Jint Hlders - We have received several inquiries regarding cmpleting the frms where there are tw r mre jint hlders. In this case, include all jint hlders as the Registered Sharehlder. All jint hlders must sign the frm. Only ne frm needs t be cmpleted t retract the jint psitin.

2 It is the Registered Sharehlder s respnsibility t crrectly cmplete and deliver a retractin request. Fr a retractin request t be eligible the belw items must be prperly cmpleted when submitting ne s retractin request. Each Registered Sharehlder in CareVest Mrtgage Investment Crpratin must cmplete a separate frm. If yu hld shares bth directly and thrugh an intermediary, frms will need t be cmpleted fr each psitin. If yu are the beneficial wner cntact yur brker, trustee, financial institutin r ther nminee s they can submit a retractin request. The fllwing must be cmpleted: 1) Registered Sharehlder Must be cmpleted with the exact name f the Registered Sharehlder. If yu hld yur shares thrugh a brker, trustee, financial institutin r ther nminee, (fr example, shares are held in an RRSP, TFSA, RRIF, LIFA, Brkerage Accunt hlding Nn-Registered shares, etc), then yu are a BENEFICIAL OWNER and nt the Registered Sharehlder. The name f the Registered Sharehlder is the nly ne listed in ur recrds. Yu will need t cntact yur intermediary t instruct them n yur behalf. Clients f Olympia Trust Cmpany shuld review item 6 belw. 2) Indicate All r A Specified Whle Number f Shares Must clearly indicate whether requesting all shares fr retractin r a partial number f shares. Fr partial retractin, ne must clearly dente the exact whle number f shares being requested. Dllar values r fractinal share amunts will nt be accepted. Only ne ptin can be requested. 3) Signed by Registered Sharehlder(s) each Registered Sharehlder must sign and print their name. If yu hld yur shares thrugh a brker, trustee, financial institutin r ther nminee, (fr example, shares are held in an RRSP, TFSA, RRIF, LIFA, Brkerage Accunt hlding Nn-Registered shares, etc), then yu are a BENEFICIAL OWNER and nt the Registered Sharehlder. We are required t take instructins frm the Registered Sharehlder nly, and the Registered Sharehlder must sign the retractin request. If the Registered Sharehlder is a crpratin, we will require an riginal crprate reslutin dated within the last 6 mnths. Sample reslutins are available n the website. As we require an riginal cpy f this dcument, it must be delivered t ur ffice by mail, curier, r hand delivery during r prir t the retractin ntice perid. 4) Physical Share Certificates - Mst sharehlders are recrded electrnically in ur direct registratin system, but if at sme time since January 1, 2013 yu requested physical share certificates fr yur hldings, thse physical share certificates must be attached t the retractin request r alternatively the share certificate(s) may be delivered t us prir t the retractin ntice perid fr cancellatin. 5) Retractin Request Delivery - In accrdance with the share terms, t be eligible fr retractin during the next retractin date, the retractin request must be received nly during the Mnthly Retractin Ntice Perid: Frm - the 10th day f each mnth (ther than December and January) Until - 4:00 p.m. (Calgary time) n the 15th day f such mnth, r the immediately preceding business day in the event that such 15th day is nt a business day. - Any Retractin Requests received utside f the Mnthly Retractin Ntice Perid will NOT be valid and cannt be acted upn. - Fully executed Retractin Requests must be submitted t Carecana Management Crp., Suite 1800, 555-4th Avenue SW, Calgary, Alberta, T2P 3E7, Attn: Investr Relatins, r, Facsimile: (403) r retractin@carecanacrp.cm. Acceptable delivery methds include mail, curier, hand delivery, facsimile r scanned and ed PDF files. Any Retractin Requests delivered t any ther address r lcatin will nt be valid and will nt be acted upn. 6) Beneficial Sharehlders f Olympia Trust Cmpany (OTC) Please cntact OTC directly fr instructins t crrectly cmplete a retractin frm as requests submitted must prvide the prper name in which shares are held, the OTC accunt number fr the shares subject t retractin request, and the shares subject t the retractin request must be in the crrect number. Als failing t prvide a signed retractin frm in additin t the infrmatin nted abve will result in an invalid retractin request.

