OF INDIA UNDERTAKING) MANALI, CHENNAI NOTICE INVITING TENDER Stack Emission Monitoring Survey - AMC TENDER No: QCL/TS/WEB/Stack Emission Monitoring Survey AMC/001 Date: SUMMARY Madras Fertilizers Limited stack emissions are being monitored regularly. The monitoring survey of Urea Demister Outlet and NPK C Train common stack outlet are conducted once in a month with prior planning. The survey reports are to be submitted regularly to TNPCB and CPCB. Hence we invite the agencies approved/certified by CPCB/MOEF for submitting their lowest sealed offer for conducting monthly Stack Emission Monitoring Survey as detailed below: Urea Demister O/L for Temperature, Velocity, Volume of gas discharged, Particulate Matter and Ammonia NPK C Stack for Temperature, Velocity, Volume of gas discharged, Particulate matter and Fluoride. We invite your lowest sealed offer for the above jobs as per scope of job (section I) and terms and conditions. The sealed tender should be submitted as indicated below. Part 1 - The tenderer shall submit the technical Bid having proof for the certification/approval by CPCB/MOEF, previous similar jobs carried out by them and the technical details regarding the capability to carry out the above job in the prescribed format (Section II) in a separate cover super scribed as Technical bid for TENDER No: QCL/TS/WEB/Stack Emission Monitoring Survey AMC/001 Date: Part 2 - The tenderer shall submit the price bid in a prescribed format (Section III Price schedule) in a separate cover super scribed as Price bid for TENDER No: QCL/TS/WEB/Stack Emission Monitoring Survey AMC/001 Date: Cover containing Part 1 and Part2 shall be submitted in single cover super scribed as Quotation for TENDER No: QCL/TS/WEB/Stack Emission Monitoring Survey AMC/001 Date: latest by 16:00 hours on 11/04/2019 at the office of The Dy. General Manager - Prod & TS, Madras Fertilizers Limited, Manali, Chennai

2 Price Bid Rate must be quoted in prescribed Format (section III). The rate should be in both words and figures. The validity of the offer shall be for 90 days from the last date of the submission the offer. Price shall be firm till completion of the job and not subject to escalation of any kind. Bid Opening: The technical bids will be opened on 12/04/2019 at 14:00 hrs. Upon finalizing the qualification in Technical bid, the price bid will be opened. Bid evaluation: Successful vendor will be arrived on L1 basis. Job Completion Period Stack Emission Monitoring Services to be done every month on prior planning with QC Lab for plant conditions. Analysis Report to be submitted subsequently within 10 days to QC Lab. Liquidated Damages (LD): In case of delay in survey after the approval from MFL and delay in submission of reports beyond 10 days liquidated Damages (LD) will be levied at the rate of 0.2% per day of the delay or part thereof, subject to a maximum of 5% of the total contract value. Subletting & transfer Tenderer shall be solely responsible for rendering any or all the services. He shall not sublet / transfer / assign the contract or any part thereof to others. All their dealing with third parties shall be without reference to MFL. Arbitration Any or all disputes arising out of the contract / agreement shall be settled by mutual discussions and in the event of failure to do so, such dispute (s) shall be referred to a sole arbitrator, who will be appointed by mutual consent for settlement of such dispute (s) and whose decision shall be final and binding. In the event of failure to appoint such a sole arbitrator, with mutual consent, then the sole arbitrator will be appointed through the high court of judicature at Madras. Subject as aforesaid, the arbitration and conciliation act 1996 shall apply to the arbitration proceedings under this clause and such arbitration shall be in English and take place in the city of Chennai, Tamil Nadu.

3 Statutory Norms: Personal protective Equipments like helmet, shoes, gloves, goggles etc. are to be provided to the workmen at site and necessary precautions are to be followed to ensure safety to the workmen and also to the equipment. ESI or accidental coverage obligations for the persons visiting MFL for survey are under the tenderer scope. Payment Terms: Payment will be made within 30 days from the date of submission of bills after the survey and submission of bills for the actual survey done subject to the operation of the plants along with the survey report every month. Madras Fertilizers Limited reserves the right to accept or reject any tender in a whole or part, without assigning any reason whatsoever. In case of part acceptance by MFL, the tenderer shall perform the same at the quoted rate. DGM Prod & TS

Job will be carried out at MFL site on any working day. 2. Your work men fulfill all statutory obligations and comply with provisions of Labor Regulations Act All your workmen shall be adequately covered by ESI, PF and evidence thereof shall be produced before commencement of this work 4. You should strictly adhere to MFL safety rules and regulations 5. Work permit shall be obtained during each shift from MFL authorities 6. All required tools and tackles will be vendor scope and required structure material also vendors scope 7. Since the job depends on the plant operations, every time before commencement tenderer to take permission from MFL. 8. MFL will give free water and power 9. You have to report to the MFL person concerned and collect the samples/conduct the survey under his supervision only.
