3 2015 Benefits Overview Montefiore Voluntary Tax Deferred Annuity 403(b) Plan The Montefiore Medical Center Voluntary Tax Deferred Annuity 403(b) Plan provides an excellent tool to help you save for your future financial security. Your before-tax contributions are automatically deducted from your paycheck and accumulate earnings on a tax-deferred basis. You make your own investment decisions based on your investment strategy and the level of risk you are willing to accept. You can change your allocations and transfer amounts among investment options. You are always 100% vested in your Montefiore Executive and under certain circumstances you may even make a withdrawal. Maximum contributions are determined by the IRS and are adjusted annually. If you are age 50 or older you can make an additional catch-up contribution. At any time during the year, to elect the Voluntary Tax Deferred Annuity 403(b) Plan, change your election and/or if you are eligible for and want to make a catch-up contribution, contact Principal at or go to 3 Automobile and Homeowners Insurance CorporateOffers 457(b) Retirement Plan If your annual base salary is greater than $150,000, Montefiore offers a nonqualified deferred compensation plan. This plan allows you to defer a portion of your compensation on a pre-tax basis and accumulate tax-deferred earnings. The maximum dollar limit you can defer is determined by the IRS and is adjusted annually. You are 100% vested in your deferrals and earnings. Payments begin when you retire or reach age 70 ½ and are paid in a lump sum or in annual installments over a maximum of 10 years. Your contributions are automatically deducted from your paycheck before taxes are calculated and withheld. This lowers your taxable income, so you save money on taxes! It s convenient and easy to use with online ordering and home delivery plus direct payment you don t have to wait for reimbursement. You create a profile and find travelers who have similar travel routes and patterns. Carpool/Vanpool share a ride with other commuters who have similar starting and destination locations. It can be an ideal commuting solution if you work somewhat far from home, have limited access to public transit or want to cut down on your driving. Bicycling or Walking Rideshare provides tools and services to help you make your walk or bicycle to work safe, convenient and enjoyable. Transit Itinerary Not sure how to get to work using transit? Looking for an alternate way to work? How can you get to work in a weather related emergency? Request Transit Trip Assistance and a Transit Trip Itinerary will be ed or mailed to you. 5 Insurance Benefits You have the opportunity to purchase individual insurance policies offered at group rates. Group Personal Excess Liability Insurance The Group Personal Excess Liability Program provides liability coverage for personal injury, bodily injury, or property damages you or a covered family member are liable for in excess of required underlying limits provided under your primary insurance policies. Through a Montefiore Group Policy, you can obtain higher limits of coverage and lower rates to protect your individual assets and wealth. Individual Disability (ID) Income Insurance Montefiore s group LTD program is designed to protect 60% of your total income. However, for some associates with higher incomes, the group LTD program provides less income replacement due to the monthly maximum benefit. This voluntary discounted program offers you the opportunity to close that gap and protect a larger portion of your total income. Maximum additional benefit amount up to $10,000/$15,000 per month (tax-free) based on the level of benefit provided under Montefiore s Group LTD Plan. The first $5,000 of this additional monthly benefit will be offered on a Standard Issue basis (no medical underwriting), for those actively at work. Coverage will be issued based on total income, other coverage in force, and Principal Life Insurance Company s issue & participation limits. Contact Joel A.
