3 ACCESS PROCEDURES Fr MURANO GRANDE During Opening Perid In an effrt t make life mre pleasant and rderly fr the residents during the cmpletin f cnstructin and the initial mve in perid, we ask the unit wners and their cntractrs t abide by the fllwing rules. The unit wner is respnsible t give the cntractr a key t the unit and a key fb fr the elevatr. MURANO GRANDE persnnel cannt perate the elevatrs, nr pen r lck units fr cntractrs. The MURANO GRANDE Cndminium Assciatin, Inc. (the Assciatin ) shall als nt be respnsible fr any drs t units left unlcked by anybdy, r fr damage r lss t any units r their cntents. T the extent that an Assciatin emplyee des in fact pen a unit fr smene at the request f an wner, r smene cntracted by an wner, this is nly dne t assist the wner and des in n way imply that the Assciatin assumes any liability. All unit wners are respnsible fr any damage t the Assciatin prperty r infractin f the Assciatin Rules and Regulatins by anybdy cntracted by them. Safety f persns and prperty is a very high cncern fr all. Service Cntractrs will be prvided with a designated area by the lading dck at the Suth side f the prperty t lad and unlad nly. N parking will be prvided t Service Cntractrs in the Assciatin s prperty. All cntractrs and their wrkers must use the receiving entrance nly. Upn due authrizatin, the cntractrs will be issued pht ID badges. The pht ID badges are ne time use. Any wrker encuntered in the building withut a valid badge will be escrted ff the prperty. Repeated vilatin f these prcedures can lead t barring the ffender permanently frm entering the prperty. Usage f trash chutes is strictly prhibited and any cntractr fund using the chute will be escrted ff the premises and denied future access. 4 RELEASE, INDEMNIFICATION AND HOLD HARMLESS AGREEMENT Muran Grande at Prtfin Cndminium Assciatin, Inc. This Release, Indemnificatin and Hld Harmless Agreement ( Release ) is executed this day f, 20 by the undersigned Owner(s) r Lessee(s) f Unit lcated in the Muran Grande Cndminium Assciatin. Whereas, the Assciatin will permit the undersigned t engage cntractrs and vendrs (including all thse wrking by, thrugh, r under them, the Persnnel ) t perfrm wrk within the undersigned s unit subject t the terms and cnditins set frth hereinafter. The cntractr must submit a current certificate f insurance f general liability insurance with limits f at least $1,000, and the MURANO GRANDE CONDOMINIUM ASSOCIATION, INC. as an additinal named insured, a current certificate f applicable Wrker s Cmp Insurance will be required and a cpy f applicable licenses and required permits. Nw, Therefre, in cnsideratin fr being permitted the benefits f allwing the Persnnel t perfrm wrk within the undersigned s Unit and ther gd and valuable cnsideratin, the receipt and sufficiency f which are hereby acknwledged, the undersigned specifically agrees t the fllwing: The abve recitals are true and crrect and are incrprated herein by reference. The undersigned acknwledge that the Wrk perfrmed by such Persnnel within their Unit shall be at the undersigned s sle risk and the Assciatin shall nt have any respnsibilities r liability fr the Wrk perfrmed by such Persnnel and further acknwledge and agree that the Assciatin has made n representatins regarding the Persnnel s ability r qualificatins t perfrm the Wrk. 6 Welcme t the Muran Grande CONTRACTOR RULES AND REGULATIONS We are pleased that yu have been selected t wrk n unit #. It is ur gal t help yu and the resident with a smth delivery f yur services. It is very imprtant that everybdy ding wrk at Muran Grande cmplies with the fllwing Rules and Regulatins in rder t make everyne s experience as pleasant as pssible. HOURS OF OPERATION 1. Cntractr hurs fr all the building are 8:30 AM t 4:30 PM Mnday thrugh Friday. Latest time t be allwed in is 3:00 pm. ELEVATORS 2. The Assciatin des nt reserve elevatrs fr cntractr wrk and/r deliveries. Elevatrs are used in a first cme, first serve basis. 3. Cntractrs must use the designated (padded) elevatrs nly. INSURANCE 4. All cntractrs must be licensed and insured. 5. Yu must prvide a certificate f insurance fr each unit that yu wrk n. TURNAROUND TIME FOR CONDOMINIUM APPROVAL AND QUESTIONS 6. Letters f Apprval frm the Cnd Assciatin will be issued after all paper wrk has been submitted, 3 business days fr flring wrk, and 5 business days fr architectural mdificatins. 7. All questins regarding cnstructin wrk must be addressed t the Management Office, r ffice@murangrande.inf. PERMITS 8. Permits are required fr flring, plumbing, electrical wrk and demlitin. Fr infrmatin n pulling a permit at the City f Miami Beach call: (305) T pull a permit at the City f Miami Beach, it is required t present a Letter f Recrdatin and a cpy f the Warranty Deed. The letter is prvided by the unit wner s clsing agent. RECEIVING DOCK, ACCESS & PARKING PROCEDURES 10. All cntractrs must register in the receiving area. 11. A cntractr wh is nt listed n the unit access authrizatin frm will be denied access. 12. Muran Grande Management will nt pen unit fr cntractrs, unit wner must prvide key and fb. 13. Cntractrs and wrkmen must fllw parking prcedures as directed by Muran Grande staff. Until further ntice, the building cannt prvide parking fr cntractrs; they need t unlad their materials in the receiving area and find parking in the street. We aplgize fr the incnvenience but this is the nly way the building can finish and clean the garage area. 14. Crews must use the designated elevatrs nly. IMPORTANT INFORMATION ON SOUND AND WATER PROOFING Balcnies must have bth sundprfing & waterprfing. The sundprfing rating required is STC/IIC 55/55. Please be advised that we d nt have a sund rated ceiling, drp ceiling, r ceiling assembly.

7 Yu must prvide the specificatins f the sundprfing yu will be using reflecting this infrmatin. DEPOSIT, FINES, INSPECCTION, AND WORKING RULES 15. $ depsit This will cver fr damages t cmmn areas (elevatrs, trash chutes, hallways, etc.). There are abslutely n exceptins. Checks are the nly payment methd accepted. Checks shuld be made payable t: Muran Grande at Prtfin Cndminium Assciatin, Inc. Once the inspectin has been apprved, a check request will be filed fr the refund. Be aware that this prcess may take up t 30 wrking days. Refund f depsit will be based n cmpliance with Muran Grande at Prtfin Rules and Regulatins. 16. Wet saws fr tile must be used in a tub inside the unit. 17. Abslutely n refuse, water, r debris is t be left in hallways, stairwell r any ther cmmn area f the building. 18. All wrk must be cmpleted inside the unit: n wrk is t be dne in hallways, stairwells r balcnies. FINE: $ per day. 19. Abslutely n refuse r debris is t be left n balcnies edge.
