1 Sun Life Malaysia U Mobile GOLIFE Share Your Love Campaign Terms and Conditions 1. GOLIFE Share Your Love Campaign ( Campaign ) is jointly organised by Sun Life Malaysia Assurance Berhad ( SLMA ) and U Mobile Sdn Bhd ( U Mobile ) which will commence at 00:00 on 23 August 2018 and will end at 23:59 on 09 September 2018 ( Campaign Period ). 2. All entries received after the Campaign Period will be automatically disqualified. 3. By participating in this Campaign, you agree to be bound by all the terms and conditions below ( Terms and Conditions ). 4. Campaign Eligibility a. This Campaign is open to selected active U Mobile Postpaid subscribers who hold individual accounts (Corporate accounts not included) and who: (i) are 18 years of age and above; (ii) are GOLIFE policy holders; and (iii) received the Campaign invitation from SLMA via SMS, MMS or with a unique link ( Invitation ). Active means able to make and receive calls. b. SLMA or U Mobile s employees and their immediate family members are not allowed to participate in this Campaign. c. SLMA or U Mobile reserve the right to reject any entries or disqualify any participant without assigning any reason. 5. Campaign Mechanics a. To participate in this Campaign, the following steps must be completed during the Campaign Period: (i) You must: (A) share the Invitation with any of your friends or family who fulfill the following criteria ( Referee ); (1) Active U Mobile Prepaid or Postpaid subscribers who holds an individual account (Corporate account not included); (2) Malaysia citizens between the age of 18 and 45; and (3) not an existing GOLIFE policy holder. (ii) The Referee must subscribe to GOLIFE5 via the unique link in the Invitation ( Opt In ). The unique link will be used to determine the number of referees each participant referred.

When your Referee complete the step listed in paragraph 5(ii) above and receives an SMS, MMS or notification from SLMA indicating their successful Opt In ( Opt In Notification ), you will deemed to be a participant of the Campaign. No entry form is required. c. Each Referee is: (i) (ii) only entitled to sign up to GOLIFE5 once during the Campaign Period; and allowed to purchase 1 GOLIFE5 (either via this Campaign, via U Mobile SelfCare or MyUMobile App) at any one time. SLMA may terminate any of the GOLIFE5 at its sole discretion if the Eligible Referee purchase more than 1 GOLIFE5. d. Each GOLIFE 5 is valid for a period of 1 month from the date of the Opt-In Notification. GOLFIE 5 will be automatically renewed for subsequent periods of 1 month unless the Referee opt-out by sending opt-out via SMS to ( Opt-Out ). e. GOLIFE 5 is provided by SLMA subject to the applicable terms and conditions imposed by SLMA which are available at U Mobile makes no representations nor warranties on GOLIFE Complimentary Premium a. The monthly premium payable for GOLIFE 5 is RM5 ( Premium ). However, upon receipt of the Opt In notification, Referee shall receive a 2 months complimentary GOLIFE 5 subscription ( Complimentary GOLIFE5 ) which will be valid until 31 October 2018 from the date of Opt-In Notification ( Validity Period ) b. Prior to the expiry of the Validity Period, the Referee will receive an SMS notification from U Mobile of the expiry date of the Complimentary GOLIFE 5 and of the Referee s Opt-Out option ( Expiry Notification ). c. If the Referee does not Opt Out prior to the date specified in the Expiry Notification, the Referee will be charged with the Premium from 1 November 2018 onwards subject to the terms and conditions of U Mobile s carrier billing for GOLIFE 5 made available at The Premium will be deducted from the Referee s Prepaid credit, if the Referee is a Prepaid subscriber or reflected in the Referee s next Postpaid bill, if the Referee is a Postpaid subscriber. d. The Referee will continue to be charged with the Premium and GOLIFE 5 will be automatically renewed on a monthly basis unless the Referee Opt Out. 7. Campaign Rewards

3 a. At the end of the Campaign Period, you may receive the following rewards: No. Type of Reward Criteria to be fulfilled to receive the Reward i. Complimentary e-voucher worth RM10.00 from Ecart Services Malaysia Sdn Bhd ( Lazada ). The e- voucher can be used to purchase merchandise at from subject to Lazada s terms and conditions made available at For every Referee who successfully Opt In. ii. Grand Prize of 1 unit of iphone X (a) if the total number of Referees who Opt In during the Campaign Period is 500 or more: (1) SLMA will shortlist 10 participants with the highest number of Referees who Opt In as the shortlisted participants ( Shortlisted Participant ); (2) SLMA will call the Shortlisted Participant with highest number of referees and ask 2 campaign related questions; (3) if the Shortlisted Participant manages to answer the question correctly, the Shortlisted Participant will be declared as the grand prize winner; and (4) if the Shortlisted Participant is unable to answer the question or cannot be contacted by SLMA, SLMA will contact the Shortlisted Participant with the second highest referral who will be asked 2 campaign related questions. If the Shortlisted Participant manages to answer the question correctly, the shortlisted participant will be declared as the grand prize winner. For clarity, if the total Referees Who Opt In is less than 500, the grand prize will be forfeited. b. SLMA will notify the winners via SMS within 45 working days after the end of the Campaign Period of their entitlement to receive the vouchers and grand prize. SLMA may use any other method or medium it deems fit to notify the winners.

4 c. The grand prize will be couriered to the winner s preferred mailing address. U Mobile disclaims any liabilities for any loss or damage of the grand prize during the courier process. d. The voucher and the grand prize is non-transferable and non-redeemable for cash in full or in part. e. The voucher and the grand prize is given on an as is basis and U Mobile is not responsible for the merchantability of the voucher and prize. U Mobile or SLMA reserves the right to substitute or the grand prize with that of similar value at any time without prior notice. f. The winners must ensure that their respective Prepaid and Postpaid accounts are Active during the winner selection process. g. Each participant and Referee shall only be entitled to win 1 prize each in this Campaign. General Terms and Conditions 1. SLMA or U Mobile s decision on any matter relating to this Campaign are final and binding on all participants. If any matters arises which are not covered in these Terms and Conditions, they will be determined solely by SLMA or U Mobile. 2. SLMA or U Mobile may change any of these Terms and Conditions (including the terms and conditions applicable to GOLIFE5) and may terminate, suspend this Campaign or extend the Campaign Period. Any such change will be announced on SLMA or U Mobile s website. It is the participants responsibility to check these Terms and Conditions on SLMA or U Mobile s website and if you do not agree to any of the changes, you must immediately cease participation in the Campaign. 3. By participating in the Campaign, all participants : - i. consent for SLMA or U Mobile to collect, record, hold, store, use and disclose their personal information for purposes which are necessary or related to the participation in the Campaign; ii. iii. agree that their personal information shall be used by SLMA for the purpose of processing GOLIFE5 insurance application; and consent for U Mobile to disclose or publish their personal information such as their names and identities and any general information that U Mobile sees fit about the participants in any media, marketing or advertising materials.

The subscription and use of U Mobile s Postpaid and Prepaid mobile services are subject to U Mobile s standard Postpaid and Prepaid terms and conditions and where applicable, these Terms and Conditions. In the event of any inconsistency, U Mobile s standard Postpaid or Prepaid Terms and Conditions, as the case may be, will apply to the extent of such inconsistency. 5. The Campaign and these Terms and Conditions are governed by the laws of Malaysia. The participants agrees that the Courts of Malaysia have jurisdiction over all matters arising from this Campaign.
