3 Eligible Participant Requirements and Enrollment Process: Employees can enroll at any 24 Hour Fitness location or enroll online at and use CORP ID All employees may enroll during the one-year term of this agreement and must provide proof of employment upon enrollment. District participants must be at least 18 years old or 12 with parent. Monthly dues must be paid by electronic funds transfer with first and last months dues per person paid at time of enrollment. Add-on monthly dues must be debited from the same account as the primary member with the same access level. There is a maximum of six family add-ons per primary member. For employees who are current members with higher monthly dues for similar privileges, they may reduce their dues to the corporate rate at any time during this one-year agreement by contacting Member Services or the Corporate Account Manager and providing proof of employment. District participant must purchase membership to receive health and fitness consultation. You must be a current District participant of a LAUSD and eligible under LAUSD guidelines to enroll under a corporate membership program. By enrolling under a corporate membership program, you grant 24 Hour the right to verify your eligibility under LAUSD guidelines (including the right to verify your employment with LAUSD as necessary). In the event you are not currently employed or cease being a current employee of a participating company, 24 Hour reserves the right to immediately terminate your membership and/or require you to pay the full regular price of initiation fees and monthly dues. The annual fee due date is in accordance with the membership enrollment date, as long as the membership in effect. 24 Hour Fitness s membership agreement provides for an annual five-percent (5%) increase in monthly dues and annual fees for all of its members. The annual fees increase will occur only once in a calendar year; rates specified above will not change during the term of this agreement. Check-In Process: District participant members must check in at the front desk. You have two options for checking-in to the club, either by (1) enrolling in the Cardless Check-in system which allows you to access the club with a finger scan and a 10-digit check-in code (such as your phone number), or (2) bringing in an original photo ID*. You are not required to enroll in the Cardless Check-in system, but if you choose to not do so you will need an original photo ID* to access your club. *Photo IDs can be an original driver's license, state ID, student ID, military ID or passport. Fitness Facility Commitment to District Participants 24 Hour Fitness USA, Inc will provide District participants and family add-ons special corporate rates that will allow access to more than 400 clubs across the country. District participants will have a dedicated Account Manager to serve as their single point of contact for questions about the program along with local club support. DISTRICT PARTICIPANTS WILL RECEIVE THE FOLLOWING WITH THEIR CORP. MEMBERSHIP: No Initiation fee and low monthly dues for your District participants and add-ons.
