbursement plan. 7 Life Insurance You have the option to purchase additional life insurance for you and your covered dependents, through both the UFSelect Plan, as well as through the State of Florida. Rates are age banded. A free basic group term life insurance benefit of $25,000 is available to all full-time employees through the State of Florida. All employees enrolled in basic term life insurance may elect and pay for dependent spouse coverage. Disability Insurance As a faculty member of the College of Medicine, you are provided with a long term disability insurance policy at no cost to you. The coverage provides a monthly income benefit of 60% of your basic monthly earnings (tax-free) up to a maximum of $15,000 per month. There is a 180 day elimination period. This is a specialty own occupation policy. *Limitations and Exclusions apply as stated in the booklet Certificate and Master Contract. * For more detailed information and to view the policy document, go to You also have the option to purchase additional disability insurance for you through the UFSelect plans. There are 30 day and 90 day plans to choose from. Additional Benefits UFSelect Preferred Legal Plan- low-cost access to comprehensive legal assistance, advice, and representation on different types of legal services. Baby Gator Child Development Center at Newell Drive- was established as a partnership with the Colleges of Medicine and Public Health and Health Professions. Faculty and Housestaff Members whose children are between the ages of 6 weeks and 5 years are for enrollment at reduced tuition rates. Gator Care Now- Same-Day care for all UF and Shands Employees. Located at the Internal Medicine at Medical Plaza. If you are feeling under the weather, you can seek same day care on campus. 8 Retirement State University System Optional Retirement Program (ORP) The ORP is a defined contribution plan qualified under section 403(b) of the Internal Revenue Code administered by the Florida Department of Management Services. The department contracts with five Investment Provider Companies to offer a variety of mutual funds and annuities to university personnel. The benefits are determined by performance of the participants investment choices funded by employer contributions and employee contributions. 9 Additional Voluntary Savings Plans You are to participate in voluntary savings plans via payroll deductions. Employees can contribute to both a 403(b) plan and the 457 Deferred Compensation plan, up to each plan s separate IRS limit. 11 Time Away From Work Vacation (annual) Leave- provided for 12 month faculty. This leave should be taken when it is necessary for a faculty member to be absent from assigned duties and responsibilities for reasons other than for illness. Full-time faculty members on 12 month appointments accrue annual leave at the rate of hours biweekly (roughly 22 days/year). A maximum of 352 hours of annual leave may be accumulated. All requests for vacation should be submitted to the chair or supervisor as far in advance as possible. Additional information may be obtained from your department s administrator. Upon resignation or retirement, an employee may have a lifetime cash out up to 200 hours of annual leave. Sick Leave- Full-time faculty members earn 4 hours biweekly (roughly 13 days/year) of sick leave. There is no limit on total hours accrued. Sick leave must be earned before being taken. Additional information may be obtained from your department s administrator. There is no cash-out option for sick leave. December Personal Leave (PDL)- Full-time faculty members earn four additional days of leave which are credited to their account in December of each year. These days are typically used between December 26th- 31st. However, due to department staffing needs, these days will be available for use until the end of the current fiscal year (June 30th.).
