2 OVERVIEW The American College of Radiology (ACR) is pleased to present the ACR Annual MRI Conference, chaired by Diego Martin, Professor of Medicine and Head, Department of Medical Imaging at the University of Arizona College of Medicine. This three-day conference will look at the cutting-edge methods and most talked about topics in full body MRI. Faculty experts will reveal the unique insights and practical insights that radiologists need to know in order to advance clinical skills, provide optimal patient care and stay competitive in the current health care environment. Silver Package $2,500 The Silver Package contains ample space for a 6-foot display PLUS a choice of one event sponsorship including: Thursday Breaks (2) Friday Breakfast (1) Friday Breaks (2) Saturday Breakfast and Morning Break Gold Package $5,500 The Gold Package provides your company with a 6-foot exhibit area PLUS one Luncheon Product Theater. This unique opportunity allows your company a speaking opportunity described below. Three (3) Gold Packages are available.

3 PRODUCT THEATER OPPORTUNITIES The Product Theater is an opportunity to provide valuable information and insight into a particular issue or trend, to provide a case study for solution that your company supported. This program allows you a thirty-five minute presentation and 10 minutes for Q&A. Basic A/V is provided (Screen, projector, lap-top, and sound system). EXHIBITOR BENEFITS Exhibit space during the event (two (2) attendees per company). Additional attendees charged at $295 each 100-word company description in the conference materials Access to educational sessions (no CME) Participation in the continental breakfasts, breaks and luncheons (all food events will be held in the exhibit area) EXHIBITOR REGISTRATION Exhibitor registration is being handled exclusively by ACR. Exhibitor attendees must be registered and paid prior to the event start times and must be wearing an official exhibitor badge. Displays may not be assembled during exhibit hall hours and may not be broken down early. You are solely responsible for the set-up and break down of your exhibit display. EXHIBITOR SERVICE KIT An exhibitor service kit containing order forms and prices for onsite services including shipping, storage, electrical, Internet and other services will be sent to each exhibitor upon registration. For your convenience, ACR staff will be available for support and connection to hotel and convention service personnel. Reservation Deadlines: The ACR has reserved sleeping rooms for meeting attendees. Rooms are available on a first-come, first-served basis, and the ACR cannot guarantee that rooms will be available after the reservation deadline of June 1, Reservations made after June 1, 2014, will be accepted on a space-available basis and may not be at the ACR room rate.

4 INDUSTRY GUIDELINES It is the responsibility of exhibitors to ensure adherence to Food and Drug Administration (FDA) regulations, policies, practices and guidelines, and all other applicable industry guidelines (including, but not limited to, those listed below), concerning the demonstration, discussion, use and/or display of products, technologies and/or services at the ACR Annual MRI Conference. In addition, ACR encourages all exhibitors to check with their companies own medical affairs/compliance staff for guidelines adhered to by their own companies. Additional information about industry guidelines should be obtained from the organizations directly. Accreditation Council for Continuing Medical Education (ACCME) Policies on Commercial Support and Disclosure Advanced Medical Technology Association (AdvaMed) Code of Ethics on Interactions with Health Care Professionals American Medical Association (AMA) Opinion Gifts to Physicians from Industry Council of Medical Specialty Societies (CMSS) Code for Interactions with Companies Food and Drug Administration (FDA) Guidance Documents for FDA-Regulated Products Pharmaceutical Research and Manufacturers of America (PhRMA) Code on Interactions with Healthcare Professionals EXHIBITOR SHOW REGULATIONS 1. Exhibit space will be allocated on a first-paid, first-served basis. Priority will be determined by date of receipt of application for space accompanied by required payment. 2. Application for space must be accompanied by the full fee in order to guarantee reservation. Exhibiting/sponsoring company will remain responsible for payment of space. Terms are enforced regardless if sponsor/exhibitor does not attend, fails or elects not to participate in conference, or if exhibit space is resold due to notification of absence. No refunds. 3. All tabletops must fit within the confines of their assigned space so as not to impede traffic flow, infringe on the space of other exhibitors or violate the emergency exit routes set forth by the fire marshal. 4. No exhibitor shall assign, sublet, or share the whole or any part of the space contracted to them. Exhibitors agree to limit the service and/or materials displayed in their exhibit area to those purveyed by the exhibitor, with the exception that other proprietary equipment may be used solely for the purpose of demonstrating the materials or services of the exhibitor. 6. ACR reserves the right to impose limitation on noise levels and any other method of operation that becomes objectionable. Any distribution of literature or samples shall be limited to the exhibitor s booth. Exhibitors may not interfere with collateral that is not produced by said exhibitor. Violation of this will result in cancellation of exhibit space during the show without refund. 7. ACR reserves the right to rearrange the final floor plan based on unsold or unoccupied exhibit space. In said event, all exhibitors will be notified. 8. If ACR should be prevented from holding the exhibition for any reason beyond ACR s control (such as, but not limited to, damage to the building, riots, strikes, acts of government, or acts of God) or if an exhibitor cannot occupy the assigned exhibit space due to reasons beyond ACR s control, then ACR has the right to cancel the exhibition or any part thereof, with no further liability to the exhibitor other than a refund of exhibit space fee, less a proportionate share of the exposition cost incurred. 9. Neither the management of the host facility nor ACR shall be liable for the damage, loss, or destruction to the exhibits by reason of fire, theft, accident, or other destructive causes. Exhibitor shall lease booth(s) at his/her sole risk. Neither the management of the host facility, ACR, nor any of its agents, servants, or employees will be accountable or liable for accidents to exhibitors, their agents, or employees. 10. The exhibitor shall be liable to the host facility and/or ACR for any damage to the building and/or the furniture and fixtures contained therein, which shall occur through acts or omissions of the exhibitor. 11. Exhibitor assumes the entire responsibility and hereby agrees to protect, indemnify, defend, and hold harmless ACR, the host facility, their officers, employees, and agents against all claims, losses, and damages to persons and property, governmental charges or fines, and attorney s fees arising out of or caused by exhibitors installation, removal, maintenance, occupancy, or use of the exhibition premises or any part thereof, including any outside exhibit areas. 12. Exhibitor acknowledges that ACR does not maintain and is not responsible for obtaining insurance covering exhibitor s property. Exhibitors are advised to obtain business interruption and property damage and loss insurance to cover such occurrences. 13. The exhibitor assumes the entire responsibility and liability for losses, damages, and claims arising out of exhibitor s activities on the Hotel premises and will indemnify, defend, and hold harmless the Hotel, its owner, and its management company, as well as their respective agents, servants, and employees from any and all such losses, damages, and claims.
