5 What s changing for 2017 Medical Annual Maximum for Health Savings Account (HSA) increasing by $50 for single coverage. $3,400 Single $6,750 Family (no change) If you apply and qualify for the Essential Assist Plan, the HRA contributions by Trinity Health are increasing to $850 single and $1,700 family Revised Live Your Whole Life program Dental - Incentive amounts will be based separately for participation from colleague and spouse/eligible adult Both plans will cover ceramic fillings and porcelain crowns on posterior teeth Both plans will cover fluoride treatment up to age 19 (previously age 14) Complete your online enrollment through Workday. 5

9 Enrollment timing You will be able to enroll in the health and welfare benefits for 2017 from: Wednesday Oct. 26 through Wednesday Through Nov. 9, 2016 Wednesday, October 26 Wednesday, November 9, 2016 Required documentation due by November 11 9

10 Why you must enroll If you don t make benefit elections during open enrollment, you will receive a default package of benefits which includes the same elections you had last year for: Medical Dental Vision Life insurance and AD&D Legal The following contributions or elections will not carry forward and you must go online to make a new election. Health Savings Accounts Flexible Spending Accounts - (medical and dependent care) PTO Cash out (non-exempt or hourly colleagues only) Keep in Mind When you enroll new dependents, you will need to provide Social Security numbers for any dependent age one year and over. 10

12 Trinity Health benefits: Benefit eligibility Who is eligible? Dependents who are eligible for benefits include: Eligible adult Spouse, as defined by the IRS (pre-tax) Other eligible adult who shares a permanent residence and is financially interdependent to the colleague Your and/or eligible adult's natural children, legally adopted children or children for whom you/eligible adult are the court-appointed guardian Eligible through the end of the plan year in which they turn 26 or after if they meet certain disability criteria before their 26 th birthday Eligible regardless of marital status, student status, residency or financial dependency Eligible adult may not be covered as a colleague and as a dependent Eligible children may only be covered under one colleague parent Note: Children of eligible adults may be covered only if their eligible adult is covered 12

14 Trinity Health benefits: Three medical plans Three medical plans give colleagues options to meet their personal family needs Traditional PPO Pay more out of each paycheck, but less at the time of service Choose this plan if you are interested in lower costs at the time you use the insurance Health Savings PPO (HDHP with HSA) Pay less each paycheck, but more at the time of service until you meet your deductible Receive annual seed money in an HSA based on enrollment tier Choose this plan to contribute to the HSA, and maximize your tax advantage as you save for current and future health care expenses Essential PPO (HDHP; Assist plan with HRA if qualify) Pay the least amount out of each paycheck, but more at the time of service Essential PPO Assist plan, including HRA with annual seed money based on enrollment tier, available for colleagues who meet certain income requirements Choose this plan if you are interested in lower payroll contributions 14

15 Three medical network tiers Each plan offers three network Tiers Each Tier offers choice in where to receive care You receive the highest level of benefits when you use the Tier 1 network and the lowest level benefits when you use the Tier 3 network To find providers and determine tiers, go to: click on the provider search link, then get started. Enter the name and location of your provider and go to plans accepted to see which tier your provider is in TIER 1 TIER 2 TIER 3 Trinity Health network facilities and aligned providers highest benefit BCBSM in-network providers All other providers (out-of-network) lowest benefit 15

16 Health Savings Account (HSA) Health Savings PPO Included with Health Savings PPO medical plan Use it to pay for current or future health care costs HSAs are a great way to save on taxes: No taxes on the amount contributed through payroll deductions No taxes on the amount withdrawn for qualified expenses No taxes on the interest earned in the account (up to amounts set by federal law) Trinity Health will make a full contribution to HSAs in January based on the coverage level elected Any money not used during the year is carried over, without any limits Change the amount contributed through payroll deductions at any time during the plan year HSA money is yours to keep and can be used for health care expenses even if you change jobs or retire HSA 16

17 Essential PPO Assist with HRA Essential PPO Assist medical plan for colleagues who meet certain income requirements You must apply to enroll in this plan and the application period is only available during open enrollment Applications due no later than Nov. 11, scan application and to the Associate Resource Center at: If you apply and qualify, you will be enrolled in the Essential PPO Assist plan, and Trinity Health will make a full contribution to a Health Reimbursement Account an HRA in January based on your coverage level Use it to pay for current or future medical costs that occur within the plan funds are automatically withdrawn as you use your medical benefits; there is no need to make a payment and submit a claim for reimbursement Any HRA money not used during the year is carried over as long as you meet the requirements and remain enrolled in this plan the next plan year You cannot contribute to the HRA Any remaining money in the HRA when you leave Trinity Health is forfeited Essential PPO Assist HRA 17
