3 79th ANNUAL MEETING This year's Annual Meeting will be held at the Embassy Suites Columbus Airport, 2886 Airport Drive, Columbus, OH, 43219, April 30 May 1, Educational programs are scheduled as well as exhibits. This meeting provides a forum for OSHP business, current pharmacy practice issues, continuing education for pharmacists and technicians, and social events that draw the greatest number of health-systems pharmacists in Ohio each year. The schedule integrates exhibits with other meeting activities to maximize traffic in the exhibit area and interaction with the attendees. The exhibit session is scheduled during lunch on April 30. THE EMBASSY SUITES COLUMBUS AIRPORT The Embassy Suites Columbus Airport is located at 2886 Airport Drive. From I-670 take the Cassady Avenue Exit and turn west onto Airport Drive. The hotel will be on the right. ROOM RESERVATIONS Room reservations can be made by contacting the hotel directly at (614) Please identify that you are attending the OSHP meeting to receive a special $ per night rate. After April 16, 2018, the hotel will make reservations at their regular rate on a space available basis. EXHIBIT PROGRAM HOURS Monday, April 30, :00 a.m. 11:00 a.m. Exhibitor Set up 11:00 a.m. - 11:45 a.m. Lunch served for exhibitors 12:00 p.m. - 2:00 p.m. Exhibits Open (lunch for attendees served in exhibit area) 2:00 p.m. Exhibit Tear Down EXHIBIT LOCATION Exhibits will be located in the ballroom/ foyer area of the Embassy Suites. Location of exhibit tables is at the sole discretion of OSHP and will be preassigned. Applicants may request not to be located directly adjacent to two other companies of their choice. These requests will be honored on a first come, first serve basis as space allows. EXHIBITOR CONTRACT An exhibit contract is included on the last page. The exhibit fee for the Annual Meeting is $1,500. OSHP's tax ID number is The cost of booth space includes: promotion and publicity; general illumination; one 6' draped table and 2 chairs. Complimentary registration for two booth personnel is included: additional registrations are available. Each person staffing a booth must be registered and wear a nametag at all times on the exhibit floor. The cost of the booth space does not include lodging. See ROOM RESERVATIONS section for more information. Exhibitors will be responsible for their own set up and tear down arrangements. Exhibitors may bring their own table signs (these will not be provided) and skirting if desired, provided such decorations do not intrude on adjoining exhibits. All electrical requirements are to be noted on the Contract for Exhibit form. Exhibit materials may be shipped 48 hours prior to the meeting. 4 SPONSORSHIPS In addition to exhibiting at the Annual Meeting, companies have the opportunity to participate in OSHP activities through program sponsorship. Funds will be used for speaker and meeting expenses. Estimated fees are as follows: Luncheon $4,000 Reception $1,000 Dinner event $6,000 Sponsorships are separate contributions from the Exhibit program. ADDITIONAL EDUCATIONAL PROGRAMS OSHP strives to provide quality educational programs focusing on current issues in institutional pharmacy practice. We are seeking potential sponsors for one day or half-day educational programs. Companies wishing to co-sponsor a program with OSHP should contact Educational Affairs Director Jessica Winter. CORPORATE OR INSTITUTIONAL PARTNERSHIP Exhibit booth rental is complimentary for Corporate or Institutional Partners of OSHP. A completed contract is still required for our Partners exhibit space. For an annual commitment starting as low as $2,000, your partnership will provide you with two or more complimentary OSHP memberships, complimentary exhibit booth rental, recognition and/or advertising in the OSHP Bulletin and website, and other benefits. 5 EXHIBIT RULES AND REGULATIONS 1. Show Sponsorship and Management These meetings are produced by and are the property of the Ohio Society of Health-System Pharmacists, hereinafter referred to as OSHP. OSHP will provide all show management functions and establish all show policies. Each prospective exhibitor is required to sign the Application and Contract for space. By doing so, he subscribes to the General Information, Rules and Regulations, which are a part of the Application and Contract. 2. Space Allowance and Selection Procedure Space will be assigned on a first come basis, based on when the payment or purchase order is received. OSHP will allocate space from that which is available and the exhibitor agrees to accept such assignment. 3. Cancellation or Withdrawal An exhibitor may cancel or withdraw from the such subject to the following conditions: (a) The exhibitor shall give OSHP notice in writing of his intention to cancel or withdraw from the show; (b) In the event said notice is received by OSHP on or before 30 days prior to the show, the exhibitor shall receive his fee refunded in full; (c) In the event the said notice is received 29 days or less prior to the show, exhibitor is obligated and agrees to pay said space for its own convenience including selling the space to another exhibitor without any rebate or allowance to the canceled exhibitor. OSHP assumes no responsibility for having included the name of the canceled exhibitor or description of his products in programs, news releases, or other materials. 