2 Achieve Your Best Health in Benefits and Wellness Checklist Review your benefits confirmation statement. In mid-december, you should have received a statement confirming your 2019 benefits coverage elections. If you have not already done so, it s important that you review your confirmation to ensure that it correctly reflects your dependents, address and benefit elections. You should also confirm that required documentation is on file for any new dependents. If you think there are errors on your confirmation statement, please contact the Benefits Service Center at Review your paychecks. Ensure your benefits deductions are being properly administered. Contact the Benefits Service Center at if you have questions. The start of a new year is the perfect time to renew your focus on what matters most. And this year, it s all about caring for yourself because when we care for ourselves, we can better care for others as a result. We strive to provide you with a variety of tools and resources that enable you to be your best at home and at work. As we work together to continue building healthy communities, it s important that you remain mindful of your own health. Take time to think about your wellness goals and how you ll prioritize You in Review your beneficiaries. Make sure your beneficiary designations are current for your life insurance through the HealthySteps Benefits Portal and your Retirement Savings Plan through Fidelity ( Check your mail for important tax documents. You will receive your W-2 in the mail from the hospital by January 31, 2019, and your 1095-C form by March 1, Don t lose money. If you had a Flexible Spending Account (FSA), Dependent Care Flexible Spending Account, or Health Reimbursement Account (HRA) in 2018, you must submit all claims for the 2018 calendar year by March 15, This money is subject to the Use It or Lose It rule, meaning all unused funds will be forfeited. For account balance, claims submittal or a list of eligible expenses, visit learn.healthequity.com/shclpch or call Make your appointment. Schedule and complete your biometric screening through the Hospital or your Primary Care Physician to earn credit for the HealthySteps to Wellness program. See page 4 for more details. Check in on. Once you ve completed your biometric screening, schedule your preventive care visits, including annual dental and vision appointments. Sign up for a gym membership. The Stanford Recreation Center is now offering discounted pricing on passes for all Hospital employees. Visit one of the many facilities to purchase a pass from the service desk and reach out to recreation@stanford.edu with any questions. There are also discounts available for many other local gyms. 3 2019 HealthySteps to Wellness Program Start off the New Year on a path to a healthier lifestyle. The HealthySteps to Wellness Program is designed to help you improve and maintain good health through free and discounted wellness tools. And for eligible employees, by participating in certain activities, you can earn a wellness incentive of $100 to $1,000! All activities must be completed by September 30, 2019, in order to earn your incentive. What Do I Need to Do? Step 1 Step 2 Know Your Numbers Complete 3 required activities to earn $250 (employee-only) or $500 (employee + one or more covered dependent) and advance to Step 2: Biometrics* Health Assessment Tobacco Use Statement** Take Action Earn 250 points and receive either a $250 (employee-only) or $500 (employee + one or more covered dependent) incentive. Weight Management Program (Omada) points Healthy Living or Fitness Class (HIP) - 50 points Seminar or Event - 50 points and more... Biometric Screening Events Free biometric screenings will be offered to eligible employees* between the months of January and March through HealthySteps to Wellness. You can also choose to complete your screening later in the year through your primary care physician. Just be sure to accomplish this before the program deadline, September 30, For a complete list of screening dates or to make an appointment, visit wellness.healthysteps4u.org. Walk-ins are also welcome on all event dates. All points must be earned by completing the necessary steps before the incentive is paid. Incentives are deposited into your Health Savings Account (HSA) if you are enrolled in the Aetna Choice POS II Plan. If you are enrolled in the Stanford Health Care Alliance (SHCA) or Kaiser Permanente HMO plans, or if you are enrolled in the Aetna Choice POS II plan but are not eligible for the HSA, your incentives will be deposited into a Health Reimbursement Account (HRA). Incentive deposited by: Q1: January 1 March 31, 2019 May 31, 2019 Q2: April 1 June 30, 2019 August 31, 