3 Payment Any payments f retractin prceeds will be made in accrdance with the banking instructins currently n file fr the Registered Sharehlder. If yu are currently enrlled in the Dividend Reinvestment Plan, yu must cmplete a Direct Depsit frm and prvide a vid cheque in rder t receive yur payments electrnically. Any shares surrendered fr retractin will autmatically be remved frm the Dividend Reinvestment Plan. A cpy f the Carecana Direct Depsit frm is available at the Carecana Management Crp. website under the Sharehlder Administratin sectin. Ntificatin Yu will nly be ntified f valid delivery and acceptance by March 29, 2019 thrugh the address entered n yur frm. We will NOT be able t cnfirm receipt f yur retractin frm prir t this date. Any requests fr cnfirmatin made t investr relatins prir t ntificatin will nt receive a respnse. Cmmn Errrs That Will Invalidate Retractin Requests It is the Registered Sharehlder s respnsibility t cmplete the frm accurately and deliver it in accrdance with the instructins prvided. In the past, CareVest Mrtgage Investment Crpratin has been unable t accept retractin requests fr ne r mre f the fllwing errrs. This is nt a cmplete list f all pssible errrs, but belw are sme f the mst cmmn examples. - Incrrect Registered Sharehlder Fr example, the request wuld be rejected if nly ne sharehlder was specified when the shares are held jintly by tw individuals. Bth hlders wuld need t be listed, and bth Registered Sharehlders wuld need t sign-ff. Fr example, the request wuld be rejected if the shares are held in an RRSP/RESP/TFSA r sme type f Brkerage Accunt where a trustee r anther institutin is the Registered Sharehlder, but they are nt identified n the Retractin Ntice frm. - Request signed by a Beneficial Owner, but nt signed by the Registered Sharehlder Fr example, the request wuld be rejected if the shares are held in an RRSP/RESP/TFSA r sme type f Brkerage Accunt where a trustee r anther institutin wh is the Registered Sharehlder has nt signed the request. - Frm submitted utside the ntice perid An therwise valid retractin request submitted utside f the ntice perid will be rejected. Fr example, the request wuld be rejected if it is submitted in January prir t the pening f the February ntice perid r after the ntice perid has clsed. - Frm delivered t a lcatin ther than the specified lcatin r acceptable delivery methd(s) nted n the retractin ntice frm - Signature des nt match ne n file r there is n signature supplied n the frm This includes signature(s) nt matching crprate reslutins fr a sharehlder wh is a crpratin. - Request t retract a dllar value rather than the number f shares (applicable nly fr partial retractin requests) - Where a physical share certificate exists, it is nt supplied alng with the retractin request This nly applies in the case where a physical share certificate has been issued. The MIC perates under a Direct Registratin System that allws its shares t be wned, reprted and transferred electrnically withut using a physical share certificate. Instead f receiving a physical share certificate t represent yur shares, yur shares are held in yur name and recrded electrnically in the bks and recrds f the MIC.

4 Hwever, sme sharehlders may have requested a physical share certificate representing any r all f their Class A shares. The sharehlder hlding the riginal physical share certificate is required t present the riginal physical share certificate t facilitate any dealings with yur shares, including a retractin request. Registered Sharehlders hlding physical share certificates cannt fax r a cpy f the share certificate in cnjunctin with submissin f a retractin request. Original share certificate must be presented with retractin requests if applicable. - Incrrect frm submitted Only the 2019 retractin frm supplied will be accepted. Previus frms will nt be accepted. Only the crrect frm fr the crrect MIC will be accepted. Please refer t yur mnthly Carecana statement t cnfirm which retractin frm must be submitted.