4. Default or Occupancy Any exhibitor failing to occupy by 11 AM the day of the Annual Meeting show, space contracted for but not canceled as outlined above, is obligated to pay the full cost of such space. In the event, OSHP shall have the right to use said space for its own convenience, including selling the space to another exhibitor, without any rebate or allowance to the defaulting exhibitor. OSHP assumes no responsibility for having included the name of the defaulting exhibitor or description of his products in programs, news releases, or other materials. 5. Building Occupancy In case the premises of the Annual Meeting should be destroyed or damaged, or if the show fails to take place as scheduled, or is interrupted and or discontinued or access to the premises is prevented or interrupted with by reasons of strike, lockout, injunction, act of war, act of God, emergency declared by the governmental agency or by OSHP, or for any other reason, this contract may be terminated by OSHP, In the event of such termination, the exhibitor waives any and all damages and claims for damages and agrees that the sole liability of OSHP shall be to return to each exhibitor his space payment less his pro rata share of all costs and expenses incurred and committed by OSHP. 6. Exhibit Hours Each exhibitor is required to keep at least one attendant in the booth during show hours. No one will be permitted in any exhibitor s booth at any time during installation or dismantling of the show or after closing of the show unless he positively identifies himself as an employee or authorized representative of the exhibitor. The exhibitor agrees to have an authorized representative present at the exposition throughout all exhibit periods and during the period of installing and dismantling the exhibit, who shall be responsible to the Exhibit Manager. The exhibitor will notify the Exhibit Manager of the name of the authorized representative(s) no later than April 20, Booth Arrangement All exhibitors shall arrange their displays so they utilize only the booth area contracted for and in such a manner as to recognize the right of other exhibitors and show visitors and to conform to the overall pattern developed by OSHP. Once a booth assignment has been made, changes will not occur unless approved by the OSHP Exhibit Manager. All equipment and exhibit materials must remain in the booth area. Items are not permitted in the aisle areas. OSHP reserves the right to restrict the use of glaring or irregular lighting effects. The use of flashing electric signs or lights is not permitted. An exhibitor is responsible for keeping his booth sightly. Exhibitor must arrange to remove excessive amounts of trash or waste materials during the show. Exhibitors shall provide needed storage within their own booths.

6 The use of moving pictures will be permitted, subject to approval by and under the control of OSHP. Sound movies will be permitted only if unobjectionable to neighboring exhibitors. Plans for all projections must be submitted to OSHP six weeks prior to the show. Machines which create excessive noise, making normal conversation in neighboring booths difficult, may, at the discretion of OSHP be restricted to periodic operation. Exhibitor assumes responsibility for compliance with hotel requirements relating to projection equipment and sound system. 8. Delivery and Removal During Show Under no circumstances will the delivery or removal of any portion of an exhibit be permitted during the show without permission first being secured in writing from OSHP. 9. Safety, Fire and Health The exhibitor must comply with safety, fire and health ordinances regarding installation and operation of equipment. All displays, drugs, exhibit materials, and equipment must be reasonably located and protected by safeguards and fireproofing to prevent fire hazards and accidents. 10. Care of the Building No exhibitor may allow an article to be brought into or permit any act to be done in the hotel which will increase the premiums or void policies of insurance held by OSHP. No exhibitor may permit any act by its employees by which the premises shall in any manner be marred or defaced. Exhibitor must surrender the space occupied in the same condition as at the commencement of the occupancy. Any damages done to the premises by the exhibitor shall be made good to OSHP or the hotel as their interest may appear. 11. Security OSHP and the hotel will not be liable for damage or losses to exhibitor s property through theft, fire, accident, or any other cause. Exhibitor should insure his own exhibit and display materials. OSHP will not assume liability for any injury that may occur to show visitors, exhibitors, or other agents or employees or exhibitors. It is the exhibitor s responsibility to secure all drugs displayed during the open hours and when the exhibit area is closed. 12 Exhibitor Badges OSHP will provide badges for all exhibitor personnel. 13. Liability In the event the OSHP shall be held liable for any situation which might result from a particular exhibitor s action or failure to act in any manner whatsoever, such exhibitor shall reimburse OSHP and hold OSHP harmless from liability resulting therefrom. 14. General All matters and questions not covered by the Contract, Rules and Regulations, are subject to the decision of the OSHP Exhibit Manager. The Contract, Rules and Regulations may be amended at any time by the OSHP, and all amendments or additions that may be so made shall be equally binding on all parties affected as the original contract. 15. Hospitality Suites Hospitality suites shall not be scheduled concurrently with any other meeting event other than the Exhibit Show. 16. Right of Refusal OSHP has the right to refuse exhibit space to any applicant for any reason.
