1 Dear Wellness Provider, Yarmouth Wellness Fair Saturday, April 8, :00 am to 3:00 pm Rowe School 52 School Street, Yarmouth, Maine The Yarmouth Chamber of Commerce and Yarmouth Community Services would like to invite you to be an exhibitor at the 12 th Annual 2017 Community Wellness Fair. Our mission is to showcase local holistic and integrative healthcare businesses, to foster an environment of discovery and education, and to support personal and community wellness. This Fair, noted as one of the most popular wellness fairs in Maine, provides an opportunity for people to learn about many diverse health and healing practices via presentations and practical demonstrations. Exhibitors at the fair will include holistic and integrative healthcare providers that offer a wide range of healing and health-enhancing services. The event is open to the general public and admission is free. The exhibitor fee, unchanged for over ten years, is $75 for Chamber of Commerce members or $125 for non- Chamber members. The Fair is limited this year to 40 exhibitor spaces. Exhibitor Fee includes: 8 x8 space with one table, if needed, and 2 chairs. A healthy boxed lunch. Additional lunches available at $10 each, paid with your application. A listing on the Fair web page and in the Fair brochure. The event program will be a professionally printed, full color publication that includes a layout of booth spaces and information about our speakers/presentations. Advertising and public calendar notices will be placed in The Notes, The Forecaster, Yarmouth Community Services Activity Guide and there will be feature articles in several of these publications as well. The Yarmouth Chamber of Commerce hosts a Wellness Fair page on their website, including up to date information and a listing of all Fair Exhibitors. Additionally, posters will be placed in our regional area promoting the event, and we encourage your business to share in our outreach via in Social Media. NEW THIS YEAR Exhibitors do not have to be setup until 9:45. This later setup time is in response to requests to shorten the length of the fair. A raffle featuring goods and services from the exhibitors has been very popular in the past, offering another opportunity for exposure. This year we will again have the raffle with the beneficiary being the YCAN and Yarmouth Food Pantry. Raffle items and ticket containers will be placed at exhibitors tables, rather than in the rotunda. Instead of selling raffle tickets, people will receive a ticket for each donation of money or food they bring. They can then place their ticket(s) in whichever raffle they choose. Please help us Stuff-the-Bus, by donating a product or service valued at $50 or more. 5 SELECTION CRITERIA AND RULES Applicants will be considered according to the following criteria and must adhere to the following policies: 1. How well the applicant s service or product fit into the scope of providing integrative and holistic healthcare options in the greater Yarmouth area. 2. The qualifications of the individuals applying. The applicant must possess usual and customary credentials for the type of service/product being presented, and in accordance with the laws of the state of Maine and local ordinances. 3. The number of applications received from providers of similar services and products (we will limit the number of similar offerings in order to maintain variety). 4. Some governing limits are based on the date the application is received and preference given to Yarmouth Chamber of Commerce members and applicants from local communities surrounding Yarmouth, Maine. Applications will be considered from out-of-town providers of products and services from which our community could benefit. 5. The applicant must assume legal and financial liability at the fair and provide proof of appropriate liability insurance coverage. 6. Exhibitors must be set up and ready for the public by 9:45am and must staff their area until 3pm, taking breaks as needed. Exhibitors may not break down their booth until 3:00 or leave until after 3:00. Exhibitors are responsible for removing all materials brought into the fair. 7. In keeping with the spirit of the Wellness Fair, we ask vendors who wish to bring snacks or samples for attendees to offer healthy alternatives rather than candy. 8. Exhibitors may distribute free samples to fair visitors but may not sell product or services. The Wellness Committee reserves the right to remove materials deemed inappropriate or not in keeping with the mission of the fair. 9. Exhibitor s booth can only be represented by the business/person(s) listed on their application. 10. Exhibitors cannot sell any items or mention any distribution opportunities. There will be no mentioning of distribution opportunities of any multi-level marketing products or services. 11. Unless your space is on a wall, table top signs are limited to 4 feet. You can request a table on the wall but it is not guaranteed. For examples on what is accepted, please click here to check out pictures from previous years. 12. Only 2 people at your booth at all times unless you purchase an additional booth. If you have multiple practitioners, we suggest signing up in blocks or mingling throughout the event.

6 2017 YARMOUTH WELLNESS FAIR HOLD HARMLESS AGREEMENT NAME: ADDRESS: COMPANY NAME: PHONE #: By executing this agreement, the undersigned agree(s) for himself, herself, itself and its/their successors, heirs and assigns that participation in the Yarmouth Wellness Fair, as described herein, shall be at the risk of the undersigned, and that the undersigned hereby releases and forever discharges and expressly agrees to indemnify and hold harmless the YARMOUTH CHAMBER OF COMMERCE, the YARMOUTH WELLNESS FAIR, the TOWN of YARMOUTH and the officers, directors, employees, agents and members of those organizations, together with their successors and assigns of and from all debts, demands, actions, causes of action, suits, dues, sum and sums of money, accounts, reckonings, bonds, specialties, covenants, contracts, controversies, agreements, promises, doings, omissions, variances, extents, execution and liabilities whatsoever including, without limiting the generality of the foregoing, claims for contribution, exoneration or indemnity, or any other thing whatsoever which might arise from the undersigned participation in the Yarmouth Wellness Fair. The undersigned acknowledges that he/she has read and understood the foregoing, and that he/she has been advised to consult with an attorney if he/she has any questions and further acknowledges that he/she must abide by all the RULES of the Yarmouth Wellness Fair outlined in Selection Criteria and Rules.
