1 How to get to Gold Vitality status To reach Gold Vitality status, you need to do certain activities to find out more about your health and how to improve it. There isn t a set order to do these activities in. You can do them in any order you want. However, this is the recommended path you can take on your journey.

VHR Points Vitality Health Review Complete the Vitality Health Review. Earn Vitality points. Online Fitness Assessment Complete the Online Fitness Assessment. Earn Vitality points. Non - smokers declaration Complete the non-smokers declaration. Earn Vitality points. Mental Wellbeing Assessments Complete the Mental Wellbeing Assessments. Earn up to Vitality points. Kid s Vitality Health Review Complete the Kids Vitality Health Review*. Earn points. * Applicable to members with child dependants (younger than 18 years)

3 Step 2: Learn about your health The next step is to learn more about your health status at pharmacies, dietitians and biokineticists in the Vitality Wellness Network. You can earn up to points. Vitality Health Check Complete blood glucose test, blood pressure test, cholesterol test and weight assessment at a Vitality Wellness Network pharmacy near you. Earn up to Vitality points. Vitality Fitness Assessment Complete the Vitality Fitness Assessment at a biokineticist near you. Earn Vitality points (you can earn this twice a year). Vitality Nutrition Assessment Complete the Vitality Nutrition Assessment at a dietitian near you. Earn Vitality points.
