1 ADDENDUM No Paving Program Village of Milford The following changes, additions, and/or clarifications to the Contract Documents shall be incorporated in said documents and shall be allowed for in the unit prices bid by the Contractor such that the unit prices indicated in the Bid Form shall represent the conditions as set forth in the Contract Documents and this addendum. The bidder shall acknowledge the receipt of this addendum on page BF-1 of the Bid Form and shall staple this addendum into the Contract Book. The Bidder shall also return, via , the acknowledgment sheet located on page 13 of this addendum. This Addendum contains 13 pages. ADDITIONS AND CHANGES TO THE SPECIFICATIONS SECTION: BID FORM 1. Delete BF-3, BF-4, and BF-5 and replace with the attached pages BF-3 Add1, BF-4 Add1, and BF-5 Add1. SECTION: SUPPLEMENTAL SPECIFICATIONS 1. Add the following to the SUPPLEMENTAL SPECIFICATIONS, SECTION 14. MACHINE GRADING: The CONTRACTOR shall note that the five (5) stations (sta) of Machine Grading included in Division A are provided as provisional quantity for Division C and Division D if determined to be necessary by the ENGINEER. 2. Delete SUPPLEMENTAL SPECIFICATIONS, SECTION 27. HURON STREET PATCH and replace with the following: Quantity has been added to this contract to saw cut and remove an approximate 10-foot by 10-foot area of HMA near the intersection of River Drive and Huron Street, just east of the Huron Street Bridge, as directed by the ENGINEER. Any base removal and replacement as determined by the ENGINEER in the field shall be paid for as Subgrade Undercut. Mobilization and Traffic Control for this patch work shall be considered as incidental to the overall Mobilization and Traffic Control pay items. Any preparation work required for the patch work as well as all project clean up and associated restoration efforts shall be considered incidental to the overall project. Page 1 of 13

2 3. Add the following sections to the Supplemental Specifications: 28. EXCAVATION, EARTH The CONTRACTOR shall note that the quantities provided in the Bid Form for pay item Excavation, Earth are intended for sidewalk and ramp construction as well as the road re-construction area on Tennyson. 29. AGGREGATE BASE Aggregate Base, 21AA Limestone quantities have been calculated for a thickness of six (6) inches in the reconstruction section of Tennyson Drive. This quantity includes providing six (6) inches of aggregate base under all proposed curb and gutter. While this thickness has been calculated and accounted for in the Bid Form, a geotechnical testing firm will be on site to provide density testing and the ENGINEER may determine if lesser thicknesses are required in specific sections or if additional sections are in need for replacement. Additional Aggregate Base quantities have been provided in Division A and are intended to be provisional and used upon the direction of the Engineer where additional aggregate base is needed in any Division of the project. Removal of any and all aggregate base, as directed by the ENGINEER or noted on the plans, shall be paid for as Excavation Earth. 30. AGGREGATE SHOULDER The Aggregate Shoulder pay item is intended only to be used where necessary, as determined by the ENGINEER and the VILLAGE. If the CONTRACTOR is able to mill and overlay the proposed roads without disturbing the existing shoulder or established grass, no aggregate shoulder shall be placed. If areas are disturbed, an aggregate shoulder will be installed per plan detail. Restoration with topsoil, seed, and mulch for the shoulder area is incidental to the shoulder construction. The Bid Form conservatively includes quantities for the entire length of the all mill and overlay roads; however, it is not expected that this entire amount will be used. 31. SLOPE RESTORATION Slope restoration is included in the Bid Form for the total project. It is the VILLAGE s intent to pay contract quantity for restoration as an incentive to the CONTRACTOR s to minimize impact outside of the road area. Page 2 of 13

Delete Pay Item Hot Mix Asphalt (HMA) Pavement from the METHOD OF PAYMENT and replace with the following: Hot Mix Asphalt (HMA) Pavement The completed work as measured for HOT MIX ASPHALT (HMA) PAVEMENT will be paid for at the contract unit price for the following contract items (pay items): Pay Item HMA, 13A HMA, 36A Hand Patching HMA, Driveway Pay Unit ton ton ton ton HMA pavement for mainline road paving shall be measured in tons based upon certified weight delivery tickets. HMA pavement construction for driveway approaches if needed and only as directed by the engineer shall be measured in place by square yards and shall be based upon the pavement length and width measurements as determined by the Engineer. Longitudinal measurements shall be made along the centerline of pavement from end to end. Transverse measurements shall extend from edge of pavement to edge of pavement. The Contractor must provide certified weight delivery tickets to the Construction Observer. Pay items that are designated as driveway shall apply to driveway approach paving work. Pay items that have no designation shall apply to main line road paving. The contract unit price shall be payment in full for all labor, materials, and equipment necessary to construct the HMA pavement in accordance with the plan details. Furnishing and applying bond coats, pavement compaction, and protection of the work shall all be considered as incidental to HMA pavement construction. The application method and coverage rate shall be in accordance with the manufacturer s recommendations. ADDITIONS AND CHANGES TO THE PLANS 1.
