Introduction The following report covers health and safety matters within Thirteen Group from 1 April 2016 to 31 March We can continue to report with confidence that statistics contained in this report are a true and fair reflection of the company s health and safety performance over the past twelve months. The Group reports statistics on the following areas: Property Services Thirteen wider staff Thirteen Care and Support Contractors Member of the public (MOP) Key activities/changes Completed fire risk assessment programme. Revised health and safety consultation groups health and safety committees and working groups, all of which are operating effectively. Provided lone working training to over 350 front line employees Reviewed lone working requirements for neighbourhood operation teams. Maintained employee protection register (EPR) system Continued to develop health and safety training requirements Completed legal compliance audits and inspections Identified new external health and safety management system Continued to provide competent and accurate advice and guidance as required Contributed to property compliance review. 2. Accidents/Incidents within Thirteen Group The total number of accidents for 2016/17 is 90, a decrease of 3.2% The total number of incidents reported for 2016/17 is 142, an increase of 145%. This increase reflects the external training on lone working and the continuous promotion on reporting instances of verbal abuse. Further information can be found in Appendix A. The Group reported 16 accidents under RIDDOR; an increase of 33.3%. 14 accidents are classed as over seven day injury. One accident is classed as specified injury (fractures). One dangerous occurrence Health and safety accident/incident statistics are reported through the health and safety committee and working groups. 3

5 Slips, trips and falls are the main accident types recorded within this year, followed by injuries whilst handling, lifting or carrying. This is the same as the previous year. The Group will continue to try and reduce these figures through awareness training and toolbox talks Incident Type Statistics
6 During this year the company reported 73 instances of violence and aggression. Instances are broken down into: 63 reports of verbal abuse Five reports of aggressive behaviour Five reports of physical abuse/assault Actions taken against tenants or members of the public include intervention from the police, notices served and formal letters sent requiring future contact to be in writing only. Employee Protection Register (EPR) Following an audit by Mazaars a thorough review of the EPR register has been completed. During this year 46 markers were entered onto the EPR. The register is regularly monitored to ensure compliance with the procedure. Control measures identified: Home visits to be carried out in pairs No home visits unless managerial approval Office visits in secure interview rooms Treat with caution 3. Health and Safety Executive (HSE) statistics for information HSE statistics 2015/16 reported: There were 72,702 non-fatal injuries to employees reported by employers (which only includes over-seven-day injuries and specified injuries). (RIDDOR) (Note: Non-fatal injuries to employees are substantially under-reported by employers, with current levels of reporting estimated at around a half; and the reporting of injuries to the self-employed a much lower proportion) Of these injuries, the most common kinds of accident were caused by handling, lifting or carrying (20%), slips and trips (19%) and being struck by moving objects (10%). There were 76,054 non-fatal injuries reported in the previous year. The rate of non-fatal injury to employees reported by employers (which only includes over-seven day and specified injuries) fell in 2015/16, continuing the long-term downward trend. 6
7 In 2015/16, an estimated 621,000 workers sustained a non-fatal injury at work. Of these injuries, 152,000 led to over seven day s absence from work and 469,000 led to up to seven days absence. HSE report 30.4 million days were lost due to work related illness and nonfatal workplace injuries in 2015/16. Injuries and new cases of ill health cost society an estimated 14.1 billion in 2014/15. This excludes long latency illness such as cancer. 34% injury and 66% ill health are the types of incident reported for this period. HSE statistics for this period report that around 33% of new self-reported illness conditions were musculoskeletal disorders. 42% stress, depression or anxiety. 25% other type of illness. Mental ill health gives rise to most working days lost according to the surveillance scheme. 4. Health and Safety Inspections/Audits Legal compliance checks and audits were completed throughout the year and copies of all inspection and audit reports are available. Risk-based programmes will be further developed for 2017/18 following the introduction of the new health and safety management system. 5. HSE/Local Authority/Fire Service Activity HSE investigated a dangerous occurrence that was reported under RIDDOR in December Potential asbestos exposure at 147 Thorntree House. Full internal investigation undertaken. HSE inspectors met with the health and safety team in January 2017 and were satisfied with the findings of the internal investigation report which detailed recommendations/improvements. No further formal action to be taken from the HSE. The Group awaits the cost of fees for intervention from this visit. Ongoing investigation by the HSE on Parkfield site Stockton. HSE investigating 2 RIDDOR notifications. Keepmoat appointed Principal Contractor for site and are assisting the HSE. No formal action from any fire brigade inspections. No local authority visits. 6. Prosecutions / Notices No prosecutions or notices have been served on the Group this year. We will continue to monitor the HSE key enforcement activities and notify all internal parties of any future HSE led initiatives. 7
