We are requesting your support for our 35th Annual ACOFP-CA Scientific Medical Seminar by displaying at the event in our Exhibit Hall. The date and location is August 11-14, 2011 at the Disneyland Hotel in Anaheim, CA. The Exhibit area is a 10ft area that will accommodate a national booth. We are expecting 350+ physicians of whom the majority will be Family Practice physicians. The Exhibit options are: 1. We have excellent attendance at our meetings averaging 350 physicians, mostly in family practice and primary medical practices, along with other specialties. The membership has enjoyed your previous participation, helping our organization to further educate California family physicians about new and existing medications and products, assisting them in continuing to provide state of the art science and treatments. Final space assignments will be sent to all exhibitors after July 15th, In the event of conflicts regarding space requests or conditions beyond our control we reserve the right to rearrange the floor plan. Exhibit needs to be dismantled Friday at 4:00 p.m. Exhibits should be staffed at all times during the hours of the show (except during lunch break, room will be locked). Removal of any equipment may be made only at the close of last day of show. If the exhibit booth is not staffed during the show hours ACOFP-CA. will not be held responsible for any exhibitor items. National was officially chartered April 4, 1950 in the state of California, the College was affiliated with the American Osteopathic Association in The ACOFP state organization was formed in 1976 and is continually growing in membership. To encourage and improve the educational opportunities for the training of family physicians in osteopathic medicine and surgery. What is the ACOFP of California Annual Meeting? The ACOFP of California Annual Meeting is one of the best osteopathic family physicians meetings in the state of California. Now in its 35 th year, the meeting will focus on continuing medical education with sessions on new advances and therapies prevailing in clinical treatment. 2

4 Who are the Attendees? Your message will reach osteopathic family physicians from throughout the state, as well as other states, who often attend our conventions. Primary practice physicians from major metropolitan areas, as well as rural communities gather to update their knowledge and make valuable contacts among the family practice community. Who Exhibits? Exhibitors provide attendees with information about diverse products and services from computer billing technology, technical communications, new pharmaceuticals, medical devices, medical publications and reference materials, physical therapy equipment and much more. Whatever products or services you have to provide to the osteopathic family physician, exhibiting at the ACOFP of California Annual Meeting will give you the opportunity to: 1. Discuss your message firsthand with physicians in a relaxed atmosphere. 2. Broaden your contacts with physicians and educators. 3. Increase your awareness of the 2011 issues facing the osteopathic community. Rates, Deposits & Refunds: A non-refundable deposit of $ must accompany all applications. All moneys paid will be retained by the ACOFP of California as liquidated damages for all cancellations received within sixty (60) days of the show opening date of Thursday, August 11, Notice of cancellation must be in writing. Subletting of Space: Exhibitors may not assign, sublet or apportion to others whole or any part of the space allocated and may not display goods or services other than those manufactured / regularly distributed by the exhibitor and approved by the ACOFP-CA. Refusal of Exhibits: The ACOFP-CA reserves the right to refuse application of those not meeting the standards required or expected, as well as the right to curtail or to close exhibits that reflect unfavorably on the character of the meeting. Booth Construction & Arrangements Exhibits table is a 6 foot skirted table in the front, with the ability for you to place a backdrop setup for your company name. There will be two chairs and a wastebasket. The exhibit hall is carpeted. There is a daily pickup fo waste. All exposed parts of any display must be finished so as not to be objectionable to other exhibitors of the ACOFP-CA. All materials used in the exhibit must be flameproof in accordance with Fire Department regulations. All electrical wiring must conform to local codes and regulations. Equipment, products or materials to be shown or demonstrated must be placed in the exhibit space contracted for in order to ensure that viewing the display or equipment will be in the booth and not impeding aisle traffic. Care of exhibit space: The exhibitor must, at his/ her expense, maintain & keep exhibit and the space contracted for in clean and good order, in accordance with these rules/ regulations & all other applicable ordinances. Flammable materials: Volatile or flammable fluids, substances or material of any nature prohibited by fire regulations or insurance carriers may not be used in any way. Attached is the application for electrical outlets. 3

5 Conduct: The following practices are prohibited: A. Noisy electrical or other mechanical apparatus interfering with other exhibitor s space. B. Operation of X-ray equipment. C. Canvassing or distributing any material outside the exhibitor s own space. D. Subleasing of exhibit space. E. Soliciting participation or otherwise harassing convention attendees. F. Publicizing and/or maintaining any extracurricular activities, inducements, demonstrations or displays away from the exhibit areas during the exhibit hours. Advertising by exhibitors must be confined to the assigned booth space. While promotional give-a-ways and/or drawings are permitted and encouraged, no exchange of currency is permitted. We will provide sign-in sheets and address labels to facilitate your obtaining attendees contact information during convention hours, if so desired. Exhibition Fee: The fee is $1500 for a basic booth, $2500 for a premier booth located by food court and microphone. Exibit map will be attached. Set-Up / Removal: by August 11, 2011, 9 a.m. (if no special arrangements have been made) may be reassigned by ACOFP of CA (see page 2 of this information package). Dismantling of booths will commence on Friday after the exhibit closes. Security & Indemnification: No professional security will be provided for the exhibit hall during the conference. It is the responsibility of each exhibitor to ensure the security of his or her own property. This contract includes a clause of indemnity. Payment & Cancellation: Please see page 3 of this information package for our payment and cancellation policies. Meals: Exhibitors are invited to participate in all meal functions taking place in the exhibit hall. Assignment of Space: Please see page 2 of this information package for our space assignment policies. We will do whatever we can to accommodate your first choice. Electrical Outlets: Make sure to check the applicable box if you need a power outlet for your display. Additional charges may apply. Badges: We will provide official show badges for exhibitors, which permit entry in to the exhibit hall. 6 Exhibit Hall Indemnification Agreement Exhibitor will defend, hold harmless, and indemnify from and against all liability cost and expenses, including reasonable attorney fees, resulting from claims of injury - to persons or damages to property arising out of the sole negligence or intentional acts of the ACOFP of California and its affiliates, employees, officers, directors, agents or invitees. ACCEPTED: Signature Company Date Exhibitor hereby assumes entire responsibility and hereby agrees to protect, defend, indemnify and save Disneyland Hotel, it s owners/operators and each of their respective parents, subsidiaries, affiliates, employees, officers, directors, and agents harmless against all claims, losses or damages to persons or property, governmental charges or fines and attorney s fees arising out of or caused by its installation, removal, maintenance, occupancy or use of the exhibition premises or a part thereof, excluding any such liability caused by the sole gross negligence of the Hotel and its employees and agents. Exhibitor shall obtain and keep in force during the term of the installation and use of the exhibit premises policies of Comprehensive General Liability Insurance and Contractual Liability Insurance, insuring in an amount not less than $2,000,000 combines single limit for personal injury and property damage. The hotel, it s owners/ operators shall be included in such polices as additional named insured. In addition the exhibitor acknowledges that neither the Hotel nor its owners/ operators maintain insurance covering exhibitor s property and that it is the sole responsibility of the exhibitor to obtain business interruption and property damage insurance insuring any losses by the exhibitor.
