REVISED APPENDIX "C"- PRICE FORM: Please refer to Appendix "C"-Price Form of the RFQ and replace it with the attached Appendix C-Price Form- REVISED ADDENDUM 2-OCTOBER 18, 2017 reflecting changes in Column (D), as well as formatting updates. Failure to return your Bid in the Appendix C-Price Form- REVISED ADDENDUM 2-OCTOBER 18, 2017 included in this Addendum may result in your Bid being declared Non-Compliant. II. QUESTIONS AND ANSWERS: Q1. I have a question about the column Individual Units per Box (D). Is the number that goes in this space the units per bag or per case. A1. Column (D) is the number of individual units per package. Please see the attached REVISED APPENDIX "C" PRICE FORM. Please be advised that there may be orders where a Client requires three months' worth of supplies and therefore may require cases to be delivered. All other terms and conditions remain the same. Please attach this addendum to your Request for Quotation document and be governed accordingly.