8 7. Health & Safety Committee and Working Groups The Health and Safety Committee and working groups are established and operating effectively. The number of health and safety working groups went from three to two in this year. Thirteen Care and Support joined the wider group health and safety working group. The formation of the working groups and committee continues to be pivotal in helping to improve awareness and performance of health and safety across the Group. 8. Fire The full fire risk assessment programme was completed for 2016/2017. Where applicable remedial action plans were forwarded to the Compliance Team to arrange works to ensure the Group remains compliant. The Group continues to form strong working relationships with fire. A Property Services compliance review was carried out and completed in February 2017 and the results of this review were conveyed to the Thirteen Board: fire safety was part of this review. The review recommended that in future fire risk assessments be completed by Property Services. A full programme for 2017/18 has been established, based on the new frequency ratings. During 2017/18 the undertaking of fire risk assessments will be handed over to Property Services. The Health and Safety Team will continue to work closely with Property Services during handover and provide support/guidance where required. During 2017/18 the Health and Safety Team will implement a programme of audits on the fire risk assessments and other compliance areas. Findings will be reported on a regular basis. 9. Overview of Health and Safety Training The following chart shows attendance for a number of internally facilitated health and safety courses. These courses have been delivered by the Training Co-ordinator. 8
9 The Property Services Training Co-ordinator joined the Health and Safety Team in this year. A full review of toolbox talks has been carried out. Changes to delivery of toolbox talks for 2017/18 will be implemented with the Training Co-ordinator focusing on more in depth health and safety training topics. A new Health and Safety e-learning provider was sought to provide ongoing e-learning training from 2017/18. All new employees will be required to complete this training within the first four weeks of employment. All existing employees will be required to complete this on a programmed basis. Modules to be covered: Getting to Grips with H&S Working Safely with computers (including workstation assessment) Lifting and carrying Avoiding violence Meeting threat from fire Two members of the Health and Safety team have been trained to deliver the full first aid course (three days) in house. This includes emergency first aid training and defibrillator training. Training programme commenced July Key Activities for 2017/18 Implement a robust H&S Audit system utilising the new H&S management system and continue to work to ensure compliance with HSE best practice. 9
10 Continue to provide competent and accurate Health and Safety advice and guidance. Support Property Services to ensure compliance with Working at Height Regulations. Complete and review the fire risk assessment programme for the Group s multi-occupied buildings. (Includes formal handover to Property Services) Review lone working systems for Property Services and maintain existing systems across the rest of the Group Deliver Phase two of lone working training to Property Services Deliver first aid training in-house Handover delivery of Health and Safety toolbox talks to Property Services Ensure all relevant policies and procedures are in place and reviewed at agreed intervals Investigate/co-ordinate accidents/incidents/near misses and associated ill health notifications and identify appropriate remedial action. Develop Channel for Health and Safety information Support People Team in delivery of Wellbeing Strategy Participate in consultation for the reviews of all existing Health and Safety regulations Attend various external working groups on Fire Safety and Health and Safety. 10

During this period, no fatalities and only one specified injury occurred. Thirteen Group did not receive any prohibitions or improvement notices. The Health and Safety Executive (HSE) completed an investigation into the Asbestos related dangerous occurrence that occurred at 147 Thorntree House on the 6th and 7 th December The Health and Safety team met with the HSE on the 4 th January to review the incident. The Health & Safety team carried out a full investigation with all internal parties and provided the HSE with a detailed report on the 20 th January The Group awaits formal notification for fees for intervention on this incident. 15 Recommendations Staff awareness programmes. Individual teams to undertake refresher training and toolbox talks on a regular basis, especially in the following areas. 1. Manual Handling 2. Slip, Trip and Fall incidents 3. Lone Working 4. Asbestos awareness. Asbestos Team to organise to train all trades and supervisory staff on how to read the different asbestos surveys in the Group s portfolio. Health and safety team will continue to promote the importance of reporting all workplace accidents/incidents using the company 001 accident/incident reporting form. This will be undertaken by means of advertising on channel, attending department s team meetings and notice boards bulletins. A new near miss reporting document will be adopted and rolled out across the group for future use. 4
