2 CHECK LIST Please note that as of 4 th of October all Change requests should only be submitted via IATA customer portal at Please note that in case you are not a portal user ( you need to register and then use the dedicated service IATA Accreditation and Changes. Change of Ownership a) Sale of an IATA Location to an Outside Party If this represents a complete change of ownership of the Agency then the following documents are required. Notice of Change Form signed by authorized person of the Agency. Application Form for Approval as an IATA Passenger Sales Agent, Answer all the questions fully and legibly, and do not forget that an authorized party must sign and date the application. Please ensure that full names of owners and shareholders are entered in Section 4. A copy of the latest Balance sheet and Profit and Loss Account within six Official certificate of company trade registration. Letter of appointment or copy of the contract signed with the GDS. Certificate of Insurance against insolvency in conformity with Art. 36 Photos of the interior and exterior of the Travel Agency location. Sample of Travel Agency s letterhead. b) Sale of an IATA Location to another accredited Agent Notice of Change Form signed by authorized person of the Agency. Official certificate of company trade registration. Letter of appointment or copy of the contract signed with the GDS. 3 Change of Shareholding Instructions and special requirements in order to notify IATA of a Change of Shareholding. a) Change of more than 30% of Shares Notice of Change Form signed by authorized person of the Agency. Copy of the legal acts which certify the change. Official certificate of company trade registration. Certificate of Insurance against insolvency in conformity with Art. 36 A copy of the latest Balance sheet and Profit and Loss Account within six b) Change of less than 30% of Shares Notice of Change Form signed by authorised person of the Agency. Change of Legal Status Instructions and special requirements in order to notify IATA of a Change of Legal Status Notice of Change Form signed by authorised person of the Agency submitted seven days before the change is to be effected. A copy of the latest Balance sheet and Profit and Loss Account within six Certificate of Insurance against insolvency in conformity with Art. 36 Official certificate of company registration.

4 Change of Name Instructions and special requirements in order to notify IATA of a Change of Name Notice of Change Form signed by authorised person of the Agency submitted seven days before the change is to be effected. Official certificate of company registration under the new legal name. Bank Guarantee/Insurance Bond if agency holds an existing bond with IATA a new bond will be required under the new legal entity name (sample attached). Change of Location Notice of Change Form signed by authorised person of the Agency submitted seven days before the change is to be effected. Voluntary Relinquishment Voluntary Relinquishment Form signed by authorized person of the Agency. Please note that the closure will be performed the last day of the month following tue request.
