Rate no 1 Diesel Driven Vehicle hired charges for providing Indica Vista Car (Diesel) driven model-2016 for NADIAD CIRCLE office for 12 Hrs. a day, incl. all labor, operating cost, consumable fuel, & lubricants, repairs, maintenance, incidental cost and other expenses complete in all respect up to 3000 km monthly minimum run, for 1 (One) years. 2 Rate for the extra KMs run over 3000 K.M. 3 Overtime Rate per Hours after 12 HRS up to 2HRS per Day 1Month Per Km Per Hours 4 Night Hault Charges Per Night N. 4 Sign of contractor SUPERINTENDING ENGINEER (O&M) MGVCL, C.O. NADIAD TERMS & CONDITONS FOR HIRE VEHICAL 1.PERIOD OF CONTRACT. The contract period is for one years for 12 hours daily with weekly off. Also MGVCL reserves the right to terminate or extend the contract at any time without giving any reasons. 2. CONTRACT VALUE. Value of order is for 12 hours daily monthly run up to 3000 K.M. for the period of one year. As per schedule A attached herewith. 3. HIRING CHARGES PAYABLE FOR TAXI PASSING CAR (DIESEL) Vehicle hire charges payable for TAXI PASSING CAR VEHICLE MODEL 2016 monthly fix charges as per your written consent given. i. e. your negotiation rate for 12 hours per day and up to 3000 k.m. Per month. 4. SECURITY DEPOSIT. You shall have to pay security deposit 5% of order value as above 2.5 % in cash and 2.5 % deducted from the 1 st R. A. bill at Nadiad (O&M) Circle with in 07 days on receipt of this order. If you will discontinue the contract S.D. will be forfeited. 5. PENALTY. MGVCL shall have right to hire vehicle from other agency at your risk and cost in case of default of vehicle. In absence of vehicle for complete short of the day at Rs.900/-per day will be charged as a penalty and the amount of regular bill will be deducted for that day. The Driver should be informed to bring lunch box compulsorily, and 30 minutes recess for lunch allowed. Further you shall be responsible for payment of any charges, fine, or penalty, if imposed by an authority for non observation of statutory rules and regulation of R.T.O. & Police. 6. AGREEMENT AND INDEMNUITY BOND. You shall have to execute an agreement on a stamp paper of Rs.100/- and indemnity bond on a stamp paper of Rs.100/- at your cost. As per provision of vehicle before commencement of the contract. 7. VEHICLE MODEL ACCEPTANCE IN CONTRACT. Vehicle is one number of TAXI PASSING CAR (DIESEL) VEHICLE MODEL 2016 is accepted in the contract. Commencing from the work order issued, Document for

5 the same R/C Book comprehensive Insurance certificate etc., may please be submitted in this office as required. 8. AREA OF WORK. Vehicle is to be provided for normally the entire jurisdiction of Nadiad (O&M) Circle and if required out of Nadiad within limit of Gujarat State. 9. MAINTENANCE OF VEHICLE. Vehicle should be keep in good working condition you are fully responsible in any case of accident of vehicle, your driver and MGVCL s staff, so far that you shall be required to maintain vehicle in up to date condition in all respect to avoid an accident. In case of failure of vehicle you should make arrangement of another vehicle. 10. TIMING. At present you should provide your vehicle FOR 12 HOURS a Day strictly and above timing can be changed by the under signed if desire by the authority 11. PRICE VARIATION. If any rise / fall on price of fuel during validity of contract by Rs. 1/- the difference will be payable considering Rs ps. Per Km. for complete one rupee is admissible. For this you shall have to submit documentary evidence for price variation. This price variation is applicable for all KM run for MGVCLs work only. The statutory price variation i.e. the cost of oil, tyres, taxes, spares etc., will not be payable by the MGVCL (Basic Diesel Rate Rs per Ltr.) 12. INCOME TAX & ANY OTHER TAX Income tax as applicable will be deducted at source. 13. PAYMENT OF BILL. You will be required to maintain log book of vehicle for every day as per format. At the end of every day the log book must be complete and signed by the competent authority of the vehicle of MGVCL. And drive. At the end of the every month bill in triplicate along with the copy of the log book are to be submitted to the controlling officer of the vehicle in office for recording and for passing of the bill. The bill must be submitted before 5 th of each month. The Payment of Bill will be given within 30 days from the bill receive by circle office from Controlling officer of circle office. The payment of vehicle will be made for KM run made for MGVCLs work only. Any discrepancy there is to be refereed to the C.O. and the decision will be final and binding to the contractor. 14. ACCIDENT. In case of an accident to the vehicle, the contractor shall have to bear the cost of damage or compensation what so ever. MGVCL will have no liability in case of any accident to the vehicle, i.e. all liability will be remaining with contractor. 15. Termination of contract: -

6 In case of any of the term of the contract unsatisfactory service, the M.G.V.C.L. without prejudice to other clause of the contract may cancel the work order and contractor by giving a clear notice of not less than 15 days, In termination of contract on account of any breach of any condition or for other reasons the M.G.V.C.L. may forfeit S D and reserves the right for recovery for liquidated damages. In the event of any dispute under the contract the decision of M.G.V.C.L. will be final and bind to the contractor. 16. DOCUMENTS. Before putting the vehicle the following documents should be got certified from Sup. Engr. (O&M) Nadiad and attested zerox copies should be submitted to him. Comprehensive Insurance certificate. R/c book duly tax paid up to date. Road permit. Contract carriage permit. Ration card. Delivery note of vehicle No..form where the vehicle is purchased. Invoice of the said vehicle. Fitness certificate and any other relevant document relating to hire vehicle. Legal document of taxi passing for carriage contract. Driving license of concern driver and photograph. Owner has to submit under taking on Rs.100/-stamp paper if the vehicle should be driven by you or owner or the PF registration no if vehicle should be driven by other than Owner. 17. The driver provided for the above vehicle should have driving license issued by competent authority and having knowledge of road and minor repairing of vehicle. 18. In case of failure of Milo meter it should be replaced under intimation to this office immediately, the distance will be decided by the Su. Engr. /Ex. Engineer will be final. 19. All the dispute relating to this office order and during. The contract period will be subject to Nadiad Jurisdiction only. 20. Maintain of vehicle (allowed 12 Hours for month) Continuous availability of vehicle for all the days of a month. However vehicle goes out of the road due to break down beyond one s control to repair the same and again put on the road. In

7 such eventuality contractor may put up his effort to provide another vehicle to cope up the week. 21. A Log book should be maintained by driver with signature of concerned Engineer. 22. All the charges for diesel, oil, tyre, tax, maintenance, driver salary, etc, will be born contractor. 23 MGVCL have reserves the right to extend the contract for another year looking to the condition of the vehicle. 24 Driver must have to keep mobile in ON and working condition. 25 The Vehicle will be utilized for the staff as well for materials within jurisdiction of Nadiad (O&M) circle and for outside duties whenever necessary within Gujarat State. 26 Contractor has provided jeep Vehicle for seating arrangement to 3+1 seter In addition to the above other terms and condition of the tender as well as booklet of terms and condition also will be applicable.