2019 Q3: July 1 September 30, 2019 November 30, 2019 Aetna Choice POS II: Funds deposited to HSA/HRA Kaiser Permanente HMO: Funds deposited to HRA Funds in your Health Equity HSA or HRA may be used to pay for IRS-qualified health care expenses. Your incentive will be based on your medical enrollment status (employee-only or employee + 1 or more dependents) as of the last day of each quarter. To earn your incentive, you must be enrolled in a hospital-sponsored medical plan and be an active employee at the time funds are deposited, or the funds will be forfeited. The Stanford Children s Health Center for Healthy Weight offers free classes teaching healthy habits for overweight children ages 8 15 and their families through their Pediatric Weight Control Program. This six-month educational program promotes healthy habits for the whole family, including: Identifying and avoiding high-calorie, low-nutrient foods Developing better exercise habits Reducing sedentary behaviors (activities that require little or no physical activity) Following a healthy, balanced diet, even in difficult situations like family gatherings, holidays and parties Maintaining a healthy weight over the long-term To be eligible for the program, both you and your child(ren) must be enrolled in either the Aetna Choice POS II Plan or Stanford Health Care Alliance (SHCA) Plan. HealthySteps to Wellness participants can earn 100 points for completing the challenge. WELLNESS SPEAKER SERIES: FOOD FADS OPTIMIZING WELLNESS THROUGH FOOD AND NUTRITION February 14 12pm - 1pm Lucile Packard Children s Hospital Auditorium, Room 180 Which diet is more effective, a healthy low-fat or healthy low-carb diet? Is there truth in current food fads? Christopher Gardner, PhD, will debunk common diet myths and share how to optimize wellness through nutrition. HealthySteps to Wellness participants earn 25 points for attending this event in person, via WebEx, or by viewing a video recording after event. 6 Understanding Your Plan Coverage To learn more about your selected plans, visit or call the Benefits Service Center at (Monday Friday, 5 a.m. 5 p.m. PST) It s important to understand the details of your benefit plans before you go to the doctor. Being prepared in this way can save you both time and money. As a reminder, here s a look at how the Hospital s benefit plans have changed since last year. New for the Aetna Choice POS II Plan Mental health and substance abuse benefits are now being managed by Aetna, providing you with access to a larger group of medical professionals and facilities. As a reminder, there are no changes to the benefits available or how much you pay for services. If you are undergoing treatment that has extended into 2019 with a provider who is not part of the Aetna network, you may be able to complete your treatment with your current Optum provider. They will need to submit a Transition of Care form to Aetna. Once it s approved, care can continue for 90 days while you transition to a provider in the Aetna network. There are several updates to your 2019 CVS/caremark pharmacy benefits, one of which lowers the cost of your maintenance medications. If you take a medication regularly, you ll save money by receiving a 90-day supply. You have the option to fill a 90-day supply of maintenance medication through mail order or by picking it up at your local CVS Pharmacy (including those inside Target stores). You should have received a letter from CVS with instructions on how to set up your 90-day fills. Please note that this Maintenance Choice program automatically applies unless you opt out. HealthySteps Mobile App Make the most of your benefits in Never be without your plan information with the HealthySteps mobile app. Did You Know? The app allows you to store your benefit ID cards, easily find contact information for your providers and plans, access the Stanford Health Care events calendar and Favorite regularly used providers to quickly access their information or make contact. New for the Stanford Health Care Alliance (SHCA) Plan The SHCA Plan network of providers and facilities has expanded to provide more options for those enrolled. For more information about providers and facilities included in the network, please call CareCounsel at or search the provider directory at stanfordhealthcarealliance.org. For those who access services within the five core counties (Alameda, Contra Costa, San Francisco, San Mateo and Santa Clara counties), the SHCA network of providers and facilities applies. Brown & Toland physicians in San Francisco and the northern peninsula cities are now part of the SHCA network, as well as Washington Township Medical Foundation physicians in the Fremont/Newark region.
