1 Afghan Social Marketing Organization (ASMO) To: From: Offerors Afghan Social Marketing Organization (ASMO) Issuance Date: August, 23, 2016 Closing Date: September, 06, 2016 Subject Reference: Request for Quotations Procurement of Power Generator RFQ KAB-ASMO Background: The Afghan Social Marketing Organization (ASMO) is a social marketing and behavioral change communication organization established in August ASMO as a free-standing, not-for-profit Afghan organization aims to create positive health outcomes by giving Afghan households the information they need to make informed and healthy choices and expanding access to high quality health products through commercial markets. The Sustaining Health Outcomes through the Private Sector Plus (SHOPS Plus) project led by Abt Associates awarded Afghan Social Marketing Organization (ASMO) a 15-months project that improves the delivery of high quality family planning and maternal and child health products to target populations in Afghanistan through the private sector. SHOPS Plus will provide technical assistance to ASMO in designing and implementing high quality social marketing interventions which leads to increased access to affordable priority health products and increased demand for priority health products and services. In addition, SHOPS Plus will collaborate with ASMO in developing a vision and roadmap for ASMO sustainability. Offer: ASMO would like to invite individuals/companies to submit quotation as detailed in section A for supply of Power Generator per details in section C. Under this guidelines, bidder/competitors will be required to offer their best and final prices. Submission guideline: Proposal/quotes must be printed copy written in English and sealed in envelop and to send no later than Sep to: Mr Khalid Ahmadzai, Procurement Manager ASMO. Submission in the form of electronic copy will not be accepted.

This means that person who handle the proposal should indicate to us at the gate that he/she has brought proposal and it should be dropped into the appropriate box available in the building entrance in ground floor. 3 Required Attachment: Appendix A: Bid Details Appendix B: Summary of Relevant Capability, Experience and Past Performance Appendix C Copy of Business License SECTION A QUOTATION NOTE: Individuals/company who wish to participate in bidding process should quote their Best and Final Offer (BAFO) for supply of power generator as detailed in C in this RFQ. Quote information should be clear, comparable and understandable. Quotes MUST provide following: - Price currency MUST be in USD (Us Dollar) and it should include cost of handling (full transportation/delivery cost) to the place buyer. (generator room ASMO main office) - Technical specification will include advanced feature based on the latest industry technology. Technical specification relevant to the performance, controlling, security & safety and environmental factor should be provide as this is marking factor. (see evaluation criteria) - Per Appendix B, description of field of experience should be provided. Information provided will be cross checked independently. Field of experience is marking factor. (see evaluation criteria) - Unit price must be used ( price of one unit of item) - Consideration for volume discount, tax, ect will be irrelevant in comparison. If such consideration included in price, there shouldn t be any separate line for this that led to confusion and lack of clarity. - Properly dated, signed and stamped and printed in the company s formal letterhead Appendix A: Bid Details While prices are required to be quoted in the table provided in Appendix A in this RFQ, additionally quotes as detailed in section A in this RFQ must be provided as supplementary information. 4 The completion and submission to Afghan Social Marketing Organization of the above item will constitute a Quotation and will indicate the Offerors agreement to the terms and conditions in this RFQ and in any attachments hereto. Issuance of this RFQ does not commit Afghan Social Marketing Organization to make an award. Offerors are required to examine all instructions and the specifications contained in this Request for Quotation. FAILURE TO DO SO WILL BE AT THE OFFEROR S RISK. Appendix B: Summary of Relevant Capability, Experience and Past Performance Using the format provided in Appendix B, the vendor shall list no more than three to four (3-4) former and/or current clients with their most up-to-date contact information, to whom commodities or services similar to those requested under this RFQ have been delivered in Kabul or other areas in Afghanistan over the last three to four years. The vendor should also provide copies of reference letters or certificates of completion from listed clients if available. Afghan Social Marketing Organization reserves the right to independently verify all submitted information, letters, and certificates. Appendix C: Copy of Business License - ASMO will not award a purchase agreement, contract, or subcontract to an organization that fails to provide a current valid copy of its active business license from either the Afghanistan Investment Support Agency (AISA) or the Ministry of Commerce. Business License must be attached to Appendix C. 5 SECTION D COMPLIANCE NOTE: D.1.Executive Order on Terrorism Financing U.S. Executive Orders and U.S. law prohibits transactions with and the provision of resources and support to, individuals and organizations associated with terrorism. It is the legal responsibility of the Offerors to ensure compliance with these Executive Order and laws. D.2Communications with USAID and Other Agencies All of Subcontractor s contractual written or oral communications with or to USAID, or local agencies relative to the Work under this Purchase Agreement must be through or with the authorization of ASMO Executive Director. D.3Language Requirement All submissions shall be provided in English. D.4 Restriction on Certain Foreign Purchases: USAID has eligibility rules concerning goods and commodities, commodity- related services, and suppliers of goods and services (other than commodity-related services). These rules are set forth. Any material that would be required for development of 10 video spots for five priority health messages in two local languages must not have originated and/or Sourced from Cuba, Iraq, Iran, Laos, Libya, North Korea, and Syria. D.5 Terms of Payment: Payment will be made in Afghani (AFN) according to rate of exchange per Da Afghanistan Bank. Afghan Social Marketing Organization will pay the vendor in accordance with the payment schedule, to be stipulated in the Purchase Agreement, after receipt and acceptance of the required deliverables. D.6 Tax Withholding Purchase to the provisions of the Government of Afghanistan Income Tax Lax 2010, Article 72.4 effective May 2010, ASMO is required to withhold income tax at the prescribed rate at the time of making payment to the legal or natural person(s) providing supplies, materials, construction and services under contract, including purchase order procurement, for transactions of AFA 500,000 and above. D.6.1 Withholding Rate The prescribed rate of income tax withholding is 2% of the gross purchase amount for contractors/vendors who have a valid business license to operate under Government of Afghanistan.

6 D.6.2 Remittance of Tax and Certificate of Withholding As the tax withholding entity, Afghan Social Marketing Organization is required to remit the income tax amount withheld directly to the Ministry of Finance designated account by 10th day of the month following the transaction. Afghan Social Marketing Organization will issue an income tax withholding certificate to the contractor/vendor listing the gross payment, the amount of income tax withheld and the net contract payment along with a copy of the payment form and deposit receipt for transfer made to the designated Ministry of Finance account.
