The new employee benefits information, enrollment forms, physician s directories and other pertinent information are located on our website. Instructions for accessing the website and form completion are as follows. PLEASE NOTE: EMPLOYEES WHO ARE TRANSFERRING FROM ANOTHER GEORIGA SCHOOL DISTRICT WILL CONTINUE THE OPTION CHOSEN DURING THE 2017 OPEN ENROLLMENT. BOOKLETS ARE AVAILABLE ONLINE FOR YOUR REVIEW. OTHER OPTIONS MAY BE AVAILABLE DURING OPEN ENROLLMENT, OCTOBER - NOVEMBER 2017, WITH A COVERAGE DATE EFFECTIVE 1/1/2018. Click on Employee Benefit Center The Dawson County Schools Employee Benefits Portal will open. There are 3 buttons. Watch the 2 videos: a) Green Start Here For Information for supplemental benefits. Click on the Video Help to information on the supplemental benefits. b) Red State Health Plan Click on the Video Help to information on the state health benefits. c) Blue Enroll Here After viewing the 2 videos, click on Start Here For Information You are now in the Dawson County Schools Benefits Center. Click on: a) I m New b) New Employee c) Follow and complete Steps One -Five. USING THE COVERAGE SELECTION/WAIVER FORM AS A GUIDE: COMPLETE THE STEPS BELOW UNDER THE EMPLOYEE BENEFITS MENU ITEM AT THE TOP OF YOUR SCREEN. Help Me Choose My Benefits - provides differing scenarios for employees to review to assist them in making their benefit choices. Long-Term Disability - The board of education provides this benefit at no charge to each full-time employee. You will be automatically enrolled in this benefit by your benefit representative. Voluntary Term Life Insurance - Complete the enrollment form for the amount of life insurance coverage desired. Whole Life Insurance - Complete the enrollment form for the amount of life insurance coverage desired. Contact the Benefits Call Center at Alexander & Co. at for rates. Dental Plan - Complete the enrollment section to enroll, or if you decline coverage, complete the declination section of the form. Vision Insurance - Complete the enrollment section to enroll, or if you decline coverage complete the declination section of the form. Critical Illness Insurance - Contact the Benefits Call Center at Alexander & Co. at to enroll. Flexible Spending Plans Complete the enrollment form for the plan(s) of your choice. (Medical and/or Dependent child care). There is a $3.50 per month charge for the FlexCard on the Medical reimbursement plan. Retirement Plans Employee must participate in either TRS or PSERS based on their job classification AND the 403(b) plan. Participation is mandatory. a) TRS/PSERS The State retirement plans are based on job classification either Teacher s Retirement or Public School Employees Retirement. b) 403(b) Plan - This is a mandatory deduction. Complete the Mid-America Salary Reduction Agreement form and print a copy of the Vanguard confirmation of account setup. Page5

These rates have already been reduced by $76.10 per month as a benefit for working for the Dawson County Board of Education. o Complete the SHBP Insurance Form to indicate your health insurance election (New Enrollment, Decline Coverage, or Transfer Coverage). NOTE: If tobacco surcharge applies, an additional $80 will be added to the SHBP Rates. 17 FY18 EMPLOYEE BENEFITS AT A GLANCE Board Paid Medical $76.10/month toward employee s premium 403b Contribution 5% of salary Long Term Disability Begins 90 days after the onset of your disabling injury or illness. Pays 60% of your before-tax monthly earnings. Employee Paid Medical Premium based on the plan chosen 403b Contribution o 1% of salary for bus drivers, custodians, paraprofessionals, and school food service personnel. These employee categories must pay into Social Security. o 2% of salary all other. These employee categories do NOT pay into Social Security. Critical Illness Dental Life Insurance Term and Whole Reimbursement Accounts Health and Dependent Child Care (with flexcard) Sick Bank Membership donation of 1 day to the bank. Vision Notes Benefits are available for positions that are 50% or greater. With employment on the first working day of the month, benefits will be effective the following month. Direct Deposit is a safe, easy and efficient banking method for your payroll funds. Direct Deposit is posted electronically to the employee s account showing as available funds on the payroll date.
