GENERAL MEMBERSHIP INFORMATION First year of membership is for one (1) year, unless noted; thereafter, membership is monthly. New Members are charged the full annual fee. Membership fees are non-refundable and non- transferable. Membership is required for participation in all activities unless otherwise stated. Membership cards are necessary to gain access to the J Sports & Fitness Complex and the Goldberg Family Early Childhood Center. New cards are made at the Front Desk free of charge upon joining. Please stop by the Front Desk or the Sports Desk to have ID picture taken for card. Cards available for pick-up 48 hours after picture taken. There is a $15 replacement fee for a new card. For your security and for registration purposes, please carry your membership card at all times. *Registration fees are applied to all new Members and to former Members who have not renewed and must be paid at the time of joining (not included in the total annual fee). GUEST POLICY We honor memberships from other JCCs. Bring your current membership card to the Welcome Desk to verify your membership status. Most Centers offer reciprocity to our Members. Out of town visitors may come to the J as often as he or she wishes as a paying guest of a Dallas Member. Dallas area residents may be a guest of a J Member four times during the year. The daily guest fee for ages 16 and over is $10 and $5 for guests under 16 years old. (the J) Participant N ame: I n considerati on of being allowed to participate in any way in any program, related events and activities at the J or sponsored by the J ( programs) I on my own behalf and on behalf of any minor children for whom I am responsible a ppreciate and agree that: 1. The risk of injury from the activities involved in programs off fered by the J may be significant, including the potential for permanent injury or even more serious consequences, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist, and; 2. 5 Aaron Family Jewish Community Center Application for electronic Fund Transfer What is the AFJCC electronic transfer plan? This convenience plan provides a way to budget your annual AFJCC membership fees on a monthly basis. With your authorization, the membership fees are deducted monthly from an account of your choosing. Who is eligible for the electronic transfer plan? Any adult, 18 years of age and older, who has an account (checking, credit, debit) at a participating financial institution. how do i sign up? By completing this authorization form and returning it along with a voided check or credit card number and your AFJCC membership application form. The first month s dues plus any application registration fees are paid up front. The check MUST be preprinted with a member s name on it. We will then complete our verification process and issue your AFJCC membership card(s). TeRmS AND CoNDiTioNS 1. I understand that if I wish to terminate after a minimum of 12 months or change my membership in any way, I must give the AFJCC a 30-day written notice. If I wish to terminate before my first 12 months, there will be a $150 buyout fee due to the AFJCC. 2. The AFJCC Board may, at its discretion, adjust the monthly rate applicable to my membership category at time of renewal. I understand that I will receive at least four weeks notice prior to any such change in my membership fees. 3. Should any membership deduction not be honored by my bank for any reason, I realize that I am still responsible for the payment, plus a service charge of no more than $25 applied by the AFJCC. This is in addition to any service fee my bank may make. I understand that it is my responsibility to notify the AFJCC in writing should I change my financial institution and/or account at any time.